Blood. This refers to the stagnant and turbid matter in the intestines. If toxic pathogens condense and obstruct, it is named hemorrhoids (zhì chuāng). Hemorrhoidal bleeding that is bright red is termed Intestinal Wind (Cháng Fēng); bleeding that is turbid with stasis is termed Visceral Toxin (Zàng Dú). Hong Lan Jiu (Red Blue Wine) is indicated to dispel blood stasis and generate new blood.
Hong Lan Jiu Formula
Lan Hua (Blossom of Ipomoea nil, 1 liang 5 qian), Hu Jiao (Pepper, 4 qian), Bai Tang (White Sugar, 1 jin).
Take the above three ingredients, ferment them in 1 jin of distilled spirits for seven days, strain off the dregs, and take warm in divided doses. When heat in the viscera condenses and forms a mass at the anus, it is called hemorrhoids. Zhu Du Yin (Eliminate Toxin Decoction) is indicated.
Zhu Du Yin Formula
Xiao Lian Qiao (Small Forsythia Fruit, 1 qian), E Shi (Fructus Arctii, 1 qian), Ji Zi Ke (Eggshell, scorched and crushed, 5 fen).
Take the above three ingredients, decoct in 1 sheng of water until 7 he remains, strain off the dregs, and take warm in divided doses. Swelling and protrusion in the nine orifices all belong to hemorrhoids. Pricking to bleed is indicated. If an ulcer forms and discharges pus, it is called Chuang (sores/ulcers). Qi Bao Dan (Seven Treasures Powder) is the primary treatment. Hemorrhoids are caused by toxins residing in the intestines. In severe cases, blood appears in the stool, which constitutes Intestinal Wind and Visceral Toxin. Wind-toxins lodging in the intestines constitute Intestinal Wind; heat-toxins lodging within the viscera constitute Visceral Toxin. For Intestinal Wind, dispelling wind-heat is indicated; for Visceral Toxin, eliminating blood stasis is indicated. Question: What is meant by near and far bleeding? Answer: Bright blood must be near bleeding (from the lower rectum/anus); turbid bloody stasis must be far bleeding (from higher up in the gastrointestinal tract). Near bleeding is easy to treat; far bleeding is difficult. Chronic hemorrhoids that become fistulas require the Xun Fang (Moxibustion/Fumigation Formula).
Xun Fang
Wu Bei Zi (Galla Chinensis, 4 qian), Shao Nao (Camphor, 4 qian), Ai Ye (Mugwort Leaf, 1 liang 8 qian).
Grind the above three ingredients into coarse powder, place in a container, ignite, and have the patient straddle over it. Continue until the smoke is exhausted. For hemorrhoidal fistulas with erosion, ulceration, and painful irritability, Mu Bie Tang (Strychnos Seed Decoction) is indicated.
Mu Bie Tang Formula
Mu Bie Ren (Seed of Strychnos nux-vomica, 5 pieces).
Pound this single ingredient into a paste. Add to hot water for fumigation and washing. Additionally, use Mu Bie powder mixed with egg white to apply externally; pain should resolve spontaneously. For hemorrhoids where toxin expulsion fails, and various fistulous sores unresponsive to myriad methods, Qi Bao Dan (Seven Treasures Powder) is indicated, accompanied by consuming turtle eggs; the response will be as immediate as a drumbeat. If a child's intestines are weak and the anus everts, it is called Chang Zhi (Intestinal Hemorrhoid). Liang Jiang Yin (Galangal Drink) is indicated.
Liang Jiang Yin Formula
Liang Jiang (Rhizoma Alpiniae Officinarum, 3 fen), Lian Rui (Lotus Plumule, 3 fen), Long Gu (Fossilized Bones, 3 fen), Jing Mi (Rice, 1 qian), Gan Cao (Licorice Root, 3 fen).
