Zhou Mengjue stated: When various types of Epilepsy (Xian Bing, 痫病) attack, the patient suddenly falls into convulsions and twitching, shouts or roars, and vomits saliva. Due to the similarity in their sounds, they are classified as Pig Epilepsy, Horse Epilepsy, Sheep Epilepsy, Ox Epilepsy, and Chicken Epilepsy. Tracing back to the source, sudden falling with loss of consciousness is caused by Phlegm misting the Heart Orifice (Tan Mi Xin Qiao, 痰迷心窍). Convulsions, enuresis, and twitching are caused by Wind entering the Liver Meridian (Feng Ru Gan Jing, 风入肝经). Although the names differ, they all pertain to the Heart and Liver meridians. The *Classic of Internal Medicine* states: If the pulse is slippery and large, it will heal itself after a long time; if the pulse is firm and small, it is incurable.
Some cases of epilepsy are acquired before birth. While in the mother's womb, if the mother suffers a sudden fright, Phlegm-Qi forces its way into the Heart and Liver, where it struggles with the original Qi and Blood to form a nest-like lesion (Ke Jiu, 窠臼). This type is incurable.
Some cases are acquired during childhood upbringing. Children have excess Heart and Liver Qi but insufficient Shen (Spirit). Upon occasional external stimulation, Fire erupts in the Heart, and Wind moves in the Liver. The Shen fails to reside in its abode, Phlegm-Saliva spreads and infiltrates, entering through gaps and taking dominance. This type is between curable and incurable.
Some cases occur in adults due to external contraction of Cold and Wind, and internal injury from diet. This causes rebellion of Zang-Fu Qi, blockage of the meridians, stagnation leading to Phlegm formation, which becomes sticky and fixed in the Heart and Liver. This type is entirely curable.
Just as unruly rebels require talent to breach gates and seize passes, and stubborn owls require courage to wield sharp weapons against them, dealing with entrenched enemies cannot be resolved merely by poetry, books, benevolence, righteousness, rites, or music. One must draw strong bows and shoot poisonous arrows to crush their momentum and destroy their nests. Similarly, Phlegm congealing in the Heart and Liver requires such an approach. Evil Qi that uses the Heart and Liver to sharpen its edge will become more vigorous if treated with warming methods; evil Qi that occupies the Heart and Liver to fortify its fortress will be hard to subdue if treated with harmonizing methods. Moreover, evil Qi that allies with the Heart and Liver to build factions will cause evil flames to rage like vultures if treated with tonification. To find a drug with a fierce nature that can reach the nest directly and perform killing actions, there is none other than Qingmengshi (Lapis Aurichalcum, 青礞石) and Shexiang (Musk, 麝香). These are holy medicines that can restore order, calm the Liver and descend Qi, and treat fright and promote Phlegm resolution.
Regarding this disease, I do not discuss congenital epilepsy, nor have I treated pediatric epilepsy. For adult patients, provided the pulse is floating and large, I generally use Mengshi Gun Tan Wan (Calamine Rolling Phlegm Pill) and Shexiang Wan (Musk Pill) for purgation. One dose of Liujunzi Tang (Six Gentlemen Decoction) may be taken as support. Countless patients have been cured. Some recovered after one month of medication, others after two months, stopping when the Phlegm was exhausted. The *Classic* says: "If there is a cause, there is no destruction," is this not true? The *Nan Jing* defines Dian (Epileptic Stupor) as stiff falling and staring straight ahead, which is no different from Xian. Recently reading the *Dian Kuang Pian* (Chapter on Epileptic Mania) in the *Nei Jing*, the content is largely similar. To claim that Xian is identical to Dian is incorrect; to claim that Xian is different from Dian is also incorrect. Since Dian and Xian are very similar, I leave this question open for further reference.
Wang Yinjun's Gun Tan Wan Formula: Qingmengshi (1 liang), Chenxiang (5 qian), Jiudahuang (8 liang), Jiuhuangqin (8 liang). First, crush the Qingmengshi, mix with 1 liang of Yanxiao (Saltpeter), place together in a clay pot, seal tightly with salt mud, dry, and fire until the stone turns golden. Grind into powder, mix with other drugs, and make water pills. Take 2.5 qian before bed, delivered with Ginger Decoction.
