Whenever preparing a plaster (Gao), regardless of whether official or herbal ingredients are used, they must be washed and finely chopped. Whether decocted for juice or pounded for juice, the liquid must be clarified and filtered to remove sediment, retaining only the clear supernatant. This clear liquid is placed in a jar and boiled down until three-tenths have evaporated, leaving seven-tenths. It is then transferred to a thin porcelain vessel, stewed over water until concentrated, and finally mixed with Aloe powder (Luhui) to dry into a paste. The consistency—whether thicker or thinner—should be adjusted according to necessity. However, if burnt, it loses efficacy; if too soft, it cannot be consolidated.
Pearl (Zhenzhu), Amber (Hupo), Agate (Manao), and Coral (Shanhu) are difficult to grind. Ancients sometimes used fire calcination, which facilitates crushing but destroys their true nature and makes them overly drying, rendering them unusable. Water-grinding removes fine dust but causes excessive loss. A better method is to wrap the material in several layers of cloth, crush it with an iron hammer, spread it out, select the fine particles, place them in a mortar, and gently grind slowly before sieving finely. This preserves their true nature with minimal loss.
All medicinal ingredients intended for the formula should be pre-ground fine, weighed, and packaged. The prepared plasters should be stored in separate vessels, fully prepared. Then, weigh each powdered ingredient accurately and gather them together. Sift them again to remove coarse particles. Next, place cooked honey (Shu Mi) into a vessel, add the weighed plaster, and melt/mix them evenly. Add the fine medicinal powders and stir until well combined. Finally, add Lead Tetroxide (Huandan) and Musk (Shexiang), consolidate the plaster, and bury the vessel in a cellar to eliminate "fire toxicity." Then transfer to a porcelain jar for storage; the longer it ages, the better. Immediate use results in a harsh medicinal effect.
The treatment of eye diseases is the most difficult aspect of medication application. Excessively hot herbs cause dispersion of pathology, while excessively cold herbs cause coagulation/stagnation. Therefore, Borneol (Bingpian) is contraindicated, and Coptis Rhizome (Huanglian) is used sparingly. External eye drops should primarily focus on melting and收敛 (astringing/consolidating) light energy. Internal decoctions should prioritize dispersing and breaking up Qi stagnation. For initial onset, reduce Fire and dispel Wind; for chronic conditions, clear nebulae (Yi) and brighten vision. Sudden tonification or sudden purgation both damage the eyes. Do not seek haste, do not act recklessly; exercise caution.
Eye medications often fail to produce rapid results, so few patients take internal medicine. An ancient saying states: "Dropping water into a pot is inferior to removing firewood from the stove bottom." Applying eye drops and taking oral medicine represents the principle of internal-external correspondence and synergy. Taking medicine before meals is ineffective. Taking it after meals is also suboptimal because during digestion/movement, the medicinal action scatters, failing to achieve therapeutic effects. Only when taken just before sleep, lying down with eyes closed, allowing spirit to settle inward, does the medicinal property rise and navigate through the orifices, thereby effectively treating both eyes.
