Tang's Niuhuang Wan: Treats tongjing (strabismus/esotropia) in children. This is often caused by accidental blows to the head, face, or forehead, or by falling and striking the body, causing the liver to be affected by wind-stroke due to fright, resulting in both eyes turning inward, known as tongjing. It is appropriate to take this prescription.
Ingredients: Niuhuang (Calculus Bovis), Baifuzi (Aconiti Kusnezoffii Rhizoma, prepared), Rougui (Cinnamomi Cortex), Quanxie (Scorpio), Xiong (Chuanxiong Rhizome), Shigao (Gypsum Fibrosum, each 1 qian), Baizhi (Angelicae Dahuricae Radix), Zhusha (Cinnabar, each 2 qian, ground), Huoxiang (Agastaches Herba, 0.5 liang), Shexiang (Musk, 1 fen).
Grind the above ingredients into fine powder, mix with refined honey to make pills the size of a Chinese quince seed. For children under three years old, administer one pill per dose, taken with mint decoction after milk or food. Avoid oily foods, wheat products, and pork.
[Xue Ji] Children with quemang (night blindness) and yan zha (eyelid twitching/frequent blinking). Vision clears after taking Zhu Gan Wan (Boiled Liver Pill), and eyelid twitching stops after taking Siwei Fei'er Wan (Four-Ingredient Child-Nourishing Pill). One child experienced eye twitching due to convulsions, belonging to wind-heat in the Liver and Gallbladder meridians. First treated the Liver with Chaihu Qinggan San (Bupleurum Liver-Clearing Powder), then tonified the Kidney with Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) for recovery. Another child had eyelid twitching and occasional convulsions due to fright. First cleared Liver heat using Jiawei Xiaochaihu Tang (Modified Minor Bupleurum Decoction) added with Yuan Yang (Chelidonii Herba) and Huang Lian (Coptidis Rhizoma), then used Liuwei Dihuang Wan to nourish the Kidney and generate Liver blood for recovery. Another child had continuous eye twitching, sometimes with redness or wiping of the eyebrows. This indicates wind-heat in the Liver meridian, likely developing into Ganchan (Liver Malnutrition Syndrome). Treated with Siwei Fei'er Wan added with Longdancao (Gentianae Radix) for cure.
Eye movement abnormalities, straight gaze, eye movements (see section on Fright and Convulsions).
Taihuang (Neonatal Jaundice) (see section on Newborns).
[Zeng Shirong] Infant urinary retention presents in two patterns: Yin and Yang. Yin Bi (Yin Obstruction) occurs when cold-dampness invades the interior due to deficiency, leading to urinary blockage. It is called Yin Bi. Treat with Baishaoyao Tang (White Peony Decoction) added with Nanmuxiang (Aquilariae Lignum Resinatum), and apply hot stir-fried salt wrapped in silk cloth to warm the area around the umbilicus. Simultaneously administer Wuling San (Five-Ingredient Powder) mixed with Lingsha (Cinnabar and Gold amalgam), taken with salt water on an empty stomach; the effect is particularly rapid. Yang Bi (Yang Obstruction) is caused by sudden heat forcing fluids, leading to scanty and difficult urination, called Yang Bi. There is also Longbi (Difficult Urination), which differs from Linzheng (Strangury). The *Neijing* states in the "Xuanming Wuqi" chapter: "Bladder dysfunction is Long." Long refers to internal Qi deficiency with heat stagnation, where transformation and diffusion fail. It is not painful or burning, but simply blocked, with abdominal distension and tightness. Simply use Juyu Wuling San (Crushed Five-Ingredient Powder) added with Cheqianzi (Plantaginis Semen) and Dengxincao (Junci Medulla), or administer Mutong San (Akebia Decoction), Yulu Yin (Jade Dew Drink), or Yi Yuan San (Benefit Origin Powder). Applying Jiangchi Bing (Ginger and Fermented Soybean Cake) to the umbilicus can also be effective. This treats both Yin and Yang patterns. Combined with Wan'an Yin (Ten Thousand Safety Drink), it is excellent.