Zhou Mengjue's Shexiang Wan Formula: Fabanxia (1 qian), Dannanxing (7 fen), Chenpi (1 qian), Zhishi (1 qian), Shexiang (2 fen), Fuling (1 qian), Zhigancao (1 qian), Shengjiang (juice used to pill).
Wan Muzhai stated: Epilepsy involves sudden falling, rigid limbs, closed eyes or upward-turned eyeballs that cannot move, locked jaw, sometimes biting the tongue, flowing saliva or no saliva, complexion either green or white (this sentence requires careful attention), or making sounds of six domestic animals. The manifestations vary; it is a severe condition in children. After a brief period of fainting, they wake up as usual. Attacks may occur every ten days, monthly, or yearly. Ancients named Three Epilepsies and Five Epilepsies, with too many treatment methods and seemingly no fixed theory, leaving later students confused. Pediatric texts contain many formulas for treating epilepsy, but none are worthy of adoption. Only my family's secret new formula, named Duanxian Wan (Severing Epilepsy Pill), is truly miraculous.
Wan Muzhai's Duanxian Wan Formula: Chuanhuanglian (1 qian), Qingmengshi (1 qian), Shichangpu (1 qian), Chensha (1 qian), Bangzhenzhu (6 fen), Tiehuafen (1 qian), Dannanxing (1 qian), Baigansui (5 fen), Shangchenxiang (2 fen), Bai Fuling (1 qian). Additionally, decoct Renshen (1 qian) and Baizhu (3 qian) in water to make a paste for pilling. Deliver with Pig Heart Decoction.
Wan Muzhai's Tongxin Wan Formula: Chensha (1 qian), Mayaxiao (1 qian), Mingxionghuang (1 qian), Zhen Shexiang (2 fen), Baifuzi (5 fen), Chen Zhike (1 qian), Chuanxiong (1 qian), Fuling (1 qian), Renshen (1 qian), Chuanhuanglian (1 qian), Jinyinbo (1 qian). Honey pills, delivered with Maidong Decoction.
Tu Weisheng commented: Zhou Mengjue's phrases "Phlegm misting the Heart Orifice" and "Wind entering the Liver Meridian" directly elucidate the root cause of epilepsy. Especially the section describing how maternal fright causes Phlegm-Qi to invade the Heart and Liver, struggling with original Qi and Blood to form a nest, thoroughly explains the genesis of incurable epilepsy. Only a wise person with practical experience could achieve such insight. Although his formulas are potent, the disease is deeply rooted; without such strong agents, efficacy is unlikely. Since the theory matches the pathology, the formulas are applicable and safe.
Wan Muzhai's discussion on epilepsy describes the symptoms but fails to clarify the root cause. However, his formulas align secretly with Zhou Mengjue's, so I included them. Chen Feixia argued that children with green or white complexions should not undergo purgative treatment, and that Phlegm misting the Heart Orifice should not be treated with overly clearing methods, erroneously criticizing Wan Muzhai. He knew the constant but ignored the variable. Indeed, children with green/white complexions should not be blindly purged, but if the disease is due to Phlegm stagnation rather than Blood deficiency, then Phlegm is the root, and the color is the branch. Phlegm is a tangible substance; the Heart generates Blood. If the Heart is misted by Phlegm, Blood cannot circulate, and Qi stagnates. With stagnant Qi and Blood, nourishment to the face decreases, causing green/white discoloration. At this point, tonifying Blood to brighten the face and Qi to moisten the complexion would be akin to feeding thieves while trying to quell rebellion. Regarding hand and foot convulsions from Wind entering the Liver Meridian, I believe this results from Phlegm misting the Heart, causing the Shen to lose clarity and mobilize all strength to fight the Phlegm, transmitting the twitching state to the limbs. This is also because Heart Orifice blockage causes stagnation in other orifices, triggering Liver Qi and Blood. Regarding the various pills mentioned above, they should be used cautiously; less is better than more.
