Pulse presenting as bilateral wiry (Xian) indicates Cold-Phlegm fluid retention. Pulse presenting as unilateral wiry also indicates Phlegm fluid retention. In Lung Phlegm, the pulse is not necessarily wiry, but there will be panting and shortness of breath. The Classics state: A pulse that is floating, fine, and slippery indicates injury by Phlegm fluid. A wiry and rapid pulse with Cold-Phlegm is difficult to treat in spring and summer. A deep and wiry pulse indicates Suspended Phlegm (fluid retention in the chest and hypochondrium) with internal pain; the patient has shortness of breath and wandering pain in the four limbs. A deep pulse indicates Retained Phlegm (long-standing fluid retention). If the pulse at the front of the Guan position on both left and right hands is floating, wiry, large, and excess, it indicates thick Phlegm above the diaphragm, and emesis should be induced. If a patient has taken many medicines without effect, and the pulse at the Guan position is hidden (Fu) and large, this is a Phlegm syndrome. Darkening of the eyelids and the area under the eyes, resembling soot stains, also indicates Phlegm. A long-term choppy (Se) pulse indicates that Phlegm and fluid are firmly consolidated, obstructing the pulse vessels, making them difficult to open suddenly, requiring prolonged regulation.
Phlegm is a disease name. It originates in the Spleen and Stomach. However, if Spleen and Stomach Qi are vigorous, food and drink are easily digested, so how can Phlegm form? Or after eating, due to anger, fatigue, fright, or wind pathogen invasion, the essence of food and drink fails to transform and transport, thereby forming Phlegm fluid. Some flow into the meridians and skin; some stagnate in the Zang-Fu organs and joints, overflowing throughout the body, reaching everywhere. Once Phlegm Qi becomes excessive, the pathogen overcomes the healthy Qi. If it invades the great collateral of the Spleen, blocking Qi mechanism, one may suddenly collapse, known as Phlegm Jue (syncope due to Phlegm). If Phlegm Qi ascends to the Lung, there is panting, urgency, and cough; if it disturbs the Heart, there is palpitations and mental confusion; if it moves to the Liver, there is dizziness, numbness, and distension/fullness in the costal region; if it affects the Kidney, there is expectoration of much Phlegm and saliva flowing; if it flows to the Middle Jiao (Zhongwan), there is vomiting, diarrhea, and alternating chills and fever; if it pours into the chest, there is obstruction in the throat and diaphragm, and pain in the supraorbital ridge; if it enters the intestines, there is gurgling sounds; if it scatters in the chest and back, there is point-specific pain upon touch; or it blocks the limbs, or causes hemiplegic numbness on one side of the back. Scattered Phlegm produces sound; aggregated Phlegm causes obstruction. Throughout the upper and lower body, it generates myriad diseases. Treatment must address the specific cause: deficiency requires tonification; fire requires descending; Qi stagnation requires smoothing; depression requires opening; food stagnation requires guiding/digestion; Wind-Cold-Damp-Heat require dispersing, clearing, and drying to eliminate. Thus, treating disease requires seeking the root. This section discusses Damp-Phlegm, Heat-Phlegm, Wind-Phlegm, Old-Phlegm, Cold-Phlegm, and Food-Stagnation Phlegm, which should be treated with modifications of the following formulas.
Er Chen Tang (Two-Aged Decoction)
Chenpi (Tangerine Peel, peeled of white pith, 1 qian) Banxia (Pinellia Tuber, soaked in hot water, 2 qian) Baifuling (White Poria, peeled, 1 qian) Gancao (Licorice Root, 0.5 qian)
Crush these into coarse pieces for one dose. Add three slices of fresh ginger, decoct with water, and take warm.
For cases of excessive Damp-Phlegm with soft and heavy body, add Cangzhu (Atractylodes Rhizome) and Baizhu (Atractylodes Macrocephala Rhizome).
For Heat-Phlegm, add Huanglian (Coptis Rhizome) and Huangqin (Scutellaria Root). Since Phlegm rises due to excessive Fire, descending Fire is the priority; add Baizhu, Huangqin, Ruan Shigao (Soft Gypsum), and Huanglian.
For dizziness and acid regurgitation/heartburn, which is Fire stirring Phlegm, also add Zhizi (Gardenia Fruit), Huanglian, and Huangqin.