[Xue Ji] Li Dongyuan stated: Urinary difficulty has distinctions between Qi and Blood levels. Urine is governed by the Foot-Taiyang Bladder Meridian. Its Qi originates at Shen hour (3-5 PM), which corresponds to Metal. Metal generates Water. If latent heat exists in the Lung, Water cannot be generated, cutting off the source of urine. The treatment method uses light, thin-aroma herbs to clear dryness and regulate Qi, such as Fuling (Poria), Zhuling (Polyporus), Zexie (Alisma), Hupo (Amber), Dengxincao (Junci Medulla), Tongcao (Tetrapanacis Medulla), Cheqianzi (Plantaginis Semen), Qumai (Dianthi Herba), and Bianxu (Polygoni Avicularis Herba). These are all bland and leaching agents that drain Lung heat and nourish the source of Water generation. If there is no thirst, the heat is in the Lower Jiao, blocking the flow and preventing urination. One must use heavy-aroma, Yin-within-Yin herbs to treat this. These two conditions differ: one is in the Upper Jiao, involves Qi, and causes thirst; the other is in the Lower Jiao, involves Blood, and does not cause thirst because dampness is present in the Blood. The distinction is easy to identify. I suggest that if fluids leak into the intestines causing diarrhea and scanty urine, use separation and drainage methods. If heat accumulates in the Lower Jiao drying fluids and stopping urination, use渗泄 (percolating and draining) methods. If Spleen and Stomach Qi is stagnant failing to regulate water passages, use Qi-regulating methods. If the nursing mother has heat in the Liver and Heart meridians, use Zhizi Qinggan San (Gardenia Liver-Clearing Powder). If Liver fire flares, use Chaihu Zhizi San (Bupleurum Gardenia Powder). If caused by parents' prior use of drying agents, use Siwu Tang (Four-Substance Decoction), Maimendong (Ophiopogonis Radix), and Gancao (Licorice). If pulse is rapid and urine yellow, use Siwu Tang added with Shanzhuyu (Corni Fructus), Huangbai (Phellodendri Cortex), Zhimu (Anemarrhenae Rhizoma), Wuweizi (Schisandrae Fructus), and Maimendong. If Lung deficiency causes short urine, use Buzhong Yiqi Tang (Middle-Wardening Qi-Supplementing Decoction) added with Shanyao (Dioscoreae Rhizoma) and Maimendong. If prolapse, impotence, and frequent urination occur, use Dihuang Wan (Rehmannia Pill). If heat binds the Bladder causing difficulty, use Wulin San (Five-Strangury Powder). If Spleen and Lung dryness prevents transformation, use Huangqin Qingfei Yin (Scutellaria Lung-Clearing Drink). If Bladder Yin deficiency fails to generate Yang causing dribbling, use Zishen Wan (Kidney-Nourishing Pill). If Bladder Yang deficiency fails to transform Yin causing dribbling, use Liuwei Wan (Six-Ingredient Pill). If caused by the nursing mother consuming rich flavors and alcohol accumulating heat, use Qingwei San (Stomach-Clearing Powder) and Wulin San. Refer to various Strangury patterns for reference.
One fourteen-year-old child presented with limb fatigue, fever with afternoon tidal fever, dry mouth and thirst, profuse phlegm, dribbling urine, sometimes red face and eyes, irritability, and aversion to clothing. This was congenital Kidney deficiency causing Yin restlessness. Treated with Buzhong Yiqi Tang and Jiawei Bawei Wan (Modified Eight-Ingredient Pill) for recovery.
One five-year-old child had urinary difficulty. After taking Wuling San and other draining agents, the condition worsened, and the genital area and scrotum swelled. My elder brother said the previous medicines damaged True Yin further. Used Liuwei Dihuang Wan material added with Niuxi (Achyranthis Bidentatae Radix), Rougui (Cinnamomi Cortex), and Cheqianzi, assisted by Buzhong Yiqi Tang for recovery.
One eight-year-old child initially had sticky urine. Taking Wuling San worsened it. Adding Mutong and Cheqianzi led to abdominal distension and vomiting of phlegm. Adding Zhike (Aurantii Fructus) and Haijinsha (Lygodii Spora) led to chest fullness, genital swelling, and generalized edema. I used Liuwei Dihuang Wan decoction to swallow Zishen Wan for recovery. All these cases resulted from inheriting the father's constitution. Treating them as Damp-Heat or Phlegm-Qi led to many deaths.
One eight-year-old child initially had yellow-red urine. After taking Wuling San and Daochi San (Guide Out Red Powder), he developed hematochezia. I considered this congenital deficient heat. Treated with Liuwei Dihuang Wan and Buzhong Yiqi Tang for recovery.
