Lu Zhongyang stated: Cough (Ke Sou) is caused by the Lung (Fei) being affected by slight cold evil (Han Xie). In the eighth and ninth lunar months, Lung Qi (Fei Qi) is at its peak; coughs occurring then are necessarily excess syndromes (Shi Zheng), not chronic conditions. Symptoms include a red face, body heat, and profuse phlegm. The treatment principle should be to purge using Tingli Wan (Descurainia Seed Pill). If it is a chronic cough, purging must not be used. Coughs in winter belong to wind-cold cough (Shang Feng Ke) and should be treated with Ma Huang Tang (Ephedra Decoction) to induce sweating. If there are heat symptoms, such as a red face, thirst, thick and solid saliva and phlegm, and throat discomfort, Gan Jie Tang (Licorice and Platycodon Decoction) is suitable. For Lung excess (Fei Sheng), characterized by cough followed by wheezing, facial edema, thirst or lack thereof, and body heat, Xie Bai San (Drain the White Powder) should be used to clear Lung heat. For cough accompanied by vomiting of saliva, phlegm, or undigested milk/food, Bai Bing Zi (White Cake Pill) should be used for purging.
However, the Lung governs Qi and corresponds to the skin and hair (Pi Mao). The Lung is the canopy of the five Zang organs (Wu Zang Hua Gai). When children contract wind-cold, the pathogen lodges in the skin and enters the Lung meridian. Mild cases present with cough; severe cases present with wheezing and urgency. Lung injury by cold leads to cough with much phlegm and rapid rattling in the throat. Lung injury by warmth leads to obstructed and stagnant cough sounds. Cold injury requires dispersing cold evil; warmth injury requires draining stagnation. Dispersing methods use acrid-sweet herbs like Guizhi (Cinnamon Twig), Mahuang (Ephedra), and Xixin (Asarum). Purging/drainage methods use sour-bitter herbs like Tingli (Descurainia Seed) and Dahuang (Rhubarb). Additionally, the sourness of Wuweizi (Schisandra Berry) and Wumei (Mume Fruit) can astringe Lung Qi and is also an essential herb for treating cough.
Chronic cough that does not resolve will inevitably lead to fright palpitations (Jing Ji) and stubborn phlegm, with blood vessels engorging the face. The cough may transmit to the five Zang organs, presenting as vomiting, excessive phlegm, cold extremities, or fear and palpitations. In severe cases, the eye sockets turn purple-black as if bruised, and the whites of the eyes become red like blood, termed "Blood Eye" (Xue Yan). The treatment is to apply a paste made from Sheng Dihuang (Rehmannia Root) and wet black soybeans to the eyes; the swelling and darkness will subside, and the blood will discharge with tears. This is an excellent prescription. Concurrently administering Mai Jian San (Wheat Decoction) will stop the cough. Chronic cough developing into epilepsy should be treated with anti-epileptic formulas. Generally, treating cough requires first dispersing cold evil, then administering Qi-regulating, phlegm-resolving, and cough-stopping medicines for recovery. Initial options include Jiu Bao Wan (Nine Treasures Pill), Hua Gai San (Canopy Powder), Tingli Wan, Bao Long Wan (Dragon-Holding Pill), or Xixin Wuweizi Tang (Asarum and Schisandra Decoction). If heat is present, Lung-cooling agents like Chaihu (Bupleurum) and Huangqin (Scutellaria) or Xie Bai San may be used. For profuse phlegm and wheezing, use Jinxing Wan (Golden Star Pill) to drain phlegm first, then administer the aforementioned medicines. Subsequently, Baibu Wan (Stemona Pill), Sheng Xi San (Raw Rhinoceros Horn Powder), or Tianma Dingchuan Yin (Gastrodia Anti-Wheezing Drink) may be taken to regulate and restore peace. For cold-type cough, Xiao Qinglong Tang (Minor Blue Dragon Decoction) with Xingren (Apricot Kernel) added and Mahuang removed is applicable. For heat or seasonal epidemic cough, Chai Hu San (Bupleurum Powder) with Xingren and Wuweizi added, or Chaihu Shigao Tang (Bupleurum and Gypsum Decoction) may be used. Qian's Sheng Xi San with modifications is also good. For cough due to fright, Tianma Fangfeng Wan (Gastrodia and Saposhnikovia Pill) treats it; Xingxing San (Awakening Powder) and Hua Feng Dan (Wind-Dissolving Pellet) are also acceptable. Jin Fei Cao San (Golden Fern Leaf Powder) and San Ao Tang (Three Unfreezing Decoction) with modifications are standard prescriptions for wind-cold cough. Hejie Tang (Harmonizing Decoction) treats four-season cold invasions and can be modified.
