Postpartum panting and urgency arise from two causes: one is extreme Yin deficiency, and the other is cold pathogen in the Lung. Since the postpartum state is characterized by great deficiency, how can there be panting due to Qi excess? If panting and urgency occur without cold pathogen in the Lung, this is due to blood loss causing Yin fluid deficiency, leaving isolated Yang without a master. Consequently, Qi becomes exhausted, short, and floats upward in a state of collapse. This actually indicates that the Liver and Kidney Qi are not connecting, representing a sign of rootless Qi about to detach, which is a critical condition. The Classic states: "The Liver dislikes tension; urgently use sweet flavors to relax it." This precisely applies to such cases. Only Zhenyuan Yin (True Origin Decoction) is the miraculous formula for treating this. If Qi deficiency is accompanied by cold, Dabu Yuan Jian (Great Tonification of the Original Decoction) or Liyin Jian (Regulating Yin Decoction) should be used. If external wind-cold invades and the pathogen enters the Lung causing panting and urgency, this must present with coarse breathing, chest distension, or frequent coughing, which differs significantly in treatment from shortness of breath resembling panting with disconnected upper and lower Qi. The treatment should focus on dispersing pathogens while tonifying, using Jinshui Liu Jun Jian (Metal-Water Six Gentlemen Decoction) or Liujunzi Tang (Six Gentlemen Decoction). If it is solely due to cold pathogen entering the Lung, causing Qi excess and stagnation without underlying deficiency, Liu'an Jian (Six Peace Decoction) or Erchen Tang (Two Aged Decoction) added with Perillae Folium (Suye, 紫苏叶) should be used.
Zhenyuan Yin (See *New Formulas Eight Arrays · Tonifying Array*) treats shortness of breath resembling panting, urgent respiration, inability to lift Qi upward, inability to swallow downward, airway obstruction, and critical conditions nearing death. People generally know this as Qi urgency with pathology located above, but do not realize it is due to the Yuan Sea lacking roots, with Liver and Kidney depletion. This is a case of Zi-Wu non-intersection and Qi detachment syndrome. It is particularly common in women whose Blood Sea is chronically deficient. One should urgently use this decoction to rescue and relieve, daring to call it a miraculous formula. In diagnosing this condition, the pulse must be faint, fine, and spiritless. If faint and tight, it is especially fearsome. If ignorant practitioners mistakenly attribute it to Phlegm rebellion and Qi stagnation, using formulas containing Realgar (Niuhuang, 牛黄), Styrax (Suhe, 苏合香), Green Tangerine Peel (Qingpi, 青皮), Aged Tangerine Peel (Chenpi, 陈皮), or Aurantium Fructus (Zhike, 枳壳) to break Qi, they will hasten the patient's demise.
Prepared Rehmannia Root (Shudi, 熟地) (7-8 qian, up to 1-2 liang in severe cases)
Roasted Licorice Root (Zhigancao, 炙甘草) (1-3 qian)
Angelica Sinensis Root (Danggui, 当归) (2-3 qian)
Boil in 2 zhong of water until reduced to 0.8 zhong, take warm. If accompanied by vomiting/nausea or aversion to cold, add 3-5 slices of baked ginger. If Qi deficiency and pulse are extremely faint, urgently add Ginseng Radix et Rhizoma (Renshen, 人参) as appropriate. If Liver-Kidney Yin deficiency with cold extremities, add Cinnamon Bark (Rougui, 肉桂) 1 qian.
Dabu Yuan Jian (Great Tonification of the Original Decoction) (See Early Menstruation due to Blood Heat)
Liyin Jian (Regulating Yin Decoction) (See Late Menstruation due to Blood Cold)
Jinshui Liu Jun Jian (Metal-Water Six Gentlemen Decoction) (See *New Formulas Eight Arrays · Harmonizing Array*) treats Lung-Kidney deficiency-cold, water overflowing into Phlegm, or elderly Yin deficiency with insufficient Qi and Blood, combined with external wind-cold invasion, presenting with cough, vomiting/nausea, excessive phlegm, panting and urgency, etc., with miraculous effects.
