**Lung Heat:** Redness on the right cheek, which becomes more intense during the Shen and You hours (3:00 PM to 7:00 PM); or constipation; or fever; or rapid breathing with coughing; or inability to expectorate phlegm resulting in a congested, red face without sputum; or thirst and rough breath; or dryness and cracking of the skin around the nostrils with sores. These are all manifestations of Lung Heat (Fei Re). Treatment is indicated with Xiebai San (White Bark Decoction).
**Lung Deficiency:** Pale and dull complexion, with no external signs of cold pathogen invasion. Treatment is indicated with Wuweizi (Schisandra Fruit), Baizhu (Atractylodes Macrocephala Rhizome), Baifuling (Poria), Chenpi (Citrus Reticulata Peel), Kuandonghua (Kalanchoe Flower), and Gancao (Licorice Root).
**Lung Cold:** Treatment uses Shensu Yin (Ginseng and Perilla Decoction). The treatment for Lung Cold and for conditions where extreme cold mimics heat is discussed in the subsequent section on Cold Damage Diseases (Shanghan Lei).
**Heart Heat:** Red and dry forehead; or red-purple tongue; or red and swollen tongue; or dark red urine that is difficult and painful to pass; or high fever. These are all manifestations of Heart Heat (Xin Re). Tuina techniques include moving the palm from Kan to Gen positions, draining the six fu organs, "Fishing for the Bright Moon at the Bottom of the Water," and "Guiding Water Up to the Heavenly River." Treatment uses Xijiao Jiedu Tang (Rhinoceros Horn Detoxification Decoction).
**Deficiency Heat:** Pale complexion, dull lips and tongue, clear and profuse urination, and thirst. Treatment uses Ren Shen (Ginseng), Bai Fushen (Poria with Hostwood), Yuanzhi (Polygala Root), Baizhu (Atractylodes Macrocephala Rhizome), and Gancao (Licorice Root). Self-healing occurs after three to four doses.
If the forehead is not red, the tongue is not swollen or red, and urination is not dark and difficult, it indicates that heat is stagnant in the heart but has not manifested in the orifices or reflected in the complexion. Burning breath, teeth grinding like sawing wood during sleep, delirious speech, and excessive tearing while crying indicate "Extreme Heat Mimicking Cold" (Re Ji Si Han). Tuina methods are the same as for Heart Heat. Medication involves drinking water in which Xijiao (Rhinoceros Horn) has been ground, or using Dalianqiao Yin (Large Forsythia Decoction).
Symptoms include low-grade fever, appearance resembling convulsions but without actual spasms, and unconscious confusion. Treatment uses Banxia (Pinellia Tuber), Chenpi (Citrus Reticulata Peel), and Jiegeng (Platycodon Root), each two qian, decocted and taken orally. Subsequently, Gancao (Licorice Root) three qian, Lianqiao (Forsythia Fruit), and Mutong (Akebia Stem), each one qian, are taken to achieve self-healing.
Jiusixuan's Da Fang Lin Cui was two months old when his condition involved spleen dampness stirring up phlegm that entered the lung orifices. His wife summoned a practitioner to perform tendon-picking, which exacerbated the condition. It was claimed that picking too late made it incurable. Upon examination, I noted the child had no voice when grasping objects. Auditory assessment confirmed phlegm entering the lung orifices. Initially, tuina combined with moxibustion (denghuo) was used. Then, Banxia (Pinellia Tuber) two qian, along with Jiegeng (Platycodon Root), Zhike (Bitter Orange), and Baizhu (Atractylodes Macrocephala Rhizome) each one qian were administered, leading to slight improvement. Lin Cui’s father, hearing my advice, heavily relied on Banxia due to its symptomatic match, which triggered mild dryness-qi reaction. I subsequently used Chishao (Red Peony Root) and other herbs to resolve it, resulting in full recovery. This verifies that for spleen-dampness-induced phlegm, tendon-picking is ineffective, whereas herbal medicine yields immediate results.
My eldest daughter was just four years old. She had a slight fever, spoke nonsensical words indiscriminately, had a purplish-yellow complexion, slightly red lips, a uniquely dry tongue tip, and burning breath. Her spirit was not weak, nor did she feel thirsty. Based on her complexion, it appeared to be Summerheat Disease (Zhongshu). However, since it was the third month, summerheat was not the prevailing pathogen. Inferring from the dry tongue tip, burning breath, and mental state, I determined it was "Extreme Heat Mimicking Cold." One cup of water in which Xijiao (Rhinoceros Horn) was ground was administered. After a short while, vomiting occurred, and she recovered immediately. This serves as a verification of diagnosing "Extreme Heat Mimicking Cold" by examining orifices and probing the condition.
