Pediatric abdominal distension may be of deficiency or excess type. (It may arise from vomiting and diarrhea, or from milk and food retention) affecting the Spleen and Stomach, causing Spleen-Stomach deficiency with Qi attacking and generating the condition. Excess type presents with chest oppression, restlessness, wheezing, and fullness; treatment can involve purgation. Qian Yi used Zishuang Wan (Purple Frost Pill) or Baibing Zi (White Cake Pellet). Deficiency type does not present with wheezing; treatment should not involve purgation, nor should it use strongly warming herbs such as Dingxiang (Clove), Muxiang (Aucklandia Root), Chuanpiao (Tangerine Peel), and Doukou (Cardamom). Because the Spleen and Stomach are deficient and Qi has not yet emerged, although there is abdominal distension, there is no wheezing. It is best to use medication to disperse Qi upward and downward, leading to recovery. Qian Yi used Taqi Wan (Collapse Qi Pill). If mistakenly treated with purgation, Spleen deficiency occurs, and Qi rises to attach to the Lung and move along it. As mother and child organs, both Spleen and Lung become deficient. The Lung governs the eyelids and cheeks; the Spleen governs the limbs. If yellow color appears indicating severe deficiency, the eyelids, cheeks, and limbs will appear yellow and swollen. Qian Yi used Taqi Wan. If deficiency Qi has already risen and attached to the Lung, internal weakness of the Spleen and Stomach ensues, frequently generating deficiency Qi that enters the limbs and face. Children are prone to deficiency-excess patterns; when the Spleen is deficient, it cannot tolerate cold or heat. Administering cold herbs generates cold pathogens, while administering warm herbs generates heat pathogens. One must recognize this to avoid errors. Chronic Stomach deficiency-heat often leads to jaundice or excessive thirst. Spleen deficiency fails to control Kidney Qi, which ascends with Lung Qi to the limbs, causing facial and ocular edema resembling water accumulation. Kidney Qi infiltrating and floating up to the Lung results in severe wheezing. Qian Yi also used Taqi Wan; if not cured, gradually increase the number of pills. After cure, if the face remains pale, it is because deficiency and weakness have not yet recovered. For treating deficiency-type abdominal distension, if Taqi Wan is ineffective and there is food stagnation in the abdomen, with yellow stool and urine, occasional mild wheezing, a deep and solid pulse, and ability to drink and eat, purgation may also be considered. This is because the Spleen was initially deficient and later developed stagnation. Treatment should first tonify the Spleen, then administer purgation. After purgation, tonify the Spleen again for recovery. If purgation is administered without prior tonification, extreme deficiency will result. Furthermore, one should not tonify the Lung, lest it generate deficiency wheezing. The formulas used by Qian Yi are recorded in this collection; other formulas are listed below:
