(Qian Yi) Treats fire in the Small Intestine, presenting with red and painful dysuria (painful urination), flushed face, mania and restlessness, oral ulcers and tongue sores, teeth grinding, and thirst. (The Heart and Small Intestine are interior-exteriorly related; heat in the Heart leads to heat in the Small Intestine, hence red and painful dysuria. The Heart belongs to Ministerial Fire (Jun Huo) and is the master of fire in the five zang and six fu organs. Therefore, heat from all meridians responds to the Heart. Flushed face, restlessness, teeth grinding, and thirst are all manifestations of Heart Heat. The tongue is the sprout of the Heart; if Heart Fire flares upward and steams the mouth, it causes oral ulcers and sores. If light pressure on the skin below the hair and above the muscle reveals heat, which worsens at noon, this indicates heat in the blood vessels, belonging to Heart Heat. When Heart Fire is hyperactive and the Small Intestine is constrained and unable to flow freely, this formula is indicated. For treating conditions such as white turbidity and sand-like dysuria, combine with Xiaoyao San.)
Shengdi Huang (Rehmanniae Radix Preparata), Mutong (Akebiae Caulis), Gancao Shao (Glycyrrhizae Radix et Rhizoma tip), Danzhuye (Lophatheri Herba).
Shengdi Huang cools Heart Blood, and Danzhuye clears Heart Qi. (Danzhu ye grows on bamboo, thus it clears the Upper Jiao.) Mutong descends Heart Fire and guides it into the Small Intestine. (Ministerial Fire should be treated with Mutong, addressing the principle of treating dysuria caused by Ministerial Fire.)
Treats lung congestion and brain heat, dry nose, and constipation with difficult passage.
Tianmendong (Asparagi Radix, heart and peel removed), Sangbaipi (Mori Cortex), Shengma (Cimicifugae Rhizoma), Dahuang (Rhei Radix et Rhizoma), Zhike (Aurantii Fructus Immaturus, stir-fried with bran), Gancao (Glycyrrhizae Radix et Rhizoma), Jingjie (Schizonepetae Herba).
Note: This formula has fewer ingredients than Renshen Xiefei Tang, but the use of Shengma allows for both ascending and descending actions to disperse constrained heat in the Upper Jiao, which is worthy of adoption.
(Li Dongyuan) Treats accumulated heat in the Stomach, causing pain in the upper and lower teeth that radiates to the head, and a feeling of heat over the entire face. The teeth prefer cold and dislike heat; or there may be gum ulceration, bleeding from receding gums (Ya Xuan), or swelling and pain of the lips, mouth corners, cheeks, and parotid region. (The Foot-Yangming Stomach Meridian runs along the lateral side of the nose, enters the upper teeth, surrounds the mouth, follows the Jiache point, and ascends to the front of the ear; it governs the upper gums, prefers cold drinks, and dislikes heat. The Hand-Yangming Large Intestine Meridian ascends the neck, passes through the cheek, enters the lower teeth, surrounds the mouth, and governs the lower gums; it prefers hot drinks and dislikes cold. The collateral branch of the Foot-Yangming connects to the brain, hence brain pain. The Yangming meridian nourishes the face, hence facial heat. Excess heat in these two meridians leads to diseases of the lips, mouth, teeth, and cheeks, and facial swelling and pain. Teeth are the surplus of bone and belong to the Kidney. Bleeding from receding gums or between tooth gaps, also known as Chi Nu (epistaxis of teeth), is due to Kidney disease. If the bleeding is profuse and spurting uncontrollably, it is due to excess heat in the Yangming, because the Yangming meridian is abundant in qi and blood. The lip belongs to the Spleen-Stomach and Large Intestine meridians; dryness causes cracking, heat causes fissures, wind causes..., cold causes...; if swollen, rough, and cracked like a silkworm cocoon, it is named Jian Chun (cocoon lip). Lip swallowing refers to the root of the muscle. A flat and full philtrum indicates Chuan Fan (lip reversal); when Chuan Fan occurs, the flesh dies first.)
Shengdi Huang (Rehmanniae Radix Preparata), Mudan Pi (Moutan Cortex), Huanglian (Coptidis Rhizoma), Danggui (Angelicae Sinensis Radix), Shengma (Cimicifugae Rhizoma). One prescription adds Shigao (Gypsum Fibrosum).
Huanglian drains Heart Fire and also drains Spleen Fire. The Spleen is the child of the Heart and is interior-exteriorly related to the Stomach. Danggui harmonizes the blood; Shengdi Huang and Mudan Pi cool the blood to nourish Yin...
