The classic states that there are nine types of parasites in the human zang-fu organs. Among them, three types of parasites are particularly prone to causing disease. If the zang-fu organs are strong, they cannot cause harm; if the zang-fu organs are weak, symptoms will arise following the disturbance by these parasites.
Therefore, the classics also separately name these three parasites. The first is called Ascaris (also known as long worm), residing between the stomach and epigastrium. When active, it causes children to vomit greenish-white foam or greenish water, accompanied by stabbing pain in the heart and abdomen. If the worm penetrates the heart, death ensues. The second is called Pinworm, residing in the rectum. When numerous, its activity leads to hemorrhoids, skin erosion, sores, scabies, and leprosy-like conditions. The third is called Taenia (flatworm), residing between the intestines and stomach. Its activity depletes the body's essence and qi, causing soreness and weakness in the waist and legs. Additionally, there is another parasite shaped like a fine thread or horse tail, hence commonly called Horse-tail Worm. This parasite is not counted among the nine worms. Some say it transforms from ingested oil and hair during diet, also residing in the stomach. Its activity causes abdominal churning and stabbing pain, unbearable for the child. Parasitic diseases in children often occur after other illnesses due to the weakness of the zang-fu organs. It is especially severe in cases of infantile malnutrition (Gan syndrome), consumptive fever, vomiting, and diarrhea. Qian Yi discussed the symptoms of parasitic movement in children: pale complexion, heart and abdominal pain, but without vomiting foam or clear water. Thus, Qian Yi further stated that heart and abdominal pain with vomiting indicates parasitic pain; heart and abdominal pain without vomiting water indicates cold pain; vomiting water without heart and abdominal pain indicates stomach cold. Furthermore, parasitic pain is slightly similar to convulsive epilepsy, but the eyes do not deviate, and the hands do not twitch. Distinguishing by this ensures no misdiagnosis. Qian Yi treated parasitic pain using An Chong San (Worm-Calming Powder). However, people usually administer anti-parasitic medicines before bedtime without regard to specific days, resulting in ineffectiveness. It is only effective when taken around the 4th or 5th day of the lunar month at Wu Geng time (early morning, 3-5 AM). By noon, the worms are completely expelled. Subsequently, one liang of a harmonizing medicine should be taken to regulate the system; excessive dosage should be avoided. Generally, worms in the abdomen face upwards in the first ten days of the lunar month, lie horizontally in the middle ten days, and face downwards in the last ten days. Therefore, taking medicine in the middle or late periods fails to enter the worm's mouth, rendering it ineffective. This is analogous to cattle and horses giving birth: those born in the early period walk ahead of the mother, those in the middle walk alongside, and those in the late period walk behind. Similarly, when a cat eats a mouse, it consumes the front part in the early period, the middle part in the mid-period, and the rear part in the late period. This is the natural law of things, known to all. However, severe parasitic pain can be fatal even without penetrating the heart.
Previously, Qian Yi treated a five-year-old girl of the Xin family suffering from parasitic pain. Other doctors used drugs like Radix Euphorbiae Lathyri (Batu) and Lacca (Qisha) without success. After five days, the child cried frequently, tossed her body up and down, slept restlessly, pressed her heart and abdomen, screamed occasionally, displayed an abnormal complexion (green, yellow, white, or black), had dull eyes, pale lips, and vomited foam. On the sixth day, her chest was elevated while lying, and she became more restless. Qian Yi was summoned, examined her carefully, and administered three doses of Wuyi San (Trogopterus Dung Powder). Seeing that the greenish hue in her eyes persisted, Qian Yi was greatly alarmed, saying, "This illness is critical. If diarrhea occurs, it becomes a reverse syndrome (incurable)." The next day, he told Mr. Xin, "She had diarrhea at midnight." Looking into the chamber pot, Qian Yi saw liquid resembling medicinal juice and stirred it with a stick, revealing pill remnants. Qian Yi said, "This child has thick muscles and should have robust qi, but the current signs indicate extreme deficiency; it is untreatable." Why? The teacher explained, "Spleen deficiency and stomach cold cause worm movement. The greenish eyes indicate Liver Wood overacting on Spleen Earth. Coupled with diarrhea, we know the qi is extremely depleted. Moreover, the pills passed with feces indicate that the Spleen and Stomach qi have collapsed. Additionally, the physical manifestation is Pi Syndrome (masses), caused by dietary injury. The classic states: Ji (accumulation) is Yin Qi, Ju (gathering) is Yang Qi. Yin sinks and remains static; Yang floats and moves. Where Qi accumulates, it is called Ji, originating from the Zang organs, with fixed locations and pain confined to those areas. Where Qi gathers, it is called Ju, originating from the Fu organs, with no fixed root and wandering pain. Thus, Ji and Ju are both manifestations of Qi disorders. Pi Syndrome initially arises from Spleen deficiency and stomach cold, leading to undigested milk and food. If not treated early, stagnation and knotting form Pi Syndrome. 'Pi' refers to eccentric pathogenic factors residing in the hypochondrium; 'Zheng' refers to proper masses residing in the abdomen. Both have distinct shapes—long or short, lumpy or flat—and are hard and painful. Caused by nursing, it is called Ru Pi Zheng (Milk Pi Mass); commonly known as Nai Pi. Caused by food, it is called Shi Pi Zheng (Food Pi Mass). Caused by fluids, it is called Shui Pi Zheng (Water Pi Mass). If pressing or turning the body produces sounds, it is Water Pi Syndrome. Thus, Pi Syndromes are all caused by diet. Therefore, children should regulate nursing and adjust warmth and cold. Neglecting protection invites disease. Irregular nursing and improper temperature regulation lead to Spleen-Stomach disharmony and Qi stagnation. Symptoms include frequent mouth puckering, pale lusterless complexion, cold breath, and loss of appetite. Medication must be used to harmonize. If untreated, it becomes Stomach Cold, characterized by indigestion or vomiting only water. Urgent warming treatment is required. If still untreated, Spleen-Stomach gradually become deficient. Symptoms include pale weak complexion, abdominal pain, and vomiting white-green phlegm foam. Urgent warming and tonification are needed. If dysentery occurs, further conditioning is necessary. If symptoms do not respond to treatment, death is imminent. The child will become comatose after five days and die on the seventh. Qian Yi's formulas are recorded in his collected works. Students should take this as a model. Apart from Gan syndrome and parasitic movements detailed in the Gan Disease section, the following are various anti-parasitic formulas.
