Xia Chuming stated: When phlegm excess causes convulsions (Jing), death may occur immediately upon onset, with loss of consciousness, mild spasms but severe fever. This is spleen phlegm invading the lungs; Tianbao Caiwei Decoction (Tianbao Caiwei Tang) should be used, with Banxia (Pinelliae Rhizoma) increased by 3 to 4 qian, Qianhu (Peucedani Radix) increased by 2 qian, Cangzhu (Atractylodis Rhizoma) increased by 1 qian, and Xiangru (Elsholtziae Herba) added in summer.
Xia Chuming stated: When wind excess causes convulsions, although the patient may appear dead from fright, revival can be achieved instantly by pinching the Jingwei acupoints. If spasms of the hands and feet persist, occurring dozens of times in a day and night, this is liver wind invading the sinews. Tianbao Caiwei Decoction (Tianbao Caiwei Tang) should be used, with Qianghuo (Notopterygii Rhizoma et Radix) and Chaihu (Bupleuri Radix) each increased by 1 qian, and Banxia (Pinelliae Rhizoma) increased by 3 qian. In summer, add Xiangru (Elsholtziae Herba) 1.5 qian.
Xia Chuming stated: When heat excess causes convulsions, the presentation is no different from wind-phlegm, except for excessive thirst. This is external pathogenic factors invading the lungs. Tianbao Caiwei Decoction (Tianbao Caiwei Tang) should be used, with Gange (Puerariae Lobatae Radix) and Jiegeng (Platycodonis Radix) each increased by 2 qian.
Apart from the above three conditions, there are convulsions originating from the Heart meridian, characterized by the tongue protruding, red and dry in color, caused by heat within the heart organ itself.
Tuina (Pediatric Massage) techniques: Open Tianmen (Frontal Bone Push), Divide Yin and Yang. For boys, push down the Six Fu organs on the left hand; for girls, push up the Three Passes on the left hand. For both genders, perform Eight Trigrams rotation in the center of the left palm, pushing from Kan to Gen, perform "Fishing for the Moon at the Bottom of the Water" in the palm, guide water to the Heavenly River, then administer Lianqiao (Forsythiae Fructum) 2 qian, Huanglian (Coptidis Rhizoma) 0.5 qian, Mutong (Akebiae Caulis) and Fangfeng (Saposhnikoviae Radix) 1 qian each, decocted in water and taken orally. Add Xiangru (Elsholtziae Herba) in summer.
Xia Chuming stated: In cases of convulsions where the child appears dead for one or two days, or does not revive after three to seven days, or revives but remains unconscious, with or without spasms, urgent moxibustion (three moxa cones) should be applied to Feishu (BL-13). If revival occurs instantly upon pinching, moxibustion is unnecessary. Instead, apply specific warming methods (Yuanxiao Huo) and administer Tianbao Caiwei Decoction (Tianbao Caiwei Tang) continuously for three to four doses, increasing Banxia (Pinelliae Rhizoma) to 4 or 5 qian. There have been no failures in recovery. In summer, add Xiangru (Elsholtziae Herba) 1.5 qian.
Xia Chuming stated: Occasionally, there are cases where there is no phlegm obstruction in the lungs, and pinching produces no sound, nor is there head shaking or eye widening; these are also livable cases. This results from prolonged illness treated chaotically with improper massage, pinching, and other interventions, exhausting lung qi to the point of aphonia. Understanding the cause of voice loss, one must absolutely avoid further massage, pinching, fire therapy, or moxibustion. Only Tianbao Caiwei Decoction (Tianbao Caiwei Tang) should be administered in equal parts; one dose leads to cure. In summer, add Xiangru (Elsholtziae Herba) 2 qian.
Xia Chuming stated: Occasionally, children suffering from convulsions lie drowsy and stiff, unable to regain consciousness but able to turn over independently, with flexible limbs. They may remain unresponsive for about ten days yet survive. This condition resembles Rou Ji (Flaccid Spasm). Tuina is unnecessary; only pinching is required. Pinch the Jingwei, Dadun (LR-1), Zhongchong (PC-9), Jianjing (GB-21) acupoints, and rub the Five Finger Joints and Feishu (BL-13) acupoint. For each of the aforementioned acupoints, apply one moxa cone using a lamp-fire technique. Administer Miduoseng (Lithargyri Pulvis) one zi (approx. 2.5 lian, finely ground and vinegar-processed) mixed with half a cup of water and wine. Then administer Tianbao Caiwei Decoction (Tianbao Caiwei Tang) with increased Jiegeng (Platycodonis Radix) and Banxia (Pinelliae Rhizoma) to achieve revival. In summer, add Xiangru (Elsholtziae Herba) 2 qian.
Xia Yuzhu stated: Regarding the aforementioned convulsion cases, my father only described the physical manifestations and did not discuss facial complexion or orifices. This is because at the moment of apparent death from fright, the face, eyes, lips, and tongue do not display their true colors. Attempting diagnosis via inspection of complexion would result in indistinguishable clarity and turbidity, leading to chaotic treatment strategies akin to driving south while aiming north. Therefore, differentiation based on physical form is essential to avoid errors.
