Commentary: The *Ben Jing* records five types of Gan syndromes, named according to the affected Zang organs. Here, we discuss Kidney Gan, which has five specific manifestations. This is the most critical and urgent type, distinct from ordinary cases. This disease presents with five stages; the progression is swift and terrifying, justifying the name "Zouma Gan" (Running Horse Gan). Initially, the child has bad breath that rises to the stomach opening, with foul and stagnant qi; gradually, the tendons are damaged, sores appear on the gingiva, swelling or rotting occurs, and teeth turn scorched black; further progression leads to sores erupting from the alveolar bone, rupturing with pus and rot; even further, heat forces its way into the vessels, causing frequent bleeding; prolonged heat causes gingival decay, widening of the alveolar sockets, and tooth loss; by age six or seven, all teeth may fall out and never regrow—how can this be treated! Now, using this excellent prescription, it should be administered promptly, adjusting treatment according to the progression of the disease, rather than waiting for severe symptoms to appear before medicating.
A prescription for pediatric Zouma Gan (Running Horse Gan), where teeth ulcerate and rot, leading to bleeding from Pengsha (borax-like discharge) and tooth loss.
Qingfen (Calomelas), Lanxiang (Litsea Cubeba Fruit, powdered, each 1 qian), Mituosen (Lithargyrum, 0.5 liang, quenched in vinegar and ground into powder).
Grind all into a fine powder and apply topically to the teeth and gums. Immediate effect is expected. Commentary: Infants suffering from illness often present with Gan syndromes, largely due to Qi stagnation in the Sanjiao (Triple Burner). Gan is categorized by the Five Zang organs; involvement of the Kidney meridian often results from deficiency, leading to a condition termed "Ji" (Urgent/Emergent), metaphorically compared to a running horse, making treatment quite difficult. These syndromes progress as follows: initially, foul breath is called "Chouxi" (Foul Breath); next, teeth turn black, called "Pengsha"; when severe, gums rot, called "Kui Cao" (Ulcerated Socket); even more severe, bleeding occurs, called "Xuanlu" (Exposure/Bleeding); critically, teeth fall out spontaneously, called "Fugen" (Rotting Roots). Once the roots have rotted, how can they be repaired? Alas! Wealthy families raising children feed them rich and fatty foods, subjecting the Kidney palace to deficient heat. Alternatively, if the mother consumed strong-flavored foods during difficult labor, the child may contract this disease. None of these occur by chance. The secret prescriptions are listed below.
