Cao (from Xishan) traveled long distances under the scorching sun, causing heat to enter the collaterals and disturb the blood. It manifested in winter, resulting in physical and mental restlessness, with hyperactive Yang and deficient Yin.
Facial swelling accompanied by asthma, incessant choking cough, and gradually hoarse voice; the descent of Qi throughout the body relies entirely on the Lung. In chronic drinkers, dampness accumulates for a long time, generating heat within the dampness. The heat inevitably steams upward, causing Lung heat to lead to swelling and asthma, resulting in blocked voice. According to *Neijing*: "When dampness overflows internally, treat it with bland and leaching agents, assisted by bitter-warm herbs." Leaching causes dampness to flow downward; drinkers dislike sweet flavors and should prefer bitter ones; warmth promotes the circulation of dampness, as dampness is a Yin pathogen. (If dampness does not descend but gathers in the middle Jiao, transforming into heat that steams upward, scorching Lung Qi and causing rebellion, with loss of Qi descent, such symptoms appear.)
Prescription: Fresh Reed Rhizome (*Imperatae Rhizoma*), Coix Seed (*Coicis Semen*), Magnolia Bark (*Magnoliae Officinalis Cortex*), Talcum (*Talcum*), Zhejiang Poria (*Poria Cocos* with Chuanbei), Apricot Kernel (*Armeniacae Semen Amarum*).
All six herbs are substances that direct Qi downward and leach out dampness.
Zou (46 years old): Pungent flavor enters the Kidney, but the Kidney dislikes dryness. Whenever pungent flavor enters the Blood level, if pungent tonics are used to move Qi, excessive draining and purging will damage Kidney Yin. In the prescription, Epimedium (*Epimedii Folium*) drains dampness, Pinellia (*Pinelliae Rhizoma*) and Polygala (*Polygalae Radix*) are pungent and drying. From the Yang level, they directly drain Qi downward. Ginseng (*Ginseng Radix*) and Schisandra (*Schisandrae Fructus*) generate fluids, yet they are also overwhelmed by the fierce attack of pathogenic drugs. Why is one so confused? (This case lacks detailed description of disease presentation, pulse, and symptoms; it merely investigates the wrongly prescribed drugs, suggesting it was likely a Yin deficiency syndrome.)
Prescription: Ginseng (*Ginseng Radix*), Poria with Hostwood (*Poria Cocos with Hostwood*), Asparagus Tuber (*Asparagi Radix*), Prepared Rehmannia Root (*Rehmanniae Radix Praeparata*), Schisandra (*Schisandrae Fructus*), Biota Seed (*Biotae Semen*).
Pounded with pig kidney to make pills.
Xiang (27 years old): Hemorrhage followed by abdominal pain resembling hunger pangs indicates injury caused by overexertion. Upon seeing bleeding, cool-natured hemostatic drugs were administered in hopes of stopping the blood. This injured the Stomach, reduced appetite, and made the condition increasingly severe. (Overexertion must damage the Yang of the Spleen and Stomach; greasy/cloying medicines must absolutely be avoided.)
Prescription: Charred Prepared Rehmannia Root (*Rehmanniae Radix Praeparata Carbonisata*), Lotus Seed Meat (*Nelumbinis Semen*), Chinese Yam (*Dioscoreae Rhizoma*), Poria with Hostwood (*Poria Cocos with Hostwood*), Euryale Seed (*Euryales Semen*), Honey-fried Licorice (*Glycyrrhizae Radix et Rhizoma Praeparata cum Melle*).
Zhang (Lishu, 47 years old): For two months, there has been fever during the day and coolness at night, with coughing, asthma, and urgency. This is Qi injury due to mid-life laboriousness. One must avoid alcohol, diaphoretic methods, and sweet-warm Qi-tonifying strategies.
Prescription: Ginseng (*Ginseng Radix*), Honey-fried Licorice (*Glycyrrhizae Radix et Rhizoma Praeparata cum Melle*), Coix Seed (*Coicis Semen*), Bletilla Tuber (*Bletillae Rhizoma*).
Stir-fried with honey water [Yellow/Golden herb implied or specific processing method].
Fang (36 years old): Pulse is fine, small, and weak at the cunchi position (root). Body moves restlessly with asthma and urgency. A young adult appears physically aged like an elder. This is lower-jiao depletion due to excessive sexual desire, with Essence and Blood internally exhausted, and Qi scattering without retention. ("Qi scattering without retention" is expressed with extreme precision.) Formative Essence and Blood cannot be regenerated by medicine alone. When Essence is severely depleted, the奇经 (Extraordinary Vessels) are already empty. Quack doctors use brute tonification; how can such brute tonification allow Essence to enter the Extraordinary Vessels?
Prescription: Ginseng (*Ginseng Radix*), Walnut Meat (*Juglandis Semen*), Poria (*Poria Cocos*), Psoralea Fruit (*Psoraleae Fructus*).
