(Tonify the Middle and Augment the Qi Decoction)加茯苓(Poria)。

The *Chanbao* (Treasures of Childbirth) states: Pregnancy edema and distension arise because the qi of the zang organs is originally weak, and childbirth causes extreme deficiency. Earth fails to control Water, blood scatters into the four limbs, leading to abdominal distension, generalized edema of the hands, feet, face, and eyes, along with scanty and difficult urination. Chen Wuyue said: Whenever a woman has pre-existing wind, cold, chill, or dampness pathogens, she tends to have swollen feet during pregnancy, commonly known as "wrinkled feet" (Zhoujiao). There are also cases of whole-body edema and distension with urgent fullness in the heart and abdomen, termed "fetal water" (Taishui).
It is stated that pregnant women should not allow their emotions to reach extremes. If the mind is calm and qi is harmonious, fetal qi will be stable. If exposed to wind-cold pathogens or other provocations, illness will follow the pathogen. During pregnancy, menstrual blood is retained to nourish the fetus; if sudden generalized edema occurs, it indicates water retained within the fetus, where water and blood contend. The Spleen and Stomach dislike dampness and govern the muscles of the body. Dampness infiltrates and weakens qi, causing muscle flaccidity, allowing water fluids to overflow, thus resulting in whole-body edema and distension. However, the etiology varies. Some cases result from diarrhea or dysentery, causing visceral laxity and depleting the Spleen and Stomach. Others result from chills, fever, malaria, restlessness, thirst, and excessive drinking, which dampens the Spleen and Stomach, all capable of causing edema of the head, face, hands, or feet. If water infiltrates the uterus, the fetus may be damaged before forming properly. When approaching delivery, slight swelling of the feet indicates that the uterine organ has less water and more blood, with water exiting outward, presenting as slight swelling, which facilitates easy delivery. Pre-existing cold pathogens, triggered by cold exposure, can cause abdominal distension and swelling.
The *Chanru Ji* (Collection on Childbirth and Nursing) states that after three months of gestation, when the fetus is formed, both feet gradually swell from the dorsum up to the calves and knees, making walking difficult, leading to asthma and chest oppression, and poor appetite, resembling water qi disease. If yellow fluid oozes from between the toes, it is called "child qi" (Ziqi), which subsides only after delivery. This is due to the woman's pre-existing wind pathogens or blood-wind in the Chong and Ren meridians. One should not rashly administer decoctions, especially if the condition is severe, for fear of exhausting energy during delivery and facing unforeseen dangers. Therefore, treatment is necessary before delivery. Ancient formulas rarely discuss this. According to the *Mingyi Lu* (Records of Famous Physicians), during the reign of Emperor Yuanhui of Song, the young emperor and Xu Wenbo practiced acupuncture incognito. Xu Wenbo saw a pregnant woman with swollen feet unable to walk. The emperor palpated her pulse and diagnosed a female fetus. Xu Wenbo diagnosed: "This is a male fetus; if on the left, the fetus is black." The emperor, angry, wanted to abort it surgically. Xu Wenbo compassionately said: "Let me treat it with acupuncture." The fetus was expelled, confirmed as male and black. This account of pregnancy foot edema appears in ancient texts. Currently, Chao Yuanfang's *Bingyuan* (Source of Diseases) only discusses "zi fan" (fetal restlessness). The *Qianjin Fang* (Thousand Gold Formulas) and *Chanbao* mention it briefly. Liu Yuxi's *Xu Zhuan Guangxin Fang* attributes fetal formation to water qi in pregnant women, and the *Taiping Shenghui Fang* also mentions it, but these views are incorrect. In the Yuanfeng era, Chen Jingchu of Huainan, a famous physician, had unique formulas for treating this disease. Initially named Xiangfu San, Li Boshi renamed it Tianxianteng San.
Xue Ji commented: For the aforementioned symptoms, if there is chest fullness, abdominal distension, urinary retention, and whole-body edema, use Likou Tang (Carp Decoction). If the Spleen and Stomach are weak, combine with Sijunzi Tang (Four Gentlemen Decoction). If the face and eyes are puffy and limbs feel like water, use Quansheng Baizhu San (Quansheng Atractylodes Macrocephala Powder); if ineffective, use Liujunzi Tang (Six Gentlemen Decoction). For Spleen deficiency with damp-heat causing lower body swelling, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) plus Fuling (Poria). If improper diet causes vomiting and diarrhea, use Liujunzi Tang. If legs and feet swell with asthma and chest oppression, or fluid oozes from finger webs, use Tianxianteng San (Aristolochiae Vine Decoction). If Spleen and Stomach are weak, combine with Sijunzi Tang; if ineffective, use Buzhong Yiqi Tang. If Spleen and Lung qi stagnation exists, use Jiaweigui Pi Tang (Modified Guipi Decoction) combined with Jiawei Xiaoyao San (Modified Xiaoyao Powder). One patient experienced limb fatigue and loss of taste at five months of each pregnancy, starting with bilateral foot swelling, spreading to the whole body and then the head and face. This indicated Spleen and Lung qi deficiency. She took Buzhong Yiqi Tang in the morning and Liujunzi Tang with Zisugeng (Perilla Stem) in the evening, resulting in recovery. In treating pregnancy, do not rigidly adhere to gestational months; instead, identify the meridian pattern and use corresponding herbs.
Another patient, aged thirty-eight, had pregnancy edema. She was treated with Likou Tang plus Wuling San (Five-Ingredient Powder with Poria) and Renshen (Ginseng Root). For heavy dampness, Cangzhu (Atractylodis Rhizoma) 1 qian, Houpo (Magnoliae Officinalis Cortex) and Chenpi (Citri Reticulatae Pericarpium) 0.5 qian were added, along with fried Lopizi (Radish Seed), Cheqianzi (Plantaginis Semen), and Huashi (Talcum) 1 qian each, prepared as one dose. For asthma and urgency, add Kutingli (Lepidii Semen). For difficult urination, add Mutong (Akebiae Caulis) and Dengcao (Juncus Effusus). For severe cases, use Cheqianzi and Jun Chuan San (Jun Chuan Powder) to eliminate damp-toxins naturally. Fangji (Stephaniae Tetrandrae Radix) treats damp-heat edema below the waist but is contraindicated in patients with internal injury and weak stomachs.
Tianxianteng San (Aristolochiae Vine Decoction):
Tianxianteng (washed, lightly stir-fried), Xiangfuzi (stir-fried), Chenpi, Gancao, Wuyao (preferably soft, white, and fragrant, regardless of origin).
Equal parts, finely powdered. Take 3 qian per dose with one large cup of water, three slices of ginger, three slices of papaya, and three leaves of Zisu (Perilla Leaf). Decoct to 70%, warm, clarify, and take on an empty stomach before meals, three times daily. When urination becomes smooth, qi and pulses flow freely, body feels light, and edema subsides, no further doses are needed. (At the end of the Yuanfeng era, Wang Anshi resided in Jinling. His family fell ill, and he wrote a poem to Jingchu: "The whole clan is poor and sick, troubling you with medicinal efficacy. What shall I send home? Ask the migrating geese." Thus, I learned this formula was passed down from Li Boshi's family and recorded at the residence of Vice Minister Zhang in Linchuan.)
