Niu Xi San (Hornbeam Root Powder): Indicated for retained placenta accompanied by abdominal distension (see *Jing Tong Lun Wai Fang*).
After delivery, when the placenta has been expelled and Qi and Blood are simultaneously depleted, sudden blackouts, dizziness, unconsciousness, clenched jaws, and coma may occur. Ancients often attributed this to lochia attacking upward due to deficiency, thus causing "Blood Fainting." However, one must understand that this condition is divided into two types: Blood Fainting (*Xue Yun*) and Qi Collapse (*Qi Tuo*). If Qi Collapse is mistaken for Blood Fainting and treated with acrid-aromatic blood-invigorating or phlegm-resolving formulas, death will ensue instantly; caution is imperative.
First, Qi Collapse (*Qi Tuo*): During childbirth, if there is massive hemorrhage, the loss of Blood leads to the loss of Qi, often resulting in fainting and unresponsiveness. Those with slight deficiency may recover after a short while; those with severe deficiency who suffer from exhaustion and collapse will die. Observation reveals signs such as pale complexion, closed eyes, open mouth, cold hands, and extremely fine pulses in all six positions. This indicates Qi Collapse. Immediately administer 1–2 liang of Ren Shen (Ginseng Radix et Rhizoma), decocted rapidly into a concentrated soup, and administered slowly. As long as it enters the throat, life can be saved. Any delay renders treatment futile. I have rescued several such patients with immediate recovery; this is the most critical method. I have also witnessed cases where patients died because Ginseng was prohibited. They claimed that Ginseng should not be used immediately postpartum as it would "trap" residual lochia, causing harm. Even when advised otherwise, they refused, leading to regretful deaths. Another erroneous belief states that Ginseng should only be used seven days after delivery. These ignorant rumors, whose origin is unknown, mislead people deeply and must never be believed.
Second, Blood Fainting (*Xue Yun*): Originally caused by Qi deficiency, leading to transient fainting. However, cases involving Blood stasis and excessive Phlegm do occur. If the patient’s constitution, breath, and pulse indicate excess (Shi syndrome), with chest/abdominal distension, pain, and upward rushing sensation, this is Blood Reversal (*Xue Ni*), indicated for Shi Yao San (Wu Tao San). If Phlegm is excessive and breathing is rough, Er Chen Tang (Two-Cured Decoction) is indicated. In the absence of distension, pain, or rough breathing, all cases belong to Qi deficiency, and large doses of Xiong Gui Tang (Ligusticum and Angelica Decoction) or Ba Zhen Tang (Eight-Treasure Decoction) should be primarily used.

In cases of sudden fainting where medication cannot be administered in time, heat a lead weight until red-hot, place it in a vessel near the bed, and pour vinegar over it, or apply vinegar to the nose and mouth. The sour aroma entering the nasal cavity helps restore consciousness. Alternatively, burn broken lacquerware or dry lacquer and use the smoke to fumigate the nose. Both methods are effective but are mild and suitable only for acute, sudden fainting. For fainting caused by severe Qi deficiency, only major tonifying formulas will be beneficial.
