Q: What is the treatment for sudden entry of foreign filaments from outdoors into the eye, causing unbearable pain? A: One may take Tangjian San (Sugar-Scattered Powder), apply Yulu Shuang (Jade Dew Frost), and it will heal in one or two applications. Alternatively, use Jingmo Zhi (Beijing Ink Juice) as eye drops. If wheat awns or rice husk fragments enter the eye, wash with decoction made from large barley. For dust or debris entering the eye, wash with water mixed with table salt. For "sky silk" (such as willow catkins) entering the eye, apply blood from an eel into the eye, then remove it by rolling with a rush pith (Jiedixin). The patient will recover immediately. (Note: The formula for Yulu Shuang is not recorded later; this remains doubtful.)
Tangjian San (Sugar-Scattered Powder)
Fangfeng (Saposhnikoviae Radix), Dahuang (Rhei Radix et Rhizoma), Danggui (Angelicae Sinensis Radix), Chishao (Paeoniae Radix Rubra), Jingjie (Schizonepetae Herba), Gancao (Glycyrrhizae Radix et Rhizoma), Niubangzi (Arctii Fructus), Chuanxiong (Ligustici Chuanxiong Rhizoma) (each 5 qian), Dancao (Gentianae Radix) (4 qian).
Decoct in water, add brown sugar, and take orally.
Q: Regarding the formation of corneal opacities (Yi), there are favorable (Shun) and unfavorable (Ni) patterns. Why do some cases present with excessive tearing and photophobia? A: This results from Cold invading the Stomach, Heat affecting the Spleen, and deficiency of both the Heart and Kidney. It is suitable to take Buxin San (Heart-Nourishing Powder) (see Question 6), Bushen Wan (Kidney-Tonifying Pill) (see Question 11), and apply Tuiyun San (Cloud-Pushing Powder) (see Question 14). In the Bushen Wan mentioned in Question 11, Chenxiang (Aloewood) is used for its aromatic nature to enter the Spleen meridian; its warmth does not aggravate Fire.
Q: What causes an ocular obstruction resembling a crescent moon accompanied by pain? A: This is due to insufficiency of the Liver Meridian and excess of Lung Qi, resulting in Metal overcoming Wood (Lung Metal overacting on Liver Wood). It is suitable to take Tangpao San (Soup-Washing Powder) and concurrently take Bushen Wan (Kidney-Tonifying Pill) (see Question 66; the Bushen Wan in Question 66 contains Xuanfuhua (Inulae Flos) and Wuweizi (Schisandrae Fructus) to target the Lung).
Tangpao San (Soup-Washing Powder)
Danggui (Angelicae Sinensis Radix), Chishao (Paeoniae Radix Rubra), Huanglian (Coptidis Rhizoma), Chantui (Cicadae Periostracum).
Grind all into powder. Steep in boiling water. Take 2 qian per dose. While hot, fumigate the eyes with the steam, and take the warm liquid orally.
Q: What causes internal cataracts (Neizhang) where the opacity gathers and disperses like clouds obscuring vision? A: This is caused by residual toxins from the Five Viscera, with Wind-Heat attacking upward, leading to fluctuating symptoms. It is suitable to take Yanggan Wan (Sheep Liver Pill). Apply Tuiyun San (Cloud-Pushing Powder) (see Question 14) and also use aqueous eye drops.
Yanggan Wan (Sheep Liver Pill)
Yanggan (Sheep Liver, one whole, washed and free of fascia), Huanglian (Coptidis Rhizoma) (3 liang), Danggui (Angelicae Sinensis Radix) (1 liang), Ruiren (Pruni Semen Persicae/Myrobalan? Note: Usually Prunus persica seed or Terminalia chebula, here likely Prunus persica seed or similar nut-based seed used in ophthalmology, often translated as Prunus Seed or Myrobalan depending on context, but strictly Ruiren is often Prunus persica seed or Terminalia chebula. In modern TCM, Ruiren usually refers to Prunus persica seed or Terminalia chebula. Given the context of clearing heat and improving vision, and pairing with liver, it might be Prunus persica seed. However, standard WHO term for Ruiren is often associated with Prunus persica seed or Terminalia. Let's use Prunus Seed (Ruiren) or Myrobalan. Actually, Ruiren in ophthalmology often refers to *Terminalia chebula* fruit or *Prunus persica* seed. Given "qu you" (remove oil), it suggests a seed/nut. *Prunus persica* seeds contain oil. *Terminalia* does not typically require oil removal in the same way. Let's assume Prunus persica seed (Prunus Seed) or simply keep Latin if ambiguous. Standard Ruiren is often *Prunus persica* seed. Let's use Prunus Seed (Ruiren)). (Remove oil, 1 liang).
First, boil the sheep liver in a clay pot until soft. Then add the powders of Huanglian and others. Mash together to form pills, sized like tung tree seeds. Take 50 pills per dose, washed down with rice soup.