Angelica Sinensis Root (Danggui, 当归) (2 qian)
Prepared Rehmannia Root (Shudi, 熟地) (3-5 qian)
Aged Tangerine Peel (Chenpi, 陈皮) (1.5 qian)
Pinellia Tuber (Banxia, 半夏) (2 qian)
Poria (Fuling, 茯苓) (2 qian)
Roasted Licorice Root (Zhigancao, 炙甘草) (1 qian)
Boil in 2 zhong of water with 3-7 slices of fresh ginger until reduced to 0.7-0.8 zhong, take warm well after meals. If loose stools and dampness prevail, remove Angelica Sinensis Root and add Chinese Yam (Shanyao, 山药). If excessive phlegm and Qi stagnation cause chest/hypochondriac discomfort, add White Mustard Seed (Baijiezhi, 白芥子) 0.7-0.8 fen. If severe Yin-cold persists with cough, add Asarum Root (Xixin, 细辛) 0.5-0.7 fen. If accompanied by exterior pathogen with chills and fever, add Bupleurum Root (Chaihu, 柴胡) 1-2 qian.
Liujunzi Tang (Six Gentlemen Decoction) (See Abdominal Pain during Menstruation)
Liu'an Jian (Six Peace Decoction) (See *New Formulas Eight Arrays · Harmonizing Array*) treats wind-cold cough, initial onset of non-stroke conditions, Phlegm stagnation, and Qi rebellion.
Aged Tangerine Peel (Chenpi, 陈皮) (1.5 qian)
Pinellia Tuber (Banxia, 半夏) (2-3 qian)
Poria (Fuling, 茯苓) (2 qian)
Licorice Root (Gancao, 甘草) (1 qian)
Apricot Seed (Xingren, 杏仁) (1 qian, peeled and tips removed)
White Mustard Seed (Baijiezhi, 白芥子) (0.5-0.7 fen, avoid in elderly with weak Qi)
Boil in 1.5 zhong of water with 3-7 slices of fresh ginger until reduced to 0.7 zhong, take well after meals. For external wind-pathogen cough with severe cold, which is difficult to disperse, add Northern Asarum Root (Beixixin, 北细辛) 0.7-0.8 fen or 1 qian. In severe winter cold with strong pathogens, Ephedra Herb (Mahuang, 麻黄) and Cinnamon Twig (Guizhi, 桂枝) may also be added. If wind predominates and pathogens are mild, add Saposhnikovia Root (Fangfeng, 防风) 1 qian, or Perillae Folium (Suye, 紫苏叶) is also acceptable. For headache and nasal congestion, Chuanxiong Rhizome (Chuanxiong, 川芎), Angelica Dahurica Root (Baizhi, 白芷), or Vitex Fruit (Manjingzi, 蔓荆子) are all suitable. If accompanied by chills and fever, add Bupleurum Root (Chaihu, 柴胡) and Perillae Folium (Suye, 紫苏叶). If wind-pathogen cough persists with Lung-Stomach fire, add Scutellaria Root (Huangqin, 黄芩) 1-2 qian; if severe, add Anemarrhena Rhizome (Zhimu, 知母) and Gypsum (Shigao, 石膏); only one slice of fresh ginger should be used. For cold-pathogen cough with difficult expectoration, add Angelica Sinensis Root (Danggui, 当归) 2-3 qian, especially suitable for the elderly. If Qi and Blood are insufficient, Jinshui Liu Jun Jian should be used in conjunction with this formula. For initial non-stroke conditions with excessive Phlegm and unsmooth Qi, add Agastache Herbs (Huoxiang, 藿香) 1.5 qian; if accompanied by distension, add Magnolia Bark (Houpo, 厚朴) 1 qian to temporarily open Phlegm-Qi, then observe cold/heat and deficiency/excess to tonify and regulate. This formula is contraindicated for sudden collapse due to Qi deficiency and for cases where Qi is calm and free of Phlegm.
