Sweat is the fluid of the Heart. When Heart Yang (Heart Fire) secures and retains it, internally it becomes Blood, and externally it flows out as sweat. If injured by external pathogens, sweat floats on the surface. By inducing sweating to expel sweat, the pathogen subsides with the sweat, and the disease must be cured; this is the method to remove the cause of illness.
Sweating itself can become a pathology.
Diseases often invade through the sweat pores. Spontaneous sweating (Zi Han) and night sweating (Dao Han), these two require investigation to find their origin. Yin deficiency leads to Yang encroachment, causing fever and thick/sticky sweat; Yang deficiency allows Yin to overpower, causing cold limbs (Jue) and sweating. When both Heart and Kidney are weak, spontaneous sweating is worrying. With loose interstices (Cou Li) and tender skin, pathogenic heat invades internally and contends with healthy qi. Heat combats the Master of the Heart, and fluids fail to be retained internally. Sweating occurs during sleep but stops upon waking. Or, due to injury from cold or heat, Yin and Yang interact, causing body fluids to leak and spread throughout the muscles and body. These all belong to night sweating, where only deficiency is sought as the root.
Irregular diet (hunger or fullness) causes Stomach sweat to escape externally. Fright depletes Essence, causing Heart sweat to flow profusely. Carrying heavy loads and traveling far causes Kidney sweat to flow leisurely. Running fast and feeling fear causes Liver and Gallbladder sweat to wander. Forceful labor and hardship cause Spleen sweat to whistle forth. The Classics state that Zang-Fu organs differ from one another. In summary, chronic sweating diseases lead to physical frailty. Symptoms include bone-steaming tidal fever, infantile malnutrition (Gan syndrome), consumption, fright convulsions, and muscle twitching. Yellowish emaciation, fatigue, and weakness, with cold sweat like oil being rubbed. All such complicated symptoms are indeed distressing. Others include: Yang collapse, where sweat is only on the head; Heart emptiness sweat, lingering at the heart region; death-bound sweat, resembling beads or oil. Judgment depends on the disease, and medical methods then prove superior.
Qian Yi said: Sweat due to deficiency of the Six Yang channels reaches up to the vertex but does not exceed the chest. It requires no treatment.
Zeng Shi said: For children with Spleen deficiency and spontaneous sweating, it mostly appears on the forehead, sticking to human hands. One must quickly rescue Stomach Qi using Chenxiang Yin (Aromatic Wood Drink). For Spleen deficiency diarrhea with spontaneous sweating, the whole body feels cold and sweating occurs intermittently; there is no sweating during diarrhea, but this symptom appears immediately after diarrhea passes. Severe deficiency urgently requires tonifying the Spleen, using Yihuang San (Yellow Benefit Powder) or Shenling Baizhu San (Ginseng, Poria, and Atractylodes Macrocephala Powder). For Lung deficiency spontaneous sweating, the right cheek color is often pale white, and the Lung pulse is powerless upon palpation. This is likely due to prolonged coughing without relief, causing phlegm stagnation and inactivity, leading to deficient Qi rising and obstructing the Lung channel, resulting in sweating. One should use Bufei San (Lung Tonification Powder) and regulate the Spleen with Huoxiang Yin (Agastache Drink). This embodies the principle of "benefiting the mother to save the child" (tonifying Earth to generate Metal). For slow convulsion with spontaneous sweating, it covers the entire body, feeling cold as ice. This condition is critical; use Jinye Dan (Golden Liquid Pill) or Guzhen Tang (Solidify True Decoction). For excess-type spontaneous sweating caused externally by wind-pathogen invasion and fever, whether unconscious or awake, with oozing sweat, one must urgently rescue the exterior and release the muscle layer, using Baijie San (Hundred Resolutions Powder). For Stomach timidity sweat, reaching from the vertex to the navel, this is Stomach deficiency, requiring Stomach tonification, using Yihuang San.
