After convulsions, paralysis may develop. As Zuo Qiuming stated, "Wind pathogen invading the extremities leads to disease," which pertains to the Liver channel. The Liver governs the sinews (tendons). When Liver heat is present, the sinews become relaxed and elongated; elongation results in weakness, causing the hands and feet to extend but not flex. When Liver cold is present, the sinews contract and shorten; shortening results in spasticity, causing the hands and feet to flex but not extend. In both cases, Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) should be used as the primary formula. For those with spasticity, add Fuzi (Aconite Lateral Root) and Rougui (Cinnamon Bark); for those with weakness, add Huangbai (Phellodendron Bark), Zhimu (Anemarrhena Rhizome), Danggui (Angelica Sinensis), Niuxi (Achyranthes Root), and Xuduan (Dipsacus Root), processed into honeyed pills for administration.
Following an attack of infantile convulsion, if aphonia (loss of voice) occurs, Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) should be used, added with Bajitian (Morinda Officinalis), Yuanzhi (Polygala Root), and Shichangpu (Acorus Tatarinowii Rhizome).
In my county, a magistrate surnamed Wu had a son who was four months old. He suffered from infantile convulsion. After the convulsions ceased, he fell into a coma and did not wake up; he only woke when convulsions recurred. His eyes deviated laterally, and his right hand twitched and spasmed. I was invited to treat him, but I said, "This is true convulsion (a critical condition), which is incurable," and thus declined the request.
The Heart governs fright. Excess patterns manifest as crying, screaming, fever, abnormal eating habits, and convulsions; deficiency patterns manifest as lethargy, drowsiness, palpitations, and restlessness.
Excess patterns are treated with Daochi San (Guiding Out Red Powder) and Xiexin Tang (Drain the Heart Decoction); deficiency patterns are treated with Anshen Wan (Calming the Spirit Pill).
Daochi San (Guiding Out Red Powder): Treats Heart heat, as well as reddish urine and night crying.
Ingredients: Shengdihuang (Rehmanniae Radix), Mutong (Akebiae Caulis), Gancao Shao (Glycyrrhizae Radix et Rhizoma tip), each in equal parts, crushed. Add Zhuye (Bambusae Folium), decoct in water, and take before meals. If Huangqin (Scutellariae Radix) is added, it is called Huofu San (Fire Mansion Powder).
Xiexin Tang (Drain the Heart Decoction): Highly effective for treating fright-heat.
Ingredients: Huanglian (Coptidis Rhizoma) (remove stamens, 1 qian), ground into fine powder. Take one zi to 5 fen per dose, mixed with warm water before bedtime.
Qian's Anshen Wan (Calming the Spirit Pill by Qian Yi): Treats fright-crying caused by evil heat, Xin Gan (Heart Gi) presenting with sallow complexion, red cheeks, and high fever.
Ingredients: Maimendong (Ophiopogonis Radix), Mayatiao (Natrii Sulfas), Baifuling (Poria), Shanyao (Dioscoreae Rhizoma), Hanshuishi (Calcitum) (prepared), Gancao (Glycyrrhizae Radix et Rhizoma) (each 5 qian), Zhusha (Cinnabar) (1 liang, prepared), Naozi (Borneolum Syntheticum) (1 zi). Grind all ingredients into powder, mix with honey to form pills the size of Euryale Seed (Qianshi). Take half a pill at a time, dissolved in sugar water, at any time.
Dongyuan's Anshen Wan (Calming the Spirit Pill by Li Dongyuan): The Classic states, "When heat dominates excessively, treat with sweet-cold herbs, drain with bitter herbs." Use the bitter-cold nature of Huanglian (Coptidis Rhizoma) to remove mental restlessness and damp-heat as the Sovereign herb; use the sweet-cold nature of Gancao (Glycyrrhizae Radix et Rhizoma) and Shengdihuang (Rehmanniae Radix) to drain fire, tonify Qi, and generate Yin blood as the Minister herbs; use Danggui Shen (Angelica Sinensis Root body) to supplement blood deficiency; use Zhusha (Cinnabar) to restrain floating fire and calm the Spirit.
Ingredients: Gancao (Glycyrrhizae Radix et Rhizoma) (5.5 qian), Huanglian (Coptidis Rhizoma) (6 qian, wine-fried), Danggui Shen (Angelica Sinensis Root body) (2.5 qian), Shengdihuang (Rehmanniae Radix) (1.5 qian), Zhusha (Cinnabar) (5 qian, water-prepared into powder). Grind all ingredients into powder, bind with steamed bread paste to form pills the size of Millet seeds, coat with Cinnabar. Take 10 to 30 pills per dose, washed down with warm water.
