Danxi stated: The Wuji San (Five Accumulations Powder) from Ju Fang treats postpartum pain caused by residual blood. It uses Cangzhu (Atractylodis Rhizoma) as the sovereign herb, Mahuang (Ephedrae Herba) as the minister herb, Houpo (Magnoliae Officinalis Cortex) and Zhishi (Aurantii Fructus Immaturus) as the assistant herbs. Although it contains Dang Gui (Radix Angelicae Sinensis) and Shaoyao (Paeoniae Radix) for nourishing blood, their dosage is only one-third of that of Cangzhu. One must consider what cold pathogen a postpartum woman would have, given that her Qi and Blood are not yet replenished, making diaphoresis difficult. Even if one argues that the properties are mild and can eliminate stale matter to generate new tissue, how can one use the fierce nature of Mahuang (Ephedrae Herba) combined with the dispersing actions of Cangzhu (Atractylodis Rhizoma), Zhishi (Aurantii Fructus Immaturus), and Houpo (Magnoliae Officinalis Cortex)? This would cause deficiency upon deficiency, and disaster would follow immediately.
Appendix [Itching in the Intestines]
To treat unbearable itching in the intestines of women after childbirth: Place a needlework bag under the mattress where she sleeps, without letting others know. Another prescription: Take arrow bamboo and [missing character], place it under the sleeping mat, without informing the puerpera.
[Da Lun] Postpartum distension, fullness, and Qi-type pain in both hypochondriac regions are caused by retained water previously stored in the Bladder. Because lochia is not completely discharged after childbirth, water accumulates, creating a sensation of obstruction. Obstruction Qi clashes with Water, accumulating in the Bladder, thus causing distension and fullness in the hypochondriac and rib areas. The agitation between Qi and Water causes pain.
[Xue Ji] For the aforementioned syndrome, if it is Blood stasis in the Liver Channel, use Xuanhusuo San. If it is Qi stagnation in the Liver Channel, use Sijun Zi Tang (Four Gentlemen Decoction) with Qingpi (Tangerine Peel) and Chaihu (Bupleuri Radix). If it is Blood deficiency in the Liver Channel, use Siwu Tang (Four Substances Decoction) with Renshen (Ginseng Radix et Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), and Chaihu (Bupleuri Radix). If both Qi and Blood are deficient, use Bazhen Tang (Eight Treasures Decoction) with Chaihu (Bupleuri Radix). If Kidney Water is insufficient and cannot nourish Liver Wood, use Liuwei Wan (Six-Ingredient Pill with Rehmannia). If Lung Metal is overly vigorous and suppresses Liver Wood, use Xiebai San. Refer to the main treatments discussed previously. A puerpera experienced distending and painful hypochondriac regions due to anger, with severe hematemesis, fever, aversion to cold, and chest/abdominal distension and pain. I diagnosed this as deficiency of both Qi and Blood, and used Bazhen Tang (Eight Treasures Decoction) added with Chaihu (Bupleuri Radix), Danpi (Moutan Cortex), and Paojiang (Prepared Ginger). The bleeding stopped instantly. Then, using Shiquan Dabu Tang (Ten Great Tonifying Decoction), the chills and fever gradually subsided. In this case, unless pungent-warm herbs like Jiang (Zingiberis Rhizoma) and Gui (Cinnamomi Cortex) were used to assist the Spleen and Lung in promoting the action of the medicines, not only would the efficacy be lost, but the distension would be exacerbated.
