Chen Zishan stated: The five tones correspond to the five Zang organs. The tone of Metal is clear and resonant, the tone of Earth is turbid and heavy, the tone of Wood is long-drawn, the tone of Water is clear, and the tone of Fire is dry and harsh. In Liver disease (Gan Bing), the voice is mournful; in Lung disease (Fei Bing), it is hurried; in Heart disease (Xin Bing), it is bold; in Spleen disease (Pi Bing), it is slow; in Kidney disease (Shen Bing), it is deep; in Large Intestine disease, it is long; in Small Intestine disease, it is short; in Stomach disease (Wei Bing), it is distant; in Gallbladder disease (Dan Bing), it is clear; in Bladder disease (Pang Guang Bing), it is faint. A clear voice indicates Qi deficiency (Qi Xu); a heavy and turbid voice indicates pain and Wind pathogen (Feng Xie); a high-pitched voice indicates excess Heat (Re) approaching mania; a choking voice indicates stagnant Qi (Qi Bu Shun). Panting indicates rapid breathing; an urgent voice indicates Convulsion (Jing Feng); a blocked voice indicates Phlegm (Tan); a trembling voice indicates Cold (Han); a turbid, deep, and quiet voice indicates Infantile Malnutrition (Gan Ji); sneezing indicates Wind-Cold injury (Shang Feng); yawning indicates mental fatigue; a low and dull voice indicates critical illness; if a newborn never cries loudly but makes only a squeaking sound, it indicates premature death. Having understood the changes in voice, one must further distinguish the complexion to ascertain the root cause of the disease.
Chen Zishan stated: The Heart governs the voice, which emanates from the Lungs. When Lung Qi fails, crying produces no sound; frequent crying often indicates Liver-Gallbladder Wind pathogen (Gan Dan Feng Xie); sluggish Qi with mental fatigue and severe convulsions; hoarseness indicates Evil Heat invading the Spleen; a clear voice indicates no invasion by Fire Toxin; a crow-like voice accompanied by twitching is a specific sign; a straight, direct voice without tears indicates pain; continuous crying with abundant tears indicates fright/convulsion; prolonged illness with a faint voice and irritability indicates a difficult-to-cure condition; a curled body with a hurried voice indicates contraction of Cold (Gan Han).
Tu Weisheng stated: This section differs slightly from the original text because the original contains many ambiguous and hard-to-understand parts, likely due to printing errors. Therefore, slight modifications were made to supplement Chen Zishan’s exposition.
Chen Zishan stated: Purple-red complexion indicates Cold Damage (Shang Han); green indicates Wind; white indicates Gan Ji; black indicates poisoning by malignant evil (Zhong E); yellow indicates Spleen stagnation.
Tu Weisheng stated: In the phrase "Green Wind," the character "Wind" was originally "Fright" (Jing). Because it differs from "Wind," it was changed to "Wind." However, carefully deducing Chen Zishan’s intent, his use of "Fright" essentially refers to "Wind." The same logic applies elsewhere.
Qian Yi stated: For infants under six months old who are ill, use the middle and ring fingers bent to press below the hairline above the forehead. If all three fingers feel hot, it indicates exposure to Wind pathogen, with nasal congestion and coarse breathing. If all three fingers feel cold, it indicates exposure to Wind-Cold, with cold internal organs leading to vomiting and diarrhea. If the index and middle fingers are hot, it indicates upper Heat and lower Cold. If the ring and middle fingers are hot, it indicates a pattern involving Wind. If the index finger is hot, it indicates fullness of Qi in the chest and diaphragm, and indigestion of milk and food.
Tu Weisheng stated: Qian Yi’s method places the index finger on top and the ring finger at the bottom. Carefully analyzing the statement "all three fingers hot, exposure to Wind pathogen," this should be a type of Wind-Heat syndrome. This is inferred from the subsequent statement "all three fingers cold, exposure to Wind-Cold." Similarly, "ring and middle fingers hot, pattern of Wind involvement" should indicate lower Heat and upper Cold, given the prior mention of "upper Heat and lower Cold." It is crucial to note that in summer, an infant's forehead and whole body are usually cooler, whereas in winter they are warmer. This is because air temperature varies with seasons, and human body temperature is perceived relatively through comparison with the environment. Although a patient's temperature can shift due to illness, failing to understand this environmental relativity can lead to misdiagnosis.
