[Xue Ji] The condition of "Fan Hua" (Flower-Flipping Ulcer) arises after an ulcer has ulcerated, where Wind-Cold pathogens invade the affected area, or due to Liver Fire causing Blood Dryness and generating Wind, or caused by the nursing mother's Liver Fire generating Wind. This inevitably leads to flesh protruding from the wound opening like a mushroom or a finger, varying in size and length. If caused by Wind-Evil invading, first use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) with Fangfeng (Saposhnikovia Root) and Tianma (Gastrodia Rhizome). If caused by stagnation of Wind-Cold, first use Shixuan San (Ten Acupoints Powder) with Qianghuo (Notopterygium Rhizome) and Tianma (Gastrodia Rhizome). If the infant has Liver Fire generating Wind, first use Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) with Tianma (Gastrodia Rhizome) and Qianghuo (Notopterygium Rhizome). If the cause is the nursing mother's Liver Fire generating Wind, first use Jiawei Xiao Chai Hu Tang (Modified Minor Bupleurum Decoction), followed by Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) with Lulu (Euphorbia Root) and Tianma (Gastrodia Rhizome). For cases invaded by Wind-Evil, apply Dou Chi Bing (Fermented Soybean Cake) externally. For cases with stagnant Wind-Cold, apply Cong Yun Fa (Scallion Moxibustion Method) externally, and further protect the wound with Taiyi Gao (Great One Ointment). If the protruding flesh does not subside, apply Li Lu Gao (Veratrum Ointment). If the wound fails to heal accompanied by aversion to cold and fever, this indicates deficiency of Yuan Qi (Original Qi); use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). If there is afternoon fever (Bu Re) and internal heat, this indicates deficiency of both Qi and Blood; use Bazhen Tang (Eight Treasures Decoction) with increased amounts of Renshen (Ginseng Root) and Baizhu (Atractylodes Macrocephala Rhizome). If there is poor appetite and difficulty digesting food, this indicates Spleen Qi deficiency; use Wuwei Yigong San (Five-Ingredient Extraordinary Merit Powder). If there is loss of appetite, irregular bowel movements, emaciation, and clear urine, this indicates a concurrent Gan Pi Gan Syndrome (Liver-Spleen Malnutrition Syndrome); use Jiuwei Lulin Wan (Nine-Ingredient Aloe Pill) to clear Liver Fire, and Wuwei Yigong San (Five-Ingredient Extraordinary Merit Powder) to tonify Spleen Qi, while continuing external moxibustion treatments.
Li Lu Gao (Veratrum Ointment): Treats protruding flesh at the wound opening or growths of two to three cun in length.
Li Lu (Veratri Radix) (quantity as needed) is ground into powder and mixed with fresh pork lard. Apply to the protruding flesh.
[Xue Ji] Duo Gu Ju (Multi-Bone Abscess) arises from long-standing ulceration leading to Spleen-Stomach damage, where Qi and Blood fail to nourish the affected area. Pathogenic factors sink and invade; over time, tendons rot and bones decay, causing bone exfoliation. This is not due to congenital fetal toxins. Treatment should focus on tonifying the Spleen and Stomach and strengthening Yuan Qi (Original Qi). Internally, use Dabu Tang (Great Tonification Decoction) and Dihuang Wan (Rehmannia Pill). Externally, use Fuzi Bing (Aconite Cake) and Cong Yun Fa (Scallion Moxibustion Method) to dispel Cold-Evil and replenish Yuan Qi. Then the bone will naturally exfoliate, and the wound will heal. If harsh purgative or attacking agents are used, damaging True Qi further, danger is inevitable. For infants suffering from this, the nursing mother should be regulated and tonified, and external scallion moxibustion should be applied for an extended period. Caustic or corrosive drugs must strictly be avoided.
