The Lung is the "Canopy" (Hua Gai), covering the other organs. It has a exterior-interior relationship with the Large Intestine. The Large Intestine has an exit, and the anus is this opening. The anus is essentially the end of the intestine, belonging to the internal organs. Its Qi flows freely, circulating and transporting. If the Lung has Excess Heat (Shi Re), the intestines become constipated; if the Lung has Deficiency Cold (Xu Han), the rectum prolapses and appears elongated. Whether Deficiency Cold or Excess Heat, these are the general principles. The causes of prolapse should be examined in detail. Wind Wood overacting on Earth damages the Spleen and Stomach. Summer-heat, Dampness, Wind, and Heat gather there. When Clear and Turbid Qi mix, severe diarrhea ensues. Over time, the intestines become deficient, lacking strength for transportation. Attacked by Wind-Cold, anal prolapse occurs. Furthermore, chronic dysentery and straining hinder normal closure. Congenital weakness and insufficient Shen-Qi can also cause prolapse. The disease is not confined to the anus; there is also anal pruritus. This results from excessive consumption of sweet and fatty foods, generating Damp-Heat that stagnates, which is unfavorable. Damp-Heat transforms into toxicity, producing parasites that erode the anus, causing itching and even palpitations. Accompanied by bloody stools and fever, the patient feels distressed and panicked, emaciated with sores all over the body. Treatment should focus on clearing the Stomach and eliminating Heat, while supporting the Spleen. Internal and external treatment combined is the key method. If parasites erode the anus and penetrate internally, the child will surely die young.
Chao Yuanfang stated: The Lung and Large Intestine are exterior-interior pairs; the anus is the gate of the Large Intestine. Lung Excess Heat leads to constipation and blockage; Lung Deficiency Cold leads to the protrusion of the rectal tip. Some cases arise from chronic dysentery with abdominal urgency and tenesmus, where straining opens the anus, allowing external Wind to blow in. Or, latent summer-heat causes diarrhea with intestinal slipping and incontinence. Or, congenital weak constitution makes one susceptible to Cold, leading to Large Intestine deficiency and prolapse. All pediatric diarrhea and dysentery result from Summer-heat, Dampness, Wind, and Heat pathogens taking advantage of Spleen-Stomach deficiency. The treatment method should tonify the Spleen and warm the Stomach, allowing Metal (Lung) to receive benefit from its Mother (Earth/Spleen), thereby strengthening Qi. Huoxiang Yin (Agastache Decoction) is suitable. Next, administer internal agents to secure the intestines, such as Jianpi Yin (Spleen-Tonifying Decoction) and Yangzang Tang (Nourishing Viscera Decoction). Externally, apply Fulonggan San (Dragon's House Soil Powder) to facilitate natural retraction.
Chen Cangqi stated: Pediatric anal pruritus may result from indulging in sweet and fatty foods, causing Damp-Heat stagnation in the Large Intestine, or damp-toxicity generating parasites that erode the anus. If due to Damp-Heat stagnation, Siwei Fei'er Wan (Four-Ingredient Child-Nourishing Pill) is suitable. For constipation, Qingliang Yin (Cooling and Clearing Decoction) is used. For parasitic erosion of the anus, use Huachong Wan (Parasite-Dissolving Pill) first, followed by Siwei Fei'er Wan. Externally, insert Xionghuang San (Realgar Powder) into the anus. If the illness prevents eating, parasites starve and turn to consuming viscera or biting the anus. In this case, the gums are colorless, the tongue is entirely white, limbs are fatigued, ulcers appear inside the upper lip, and hematemesis resembles millet grains, accompanied by mental restlessness. This indicates parasites feeding on viscera above. If ulcers appear on the lower lip, it indicates parasites feeding on the anus below. If parasitic erosion penetrates internally, it is incurable.
