The pulse at the right Cun and Guan positions is surging (Hong) and rapid (Shu), or wiry (Xian) and surging. This indicates wind-heat in the stomach and intestines, causing toothache. If the Chi pulse is surging yet empty upon pressure, it indicates Kidney deficiency, manifesting as loose and sparse teeth due to ministerial fire flaring upward.
Teeth are the essence of bones. Bones are governed by the Kidneys; thus, the blood vessels and fluids of all meridians can moisten and cool them. When blood is abundant, there is coolness, which makes bones firm; when blood is deficient, there is heat, which causes teeth to float. The upper lip and the lower teeth below the philtrum belong to the Kidney meridian. The lower front teeth belong to the Ren Mai (Conception Vessel). The cheeks above and below and the area around the mouth belong to the Yangming meridians. Therefore, the gums correspond to Earth, and teeth are like trees planted in soil. Cool soil stabilizes roots; hot soil causes teeth to shake. Teeth are bone; bone itself does not hurt. Pain arises from the gingival muscles. Eating too many plums causes teeth to shrink and numb, becoming weak and unable to chew—this is acid controlling Earth. Gingival recession is caused by Stomach fire. Sparse and brittle teeth are caused by Kidney ministerial fire. The Classics state: "All pain, itching, sores, and ulcers belong to Heart fire." Below Heart fire, Yin essence should support it. If this support is unbroken, why would there be pain? Dental diseases include wind-tooth, worm-tooth, dental gangrene (Ya Gan), and periodontal disease (Ya Xuan). These arise from imbalanced fire. Heat generates wind; the character for "wind" contains "worm." Also, blood boils and bleeds when encountering fire, leading to Ya Xuan. Heat combined with external pathogens causes swelling and pain. When true Yin is undeveloped but heat is blazing, it is called Gangrene, leading to rapid ulceration. In treatment, preserving Kidney water addresses the root; clearing Stomach fire addresses the branch; dispersing wind pathogens addresses the expedient measure. Wind herbs effectively unblock meridians and open pores, aligning with the principle that trapped fire should be dispersed. Those cultivating health should eat blandly to prevent blood stagnation; avoid rich, sweet, spicy, and pungent foods to prevent fire accumulation. If dental remedies fail, it is due to accumulated heat, often from excessive sexual activity causing Yin deficiency, or mistaking rich flavors for tonics, thereby increasing fire. Moderating desires now ensures comfort in later years. Those with excess Yangming fire are prone to dental issues. Using hot and pungent herbs like Zanthoxylum (Huajiao), Ginger (Shengjiang), Croton (Badou), or Piper Longum (Bibo) to stop pain provides temporary relief but deepens pathogenic factors. Take heed!
Children with dental gangrene or toothaches suffer from indulgence in sweets and flavors without restraint, wearing heavy clothes and thick quilts without care. Infants developing gangrene inherit maternal heat. Heating bedding for warmth during sleep harms children because their visceral true Yin is undeveloped, causing sudden harm. One must study this carefully.
The Classics say: "When Kidney qi declines, teeth become loose; when essence is secured, teeth remain firm."
Discussion on all tooth swellings and pains: These belong to excess fire in the Stomach meridian, often caused by consuming spicy, hot, and rich foods or taking too many warming medicines, resulting in Stomach heat. Symptoms include pain in upper and lower teeth radiating to the head with heat sensation, and a preference for cold over heat.
Modified Qingwei San (Clear the Stomach Decoction):

For receding gums with bleeding, add Ce Baiye (Platycladi Cacumen), Huangqin (Scutellariae Radix), Jingjie, and Zhizi (Gardeniae Fructus). For toothache in patients with deficiency and damage, add Huangbai (Phellodendri Cortex), Zhimu (Anemarrhenae Rhizoma), Renshen (Ginseng Radix et Rhizoma), and Gancao (Glycyrrhizae Radix et Rhizoma). For floating pain throughout the mouth making chewing difficult, add Lianqiao (Forsythiae Fructus), Xuanshen (Scrophulariae Radix), and Shaoyao (Paeoniae Radix Alba).
