The Kidney opens into the ears. This is indeed so. However, the Kidney meridian cannot ascend to the head. The Kidney interacts with the Heart, borrowing the pathway of the Heart's Fu organ (Small Intestine) to ascend to the acupoint in the ear called Tinggong (Auditory Palace). The meridian of the Small Intestine passes through it, serving as the residence of the deity responsible for hearing. Its shape resembles a pearl, with skin and membrane wrapping around the True Water (Jing). If the True Water breaks, deafness occurs immediately. Some become deaf due to being startled by loud noises; this is due to the rupture of the skin and membrane. Whether one becomes deaf or not depends on whether the Heart and Kidney interact. Ci Zhu Wan (Magnetite and Cinnabar Pill) should be used to harmonize Water and Fire. Those who experience tinnitus first and then deafness suffer from Kidney deficiency failing to open the storage, causing Yin Qi to obstruct the Yang orifices. Liu Wei Wan (Six-Ingredient Rehmannia Pill) should be used, removing Dan Pi (Moutan Cortex), heavily adding Ci Shi (Magnetitum), and further adding Wu Wei Zi (Schisandrae Fructus) and Gui Ban Jiao (Colla Testudinis et Carapacis) to form pills. This allows Yin Qi to flourish naturally in its home palace without touching the Yang orifices. If sudden deafness occurs due to cold pathogen invasion, it generally does not go beyond the Shaoyang channel. The Foot-Shaoyang Gallbladder meridian winds around the ear, and the Hand-Shaoyang Sanjiao meridian enters the ear. Pathogenic qi is congested and excess, obscuring the auditory organs. Xiao Yao San (Free and Easy Wanderer Powder) should be used, removing Bai Zhu (Atractylodis Rhizoma), and adding Huang Qin (Scutellariae Radix), Ban Xia (Pinelliae Rhizoma), Sheng Jiang (Zingiberis Rhizoma Recens), Yu Zhu (Polygonati Odorati Rhizoma), and Da Zao (Jujubae Fructus) as the main treatment. If Wind and Fire stir mutually, Fang Feng Tong Sheng San (Siler and Gardenia Powder to Unblock the Stagnation) should be used. If Liver Fire is blazing, Lu Hui Dang Gui Wan (Aloe and Angelica Pill) should be used. If the Chi pulse is weak, Gui Fu Di Huang Wan (Cinnamon and Aconite Rehmannia Pill) should be used. If the Chi pulse is rapid, Da Bu Yin Wan (Great Tonify Yin Pill) should be used. Both of these two pills can have Ci Shi (Magnetitum), Shi Pu (Acori Tatarinowii Rhizoma), and Rong Rong (Cynomorii Herba) added. Spiritual understanding lies within the practitioner; it cannot be transmitted merely by writing.
Floating and large indicates Wind. Surging and rapid indicates Fire. Surging, large, and excess indicates Wind-Fire. Rapid at the Chi position indicates Yin Fire. Slow and soggy indicates Kidney deficiency.
Hernia refers to swelling and pain in the lower abdomen and testicles. There are seven types. Han Shan (Cold Hernia): the scrotum is cold, hard like stone, the penis does not erect, or there is pain radiating to the testicles. Shui Shan (Water Hernia): the kidney sac swells and hurts, sweat appears intermittently from the yin area, or the swelling looks like crystal, or the scrotum itches and discharges yellow fluid. Jin Shan (Sinew Hernia): the penis swells, or ulcerates, or suppurates, or there is urgency with sinew contraction, or white discharge occurs. Xue Shan (Blood Hernia): shaped like a cucumber, or located between the ends of the transverse bones on both sides of the lower abdomen, commonly known as Bian Yong (Perineal Abscess). Qi Shan (Qi Hernia): extends upward to the kidney region and downward to the scrotum; it swells when crying or angry, and dissipates when calm. Hu Shan (Fox Hernia): when lying down, it enters the lower abdomen; when walking or standing, it exits the lower abdomen. Ti Shan (Scrotal Hernia): the scrotum hangs swollen, like a measuring cup or bucket, without itching or pain. However, one need not be rigidly bound by these definitions. The Classics state: "Diseases of the Ren Mai cause internal formation of seven hernias in men and leukorrhea and masses in women." It also states: "Liver disease of the Foot-Jueyin causes ti hernia in men and lower abdominal swelling in women." Generally, Ren Mai and Liver diseases are most common, followed by Small Intestine diseases, with occasional involvement of other meridians. The treatment method uniformly uses Er Chen Tang (Two-Cured Decoction), adding Ze Xie (Alismatis Rhizoma), Zhu Ling (Polyporus), Bai Zhu (Atractylodis Rhizoma), Gui Zhi (Cinnamomi Ramulus), Xiao Hui Xiang (Foeniculi Fructus), Mu Tong (Akebiae Caulis), and Jin Ling Zi (Meliae Fructus). When I first used this formula in my youth, I suspected it was merely accidental efficacy. Later, repeated trials confirmed its effectiveness, realizing its great merit in regulating Qi. Zhang Jingyue anticipated my thoughts. Jingyue said: "The disease name 'Hernia Qi' is not meaningless. Cold has cold qi, Heat has heat qi, Dampness has damp qi, Reversal has reversal qi, Sinking has sinking qi. In the Yang division, there is qi within qi; in the Yin division, there is qi within blood. When treating based on Cold, Heat, Deficiency, and Excess, Qi-regulating herbs must always be included. My formula transforms the Qi of the Bladder, thereby regulating all Qi. The principle is detailed in the chapter on Distension; it should be studied carefully. The root pulse should be firm and urgent; weak and urgent is unsuitable."
Besides the modified Er Chen Tang: ○ If Cold is severe, add Gan Jiang (Zingiberis Rhizoma) and Fu Zi (Aconiti Lateralis Radix Praeparata). ○ If Heat is extreme, add Huang Bo (Phellodendri Cortex) and Zhi Mu (Anemarrhenae Rhizoma). ○ If urine is like paste, add Shi Pu (Acori Tatarinowii Rhizoma) and Bi Xie (Dioscoreae Hypoglaucae Rhizoma). ○ If Qi rushes upward, remove Bai Zhu (Atractylodis Rhizoma), add Rou Gui (Cinnamomi Cortex), Wu Zhu Yu (Evodiae Fructus), and Dang Gui (Angelicae Sinensis Radix). ○ If the cyst is like crystal, add Yi Yi Ren (Coicis Semen) and Sang Bai Pi (Mori Cortex). ○ If pain is unbearable, fearing blood stasis turning to pus leading to ulceration, add Tao Ren (Persicae Semen), Hong Hua (Carthami Flos), and Ru Xiang (Olibanum). ○ If sinews contract, add Yi Yi Ren (Coicis Semen) 1 liang and Mu Gua (Chaenomelis Fructus) 2 qian. ○ If there is stubborn numbness without pain, add Chuan Xiong (Ligustici Chuanxiong Rhizoma) and Bing Lang (Arecae Semen). ○ If there is itching, add Ci Ji Li (Tribuli Fructus) 3 qian.
Qianjin Yi Xi Fang (Prescription from Qianjin Yi for Washing) treats men with scrotal swelling like a bucket and pain in the testicles.
Xiong Huang Mo (Realgar Powder) (1 liang)
Fan Shi (Alumen) (2 liang)
