A patient suffered from a sore throat, with qi stagnation at night preventing him from lying on his pillow. He had coughing with phlegm that could not be expectorated, sores all over his body, edema of the face and feet, nighttime fever, and profuse cold sweats upon waking. This was caused by damp-heat in the Spleen meridian, leading to impaired water metabolism. [Yiyiren Tang] (Coix Seed Decoction) consisting of Coicis Semen (Yiyiren), Farfarae Flos (Kuandonghua), Citri Reticulatae Pericarpium (Chenpi), Fritillariae Bulbus (Beimu), Peucedani Radix (Qianhu), Raphi Semen (Luobozhi), Platycodonis Radix (Jiegeng), Morus Albae Cortex (Sangbaipi), Poria (Fuling), and Glycyrrhizae Radix et Rhizoma (Gancao) were administered. After taking this prescription, the wheezing and coughing subsided significantly, allowing him to sleep on his pillow again. However, swelling in the lower body persisted, with tight and hard skin, yellow urine, and dizziness upon movement. A formula containing Trogopterori Massam (Dafupi), Lagenariae Spongiosa Cortex (Fulingpi), Citri Reticulatae Pericarpium (Chenpi), Morus Albae Cortex (Sangbaipi), Acanthopanacis Cortex (Wujiaopi), Zingiberis Recens Cortex (Shengjiangpi), Chaenomelis Fructus (Mugua), and Curcumae Longae Radix (Jianghuang) was used for four doses, resulting in the resolution of the swelling.
He Xixin, who was proficient in the I Ching and numerology, always experienced abdominal distension and diarrhea after consuming slightly cool food, whether congee or thin gruel. Various spleen-regulating remedies had been tried without success, so he sought my consultation. The pulse at the three positions on the left hand was soggy and weak, as was the cun position on the right hand; the guan position was slippery, and the chi position was deep and faint. This indicated deficiency-cold in the Lower Yuan (Lower Burner/Essence). The treatment method should be warming and tonifying. Boswelliae Fructus (Buguzhi), Eucommiae Cortex (Duzhong), and Cuscutae Semen (Tusizi), each 2 qian, together with Corni Fructus (Shanzhuyu), Ginseng Radix et Rhizoma (Renshen), and Dioscoreae Rhizoma (Shanyao), each 1 qian, Poria (Baifuling) and Alismatis Rhizoma (Zexie), each 8 fen, and Myristicae Semen (Rouguo) 5 fen, were prescribed. After several doses, he recovered.
Lady Sun, wife of Cheng Xiangru, experienced vertigo, gastric discomfort (gastric嘈杂), vomiting of clear fluid, and restlessness at night after epistaxis (nosebleed). She felt hungry but food could not pass through the diaphragm. This was due to Spleen deficiency with constrained fire in the Liver and Gallbladder. Ginseng Radix et Rhizoma (Renshen), Coptidis Rhizoma (Huanglian), Atractylodis Macrocephalae Rhizoma (Baizhu), Dolichoris Semen (Bian Dou), Glycyrrhizae Radix et Rhizoma (Gancao), Citri Reticulatae Pericarpium (Chenpi), Pinelliae Rhizoma Preparatum (Banxia), Bambusae Folium (Zhuru), Poria (Fuling), and Gypsum Fibrosum (Shigao) were decocted in water and taken. With proper nursing, her condition stabilized.
My nephew Junxiao, the second son of Houxi's elder brother, suffered from headache and neck pain, stiffness of the waist and spine, pain throughout the body as if beaten with a stick, pain in the umbilical and abdominal region, thirst, insomnia, anorexia, and floating-rapid pulses in all six positions. Physicians in Wu attributed this to Yin deficiency and administered three doses of Yin-nourishing and fire-descending prescriptions, which exacerbated the condition and added vomiting and nausea. Subsequent treatments aimed at unblocking resulted in persistent heat, with agitation worsening in the afternoon. Fang Heyu was invited to consult and diagnosed an external contraction, prescribing [Renshen Baidu San] (Ginseng Detoxification Powder). The Wu physicians argued that given the patient's Yin deficiency and weakness, sweating therapy was contraindicated, and instead prescribed [Sini Tang] variant? No, [Siwu Tang] (Four Substances Decoction) plus Bupleuri Radix (Chaihu), Puerariae Radix (Gegen), Menthae Haplocalycis Herba (Bohe), Scutellariae Radix (Huangqin), and Anemarrhenae Rhizoma (Zhimu). Consequently, the heat became intense like burning, and consciousness became clouded. I was away but was summoned back to diagnose. The six pulses were floating, wiry, rapid, and striking the fingers forcefully. I told him: "This is Chunwen (Spring-Warmth disease)." The previous diagnosis was largely correct. However, internal injury complicated the case, causing severity. The Yin-nourishing agents caused stagnation and drew pathogenic factors into the Yin aspect, explaining the intensified heat and irritability. The treatment should involve clearing and resolving while eliminating stagnation. [Erchen Tang] (Two-Aged Decoction) plus Notopterygii Rhizoma et Radix (Qianghuo), Bupleuri Radix (Chaihu), Saposhnikoviae Radix (Fangfeng), Hordeum Vulgare (Maiya), and Crataegi Fructus (Shanzha) were administered. Mild sweating ensued, and the heat reduced by half. However, tidal fever occurred in the afternoon, bowel movements were absent, and waist/umbilical pain persisted. [Xiaochaihu Tang] (Minor Bupleurum Decoction) plus Puerariae Radix (Gegen), Paeoniae Radix Alba (Baishao), Citri Exocarpium Viride (Qingpi), Coptidis Rhizoma (Huanglian), and Crataegi Fructus (Shanzha) were drunk. The heat decreased further, bowel movements resumed, and waist/umbilical pain diminished. Due to lack of appetite, a formula of Bupleuri Radix (Chaihu), Glycyrrhizae Radix et Rhizoma (Gancao), Citri Exocarpium Viride (Qingpi), Aurantii Fructus Immaturus (Zhishi), Hordeum Vulgare (Maiya), Anemarrhenae Rhizoma (Zhimu), Scutellariae Radix (Huangqin), and Paeoniae Radix Alba (Baishao) was used, resolving all symptoms. Only fatigue remained. Ginseng Radix et Rhizoma (Renshen), Dolichoris Semen (Baibian Dou), and Coicis Semen (Yiyiren) were added, while Bupleuri Radix (Chaihu) and Citri Exocarpium Viride (Qingpi) were removed. With recuperative care, he fully recovered.
