Qian Yi stated: For patients presenting with wheezing and expectoration of pus and blood, this is due to Lung Heat. Ganjie Tang (Glycyrrhizae Radix et Platycodi Decoction) should be administered after meals. For chronic coughs resulting in the depletion of Lung fluids, Ejiao San (Donkey-hide Gelatin Powder) is the primary treatment. (This refers to the Bufeisan mentioned earlier.) In cases of cough with phlegm-accumulation excess syndrome, without severe wheezing but accompanied by a red face and thirst, Pianyin Wan (Pianyin Pill) should be used to purge the pathogen. (The formula is cited earlier.)
The son of Duan Zhaolang, aged four, suffered from cough, fever, and expectoration of phlegm, which progressed to hemoptysis after several days. Previous physicians treated him with Jiegeng Tang (Platycodon Decoction) and Fangji Wan (Stephania Pill), but the condition did not improve. Saliva rose upward causing continuous vomiting and wheezing. Qian Yi was consulted and first administered a large dose of Pianyin Wan to purge, followed by treatment with Bufei Tang (Tonify Lung Decoction) and Bufeisan (Tonify Lung Powder). Someone asked, "The boy's hemoptysis indicates Lung deficiency; why use purgation?" Qian Yi replied, "Although there is hemoptysis, it is caused by heat. If prolonged, it leads to deficiency and flaccidity. Currently, saliva is surging upward causing vomiting; one must first purge the saliva. If not done, the vomiting of saliva will worsen the condition, as vomiting saliva further depletes healthy qi and may induce convulsions. Phlegm-heat excess attacking upward can also trigger spasms. Therefore, according to medical principles, one must first purge the phlegm-saliva and then tonify the Spleen and Lung. Only when the saliva stops and vomiting ceases does the treatment follow the favorable course (Shun). Tonifying the Lung first would be contrary to the proper order (Ni)." This illustrates the principle of recognizing the severity and priority of the disease to determine the sequence of treatment.