For Wind-Phlegm, add Tianma (Gastrodia Rhizome), Zhike (Bitter Orange Fruit), Nanxing (Arisaema Rhizome), Baifuzi (Typhonii Rhizoma), Jiangcan (Bombyx Batryticatus), Yazao (Gleditsiae Spina), etc. For Qi deficiency, add Zhuli (Bamboo Sap); for Qi excess, add Jingli (Schizonepeta Sap). All should be taken with Jiangzhi (Fresh Ginger Juice).
For Old-Phlegm, use Haishi (Floating Limestone), Banxia, Gualou (Trichosanthes Fruit), Xiangfu (Cyperi Rhizoma), Lianqiao (Forsythia Fruit), etc. Wubeizi (Chinese Gall) assists these herbs to treat stubborn Phlegm; pills are suitable. If there is a sensation of an object in the throat that cannot be expectorated or swallowed, this is Phlegm knot; use medicine to resolve it, adding salty/herbs that soften hardness like Gualou, Xingren (Apricot Seed), Haishi, Jiegeng (Platycodon Root), Lianqiao, Xiangfu, with a small amount of Puxiao (Mirabilite) and Jiangzhi, refined honey to make pills, and hold in mouth to dissolve. If the pulse is choppy, it is hard to open suddenly and requires prolonged regulation. If Qi is excess and Phlegm-Heat is knotted, emesis is difficult; if it forms clumps that cannot be expectorated, or if Qi is stagnant, it is difficult to treat.
For Cold-Phlegm obstructing the chest, double the dosage of Banxia. For severe cases, add Mahuang (Ephedra Herb), Xixin (Asarum Root), and Wutou (Aconite Root). For Phlegm-Jue headache, add Banxia.
For Food-Stagnation Phlegm, add Shenqu (Medicated Leaven), Maiya (Sprouted Hordeum Fructus), Shanzha (Hawthorn Fruit), fried Huanglian, and Zhishi (Immature Bitter Orange Fruit) to digest. For severe cases, purgatives are necessary; pills are suitable. If accompanied by Blood deficiency, send down with Blood-tonifying medicine. Phlegm in the Middle Jiao is often due to food stagnation. Stomach Qi relies on nourishment; do not let it become deficient. If purged completely, it will become deficient. Belching and acid regurgitation indicate food stagnation with depression and heat, with Fire Qi moving upward; add Huangqin and Nanxing.
For Blood deficiency with Phlegm, add Tiandong (Asparagus Tuber), Zhimu (Anemarrhena Rhizome), Gualouren (Trichosanthes Seed), Xiangfum (Cyperi Rhizoma), Zhuli, and Jiangzhi. If blood is present, add Huangqin, Baishao (White Peony Root), and Sangpi (Morus Bark). If Blood stagnation does not move and there is fluid in the Middle Jiao, take Zhuli added with Jiangzhi and Jiuji (Leek Juice), drinking three to five cups; recovery follows after feeling restless and irritable in the chest.
For Qi deficiency with Phlegm, add Renshen (Ginseng Radix) and Baizhu. For Spleen deficiency, tonify Middle Qi to transport Phlegm; for descending, add Baizhu, Baishao, Shenqu, and Maiya, combined with Shengma (Cimicifugae Rhizoma) to lift. For internal injury with夹杂 Phlegm, add Renshen, Huangqi (Astragalus Root), and Baizhu, transmitted with Jiangzhi; adding Zhuli is excellent.
If Phlegm is above the diaphragm, emesis is mandatory. If purgatives fail and Phlegm is sticky and turbid, emesis is required. If the pulse is floating, emesis is indicated. If Phlegm is in the meridians, emesis is essential. Emesis contains the meaning of dispersing. When using emetic drugs, promote Qi ascent to facilitate vomiting; add Fangfeng (Saposhnikovia Root), Chuanxiong (Ligusticum Rhizome), Jiegeng, Cha Ya (Tea Bud), Jiangsheng (Fresh Ginger), and Qizhi (Pickled Mustard Greens), or Gouti San (Melon Peduncle Powder). For emesis, bind the abdomen tightly with cloth and perform in a non-drafty place.
