Chao Shi Bing Yuan (Source of Diseases by Chao): The syndrome of pediatric zheng, pi, and jie (abdominal masses) arises from the disharmony of the five zang organs and the dysfunction of the san jiao (Triple Burner). When cold qi invades as an external pathogen, it causes milk and food to fail to digest, leading to condensation and aggregation, forming pi (fixed abdominal masses).
The manifestations are as follows: Masses that remain fixed upon palpation with distinct boundaries are called zheng (solid masses); masses that shift position when pushed, appearing floating or sinking, are called jia (mobile masses); masses that feel tense, tight, and pulling, located on either the left or right side, are called pi. All these conditions result from the condensation and aggregation of cold qi, phlegm-fluids, water-dampness, and dietary stagnation; hence they are collectively termed "zheng, pi, and jie."
Sheng Hui Fang (Prescriptions of the Holy Grace) discusses: When infants and children fail to regulate their exposure to cold and heat properly, and food fails to digest, it clashes with the qi of the internal organs, condensing and aggregating without moving, which is named zheng. If food stagnates in the abdomen, the child prefers cold environments, experiences chills in the four limbs resembling huoluan (cholera-like spasms), cannot eat, and often suffers from dull pain; this is classified as shi zheng (food zheng).
Ying Tong Bao Jian (Precious Mirror for Infants and Children) states: In pediatric jia syndrome, there is abdominal pain, but the location of the pain is not fixed; this is jia.
Yang Daye, Attendant Scholar Hanlin, asked: What causes lumps to form in a child's abdomen? The answer was: If children are careless with their diet, frequently consuming sticky, slippery, greasy, rich, fishy foods, and fruits, it leads to stomach qi disharmony. Food becomes solidified and difficult to digest, causing obstruction in the chest and diaphragm, attacking the ying (nutritive) and wei (defensive) qi. This results in frequent vomiting and regurgitation, coarse wheezing, dry eyes, and excessive thirst. If the pathogen descends to the lower part, it causes frequent diarrhea and distension in the hypochondrium. Treatment involves harmonizing the spleen and stomach, or using mild purgatives. If food fails to digest and gradually forms accumulation, generally, these disorders stem from improper maternal feeding. Medical intervention is required to ensure recovery morning and night. Failure to address the root cause will lead to death over time.
Wu Guan Guan Zhen Zhu Nang (Five Passages Piercing the Pearl Pouch) records the pediatric zheng, jia, and pi syndrome: Any occurrence of zheng, jia, pi, or aggregation is due to negligent care and irregular feeding, causing milk and food to condense and fail to digest, thus creating this disease.
Hui Yan Guan Zheng (Observations through the Wise Eye) notes: Some infants are born with zheng pi qi (pathogenic qi of masses); gan chong cuan xin qi (qi of intestinal parasites gathering at the heart); tai jing shan qi (fetal fright hernia qi); or bao xin diao qi (hugging-heart hanging qi). Zheng qi manifests as lateral movement across the body during attacks, severe pain under the ribs, and frequent vomiting. Urgent administration of decoctions or pills to promote bowel movement is needed (see formulas in the Acute/Chronic Convulsions section), followed by Yun Qi San (Qi-Harmonizing Powder) (see Disharmony of Stomach Qi section). Gan chong cuan xin qi presents with a dark, sallow complexion; administer vermifuge drugs. If thirst persists, administer Ta Qi San (Qi-Sinking Powder) (see Distension and Edema section) combined with Huang Lian Yin Zi (Coptis Decoction) (see same section). Tai jing gan qi presents with frequent night crying after birth or upward movement of kidney qi; treat with Kuan Qi Yao Shen Ling San (Qi-Relaxing Ginseng and Atractylodes Macrocephala Powder) (see Disharmony of Stomach Qi section), and apply Cong Xian Gao (Scallion Saliva Plaster) to the external genitalia (see Newborn Urination/Defecation Obstruction section). After waking, if sweating occurs and thirst is constant, use Huai Jiao Wan (Sophora Fruit Pill) (see Abdominal Pain section) and Kuan Qi Yao for two to three days to achieve bowel regularity and qi harmony. For infants under one year to two years old presenting with scrotal swelling immediately after birth, this indicates extreme heat received in the womb or paternal transmission. Treat first with Niu Huang Wan (Calculus Bovis Pill) (see Wind-Heat section), then harmonize qi. Initially apply You Mai Zi (Rapeseed Seed) powder mixed with cold water as a plaster. If unresolved, use Han Shui Shi (Halloysitum Rubrum), Bai Chan Tu (Kaolin), and sour vinegar for topical application. Long-term maintenance involves taking Yu Jin (Curcuma Radix), Gan Cao (Glycyrrhizae Radix et Rhizoma), and Ya Xiao (Natrii Sulfas) powdered and taken with honey water.
Ying Tong Bao Jian Pediatric Zheng Jia Song:
San jiao qi moves unsmoothly, cold qi accumulates severely in the chest. Distension, fullness, and coarse breathing occur; belching releases sour odors. Milk and food fail to digest, condensing into pi. Tangible masses are named zheng; floating ones are called ji (accumulation). Tense and rigid origins define pi; remember this clearly in your mind.
Wai Tai Mi Yao (Essential Prescriptions from the Imperial Library) cites Guang Ji Fang treating residual food in the abdomen of young children and adults, accumulating into zheng pi, causing hypochondriac fullness, rapid breathing, anorexia, sallow complexion, emaciation, and abdominal distension with hardness. It eliminates hundred diseases. Zi Shuang Wan (Purple Double Pill) formula (named Dai Zhe Wan in Sheng Hui Fang):
