The transmission of Wugu Gan (Innocent Malnutrition) has two causes: it may result from bird feathers or dirty clothes coming into contact with the infant's body, or it may be transmitted from a sick wet nurse. Symptoms include sores and lymph node swelling in the neck region, purulent discharge during defecation, emaciation due to parasitic erosion of the internal organs, yellowish complexion, fever, leading to the development of Gan syndrome. For clearing heat, Chaihu Yin (Bupleurum Decoction) is appropriate; for eliminating Gan, Fei'er Wan (Child-Nourishing Pill) is as effective as a miracle.
[Note] The etiology of Wugu Gan involves two aspects: one is exposure to night dew during laundry, where feathers from the "Wugu Bird" (a legendary toxic bird) contaminate the clothes, causing the disease when worn by the infant; the other is transmission from a sick wet nurse to the child.
Clinical manifestations include sores on the neck or elastic-sized nodules within the neck that are movable upon palpation, soft in texture, painless, containing worm-like substances resembling rice flour. If not promptly drained, these worms may erode the viscera, resulting in pus and blood in stool and urine, severe emaciation, yellowish complexion, and fever. Treatment should first focus on clearing heat, primarily using Chaihu Yin (Bupleurum Decoction); subsequently, to eliminate Gan accumulation, Lu Hui Fei'er Wan (Aloe Child-Nourishing Pill) is indicated.

[Prescription Song] Chaihu Yin treats Wugu Gan, Chi Shao, Chai Hu, and Chuan Huang Lian; Ban Xia, Jie Geng, and Xia Ku Cao, along with Long Dan, Zhe Bei, Huang Qin, and Gan Cao are decocted together.

When bones protrude all over the body, it is called Dingxi Gan (Dingxi Malnutrition). Muscles are dry and rough, with incessant crying day and night. Hands and feet are withered and thin, complexion is dark and sallow. The neck is thin while the abdomen is distended with umbilical hernia. The buttocks are wasted, the body is flaccid, and spirit is fatigued. There is bone-steaming tidal fever, thirst, irritability, and urgency. Treatment requires resolving stagnation and eliminating accumulation; for nourishment and spleen regulation, Ren Shen Qi Pi Wan (Ginseng Spleen-Initiating Pill) is suitable.
[Note] Dingxi refers to the condition where bones are exposed throughout the body, resembling the character "Ding," hence the name. Symptoms include dry and rough muscles, continuous crying, withered hands and feet, dark sallow complexion, thin neck with distended abdomen and protruding umbilicus, wasted buttocks, flaccid body, mental fatigue, bone-steaming tidal fever, dry mouth, thirst, irritability, and urgency. Initially, Wu Gan Xiao Ji Wan (Five-Gan Accumulation-Resolving Pill) is used to resolve stagnation, followed by Ren Shen Qi Pi Wan (Ginseng Spleen-Initiating Pill) to regulate the spleen, leading to gradual recovery.
