Slow convulsion refers to a condition occurring after an illness, characterized by vomiting or diarrhea, leading to deficiency and damage of the Spleen and Stomach. The whole body feels cold, the breath from the mouth and nose is also cold, the limbs occasionally twitch or convulse, the patient sleeps drowsily with eyes half-open showing the whites. This indicates a lack of Yang Qi (deficiency of Yang energy), and it is primarily treated with Gualou Tang (Trichosanthes Decoction).
Generally, the manifestations of slow convulsion include vomiting or diarrhea, phlegm rattle in the throat with slight wheezing, eyes open but with a slack expression, sleeping with eyes half-open, startle jumps and convulsions alternating with periods of calm, or fever in the trunk with cold limbs, or warm limbs with cold breath, pale white or pale cyan complexion, cyan eyebrows or lips, and a pulse that is deep, slow, scattered, or moderate. These conditions often arise from excessive use of cold-natured medicines during acute convulsion, or too rapid purgation, causing transformation into this syndrome. Others result from persistent vomiting and diarrhea, sudden severe vomiting and diarrhea due to Qi deficiency, hole-like diarrhea (watery stool) due to visceral cold, Qi collapse from chronic diarrhea, accumulation removal via incorrect purgation, hematemesis or hematochezia, unresolved wind-cold pathogen transforming from Taiyang disease, chronic cough progressing to epilepsy, intestinal parasites rushing to the heart, hernia with abdominal pain, profuse sweating day and night, Spleen fatigue with excessive sleep, irritability with thirst for drinking, edema of limbs, obstruction of urination and defecation, carbuncles and sores, herpes zoster (Shingles), rapid gangrene of the gums, all of which can transform into slow convulsion patterns. Among these, only those caused by deficiency resulting from prolonged vomiting, diarrhea, and accumulation are particularly prone to rapid progression. Treatment methods must carefully investigate the origin and course of the disease; one cannot apply medications indiscriminately. If caused by vomiting and diarrhea, Lizhong Tang (Regulate the Middle Decoction) plus Muxiang (Aucklandiae Radix) should be used to warm the Middle Jiao, and Wuling San (Five-Ingredient Powder with Poria) to guide out water retention. If caused by visceral cold and hole-like diarrhea, Shuzhu Fuzi Tang (Atractylodes and Aconite Decoction) should be administered first. If resulting from purging accumulated matter, Tiaoqi San (Regulate Qi Powder) should be used first to harmonize the Spleen and Stomach. If caused by external wind-cold, Guizhi Tang (Cinnamon Twig Decoction) or Gegen Tang (Kudzu Root Decoction) may be used; other cases can be inferred analogously. However, although slow convulsion belongs to Yin syndromes, one must accurately assess the depth of Yin-Yang imbalance. One should not solely use warm, dry, and intensely hot agents. Instead, while generating Stomach Qi, add herbs that extinguish wind and stop spasms, such as Quanxie (Scorpio), Huashe (Bamboo Pit Viper), Jiangcan (Bombyx Batryticatus), Baifu (Typhonii Rhizoma), Tianma (Gastrodiae Rhizoma), and Nanting (Arisaematis Rhizoma), which constitute good prescriptions. If slow convulsion presents with remaining Yang symptoms, there is no need to restore Yang; instead, focus on extinguishing wind and regulating the Stomach. Medications can be either cooling or warming, aiming merely to balance Yin and Yang. Suitable formulas include Taiyi Baosheng Dan (Great Unity Protect Life Pill), Jubao Dan (Treasure Accumulation Pill), Chanxie San (Cicada and Scorpion Powder), Shenbao Jiji Dan (Divine Treasure Mutual Completion Pill), Laifu Dan (Restoring Vitality Pill), Wangshi Xingxing San (Wang’s Awakening Powder), Xingpi San (Awaken Spleen Powder), Da Xingpi San (Great Awaken Spleen Powder), and Wenbai Wan (Warm White Pill). If Yang is deficient and Yin is excessive, and the disease has progressed to pure slow convulsion without convulsions, retroversion, or lateral gaze, but only with lethargy, prescribe Xingxiang Quanxie San (Star-Aucklandia Scorpion Powder), Dingming Yinzi (Life-Determining Drink), Sisheng San (Four Sages Powder), Wu Xie Sijunzi Tang (Black Scorpion Four Gentlemen Decoction), Tiannanxing San (Arisaema Powder), Wuchen Tang (Black Musk Decoction), or Chenxiang San (Aquilaria Powder). Only if the hands and feet are ice-cold should one restore Yang using Huiyang (Restore Yang) methods with Huangqi (Sulfur) and Fuzi (Prepared Aconite Root). For slow convulsion with phlegm and warmth of the body, use Tiannanxing Wan (Arisaema Pill), Suhexiang Wan (Liquid Amber Pill), or Baiwanzi (White Pills); for excessive phlegm, use Shenbao Jiji Dan or Mengshi San (Malachite Powder); for extreme deficiency where phlegm cannot be purged, use Lingzhi Wan (Trogopterus Dung Pill) or Shizhen Wan (Seven Treasures Pill). Cold-natured drugs like Tongguan Lichang (Opening Passages and Purging Intestines) agents such as Niuhuang (Calculus Bovis), Shexiang (Moschus), and Bashuang (Crotonis Semen Preparatum) are strictly prohibited. Before slow convulsion attacks, if the child sleeps excessively, sticks out the tongue, shakes the head, has cyan face and erect hair, vomits milk with a fishy smell, and sweats on the forehead, this indicates wind generated by Stomach deficiency after vomiting. Treat immediately with Jiexing Xingpi San (Stop Wind and Awaken Spleen Powder), or Sijunzi Tang (Four Gentlemen Decoction) plus Quanxie (Scorpio) and Fangfeng (Saposhnikoviae Radix), Yín Bái Sǎn (Silver White Powder), or Gouteng Yin (Uncaria Decoction) minus Mahuang (Ephedra Herb). More diverse approaches and flexible medication changes are necessary; do not neglect this. During active slow convulsion, if there is diarrhea, vomiting of milk, cold sweat, closed eyes, red lips, protruding tongue, head shaking, stiff neck, rib movement, chest oppression, coarse breathing, and mouth ulcers, apply Tiannanxing powder (Arisaema powder) to the sole of the foot. Frequent administration of Renshen Tang (Ginseng Decoction) is beneficial. If later mouth ulcers appear and the child refuses milk and food, it is called Suoge (Locked Diaphragm), which is difficult to save. Any pediatric condition presenting with a mouth opening like a fish's mouth and crow-like sounds is incurable. According to *Shengji Jing* (Holy Medicine Classic), Hupo Wan (Amber Pill) and Nanxing Jian (Arisaema Decoction) can be taken. It is stated: The Heart stores the Spirit and dislikes heat. Children’s constitutions are naturally hot. If they contract wind pathogens, wind-heat clashes within the Zang-Fu organs, causing Qi stagnation and anger, invading the Heart internally, disturbing the Spirit and causing fright. If fright persists severely, palpitations and restlessness occur, constituting Jingji Disease (Fright-Palpitation Disease), treated with Zijin San (Purple Gold Powder), Niuhuang San (Calculus Bovis Powder), and Dadansha Wan (Large Cinnabar Pill). For acute convulsion, Longchi San (Dragon Tooth Powder) and Sisheng San (Four Sages Powder) are available options.