If Phlegm is in the stomach and intestines, it can be resolved by purgation, using Zhishi, Gansui (Kansui Root), Badou (Croton Seed), Dahuang (Rhubarb Root), Mangxiao (Mirabilite), etc. Overuse of draining/purgative drugs for Phlegm weakens Spleen Qi, leading to increased Phlegm production.
For Phlegm in the hypochondrium, only Baijiezi (White Mustard Seed) can reach it.
For Phlegm between the skin and fascia, only Jiangzhi and Zhuli can reach it. For Phlegm in the four limbs, only Zhuli can open it. For Phlegm in the meridians, Zhuli is used, assisted by Jiangsheng and Jiuji. For Phlegm in the diaphragm causing mania, amnesia, or Wind-Phlegm, use Zhuli, which has the same efficacy as Jingli. For Qi deficiency with little appetite, use Zhuli; for Qi excess with good appetite, use Jingli.
Whenever there are lumps anywhere in the body, head, neck, cheeks, or torso, it is Phlegm. Ask what foods they usually prefer, then after emesis or purgation, administer medicine.
Whenever there are nodules in the head, face, neck, cheeks, or body that are painless, non-red, and do not suppurate, they are all Phlegm deposits; use medicine locally to dissipate them.
Phlegm, as a pathogenic factor, reaches everywhere because it moves up and down with Qi.
Regarding Zhuli (Bamboo Sap): It greatly treats Heat-Phlegm and nourishes Blood while clearing Heat. For Phlegm-Jue causing unconsciousness near death, administering Zhuli leads to revival; it is truly a miraculous drug bringing the dead back to life.
Take young bamboo shoots, cut into two-foot lengths. Small ones split into two pieces, large ones into four. Soak in well water for about an hour. Place two thin bricks, arrange bamboo pieces on them, leaving one or two inches exposed at both ends. Apply fierce fire underneath, collecting the dripping sap in basins at both ends. Mix one part Jiangzhi with six parts Zhuli. For severe heat, add only half part. Adding Zhuli to decoctions is excellent. Jingli also treats Heat-Phlegm and is obtained similarly.
Regarding Phlegm belonging to Dampness, transformed from body fluids, caused by Wind-Cold-Damp-Heat pathogens or emotional/dietary injuries, leading to rebellious Qi and turbid fluids transforming into Phlegm fluid. Thus, "Phlegm moves due to Fire," so descending Fire is primary. "Fire arises from Qi rebellion," so smoothing Qi is key.

A person with shortness of breath and Phlegm, red and scanty urination, heel pain, Chi pulse floating and large, choppy on pressure. This is Shengqi Wan (Kidney Qi Pill) treating Fluid retention. Use Siwu Tang (Four Substances Decoction) to send Liuwei Wan (Six-Ingredient Pill) down. Recovered in less than a month. Master Zhang Zhongjing said: For Qi deficiency with Fluid retention, use Shengqi Wan to tonify and expel. Truly opening the ignorance of later learners and saving countless premature deaths. Note: The text states Shengqi Wan is Liuwei Wan, though traditionally they differ.
A scholar, fat and fond of drinking, suffered Phlegm-panting in mid-autumn. Used Er Chen Tang, Huangqin, Huanglian, condition worsened. Added Sangpi, Xingren. Night sweats and shortness of breath, added Zhimu, Zhike. Occasional fever, I considered it Spleen-Lung deficiency-cold. Used Bawei Wan (Eight-Ingredient Pill) to tonify Earth-Mother, and Buzhong Yiqi Tang (Tonify Middle and Augment Qi Decoction) to calm Middle Qi, and recovered.
A woman vomited Phlegm due to anger, chest fullness and pain. Took Siwu Tang, Er Chen Tang, Huangqin, Huanglian, Zhike, etc., without effect. Added Wind-dispelling agents. Resulted in hemiplegia, tendon contraction, limb flaccidity, worsening at dusk, internal heat, dry mouth, fatigue. I considered it Depression/Anger injuring Liver-Spleen, causing further Blood-Qi damage. Thus used Xiaoyao San (Free and Easy Wanderer Powder), Buzhong Yiqi Tang, and Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) for treatment. Glad she was careful with her health, fully recovered after more than a year, body healthy.
