Dushen Tang (Single Ginseng Decoction) Huisheng Dan (Resurrection Pill)
After a miscarriage, if there is sudden and profuse bleeding that does not stop, with sallow complexion and pale lips, use Dushen Tang (Single Ginseng Decoction). If lochia (stagnant blood) fails to discharge, causing distension and pain in the abdomen and hypochondrium, appropriately select Huisheng Dan (Resurrection Pill) or Yimu Wan (Motherwort Pill) for treatment.
[Note] After the expulsion of the fetus during pregnancy, if there is sudden and profuse bleeding accompanied by sallow complexion and pale lips, this condition is named "Tuo Rong" (Loss of Splendor). It is appropriate to use Dushen Tang (Single Ginseng Decoction) to vigorously tonify Qi, thereby generating Blood, which is referred to as "the formless [Qi] can generate the formed [Blood]." If the cause is stagnation of lochia preventing its discharge, leading to abdominal and hypochondriac distension and pain, one should choose between Huisheng Dan (Resurrection Pill) and Yimu Wan (Motherwort Pill), carefully considering the patient's deficiency or excess and the urgency of the condition to apply the appropriate remedy.
If the fetus dies in the uterus, urgent purgation is required; a bluish tongue, abdominal pain, and coldness like ice are signs. If foul odor emanates from the mouth after some time, carefully weigh the cold/heat nature and the severity of the purgative method.
[Note] In all cases where fetal injury leads to intrauterine fetal death, urgent purgation must be employed to prevent pathogenic factors from rushing upward to the chest and heart. However, one must verify signs such as a bluish tongue, reddish face, abdominal distension, coldness of the abdomen like ice, and the emission of foul odor from the mouth after a prolonged period, before discussing the use of purgatives. Furthermore, it is essential to examine the mother's constitution regarding deficiency/excess and cold/heat, determining whether cold purgation, heat purgation, severe purgation, or mild purgation is suitable, and applying treatment accordingly.
Foshou San (Buddha's Hand Powder) Pingwei San plus Mangxiao Formula
The mild formula for expelling the fetus is Foshou San (Buddha's Hand Powder); the severe formula is Pingwei San (Stomach-Ping Powder) added with Mangxiao (Mirabilite). Whether cold or heat purgation is indicated must be carefully examined; the mother's deficiency or excess must not be confused.
[Note] For pregnant women with an intrauterine fetal death requiring expulsion, Foshou San (Buddha's Hand Powder) is used for mild purgation, while Pingwei San (Stomach-Ping Powder) added with Mangxiao (Mirabilite) is used for severe purgation. Whether cold or heat purgation is indicated must be meticulously assessed before administering medication. This is because the mother's constitution varies between deficiency and excess; the choice between mild or severe treatment must not be confused or applied rashly to avoid misdiagnosis and error.
Foshou San (Formula see Volume 1)
In pregnancy, any critical condition allows for the prediction of life or death for both mother and child: red face and blue tongue indicate fetal death; blue face and red tongue indicate maternal peril; blue face and tongue with foaming at the mouth indicate the death of both mother and child.
[Note] Regarding all ominous and critical conditions during pregnancy, to determine whether the mother and child survive, one should ascertain this by observing the color of the pregnant woman's face and tongue. If the face is red and the tongue is blue, the fetus must be dead; if the face is blue and the tongue is red, the mother will perish; if both the face and tongue appear blue, with saliva foam flowing from the corners of the mouth, then neither the mother nor the child can be saved.
