Postpartum blood syncope is caused by failed blood flowing into the Liver meridian. The patient sees black flowers before their eyes, experiences rotational vertigo of the head and eyes, cannot rise or sit up, and may even become comatose and unconscious.
This is because rebellious Qi rushes upward to the heart and chest, leading to coma and loss of consciousness, with chaotic breath nearly ceasing.
However, there are three causes: fainting due to excessive physical exertion, fainting due to excessive postpartum bleeding, and fainting due to insufficient postpartum bleeding. Although the symptom is syncope in all cases, the treatment methods differ significantly, requiring detailed examination.
For fainting due to excessive bleeding, characterized only by drowsiness and irritability, one should use herbs that tonify blood and clear the Heart.
For fainting due to insufficient bleeding, caused by lochia failing to descend and rushing upward to the Heart, resulting in fullness and urgency below the heart, coma, locked jaw, and complete unconsciousness, one should use herbs that break stasis and promote blood circulation.
Ancient methods record: Immediately after delivery, preheat a scale weight, yellow stones from rivers, or hard charcoal until red-hot, place them in a vessel, and immediately pour vinegar over them near the bed. The resulting vinegar vapor can eliminate blood syncope. It is beneficial to use this method anytime within one month postpartum.
All syncope cases are due to deficiency-heat, with Qi and Blood rushing recklessly and abdominal emptiness. Before delivery, the first priority is to take sour vinegar, apply it to the mouth and nose, keep vinegar nearby for inhalation, and drink it slowly. This is the superior method.
If feeling faint, apply vinegar to the face; once awake, drink vinegar diluted slightly with water. Another method suggests diluting vinegar with water. Another method involves burning dry lacquer to produce thick smoke and fumigating the mother's face to restore consciousness. If dry lacquer is unavailable, burning old broken lacquerware also works well.
After menstruation, if confusion occurs, it may be due to excessive blood loss causing stagnation. Excessive blood loss leads to Blood deficiency, which causes Heart deficiency. Heart deficiency results in trembling limbs, chaotic spirit, restlessness, seeing black flowers, and severe confusion.
Insufficient blood loss leads to Blood confusion. Confused Blood flows upward with Qi, lodging in the Heart orifices, causing subcardiac fullness and urgency, with nowhere to settle. The spirit becomes intoxicated, and speech is impaired.
Postpartum confusion belongs to blood syncope. In blood syncope, due to deficiency, there is no proper residence for Qi and Blood, leading to reckless movement, obstructing the Heart and Lungs, causing visual disturbances, vertigo, coma, cyanotic lips, cold flesh, and cessation of breath. Even if these symptoms persist for days, upon feeling coma and suffocation, Ba Bao Wan (Eight Treasures Pill) should be taken.
Postpartum blood syncope occurs because varying amounts of postpartum bleeding disturb the Five Zang organs, leaving Qi and Blood unsettled. Failed blood surges, attacking the Liver and Heart. Excessive bleeding leads to extreme Blood and Qi deficiency. If bleeding is accompanied by rebellious Qi, Blood follows Qi upward to oppress the Heart. Both conditions cause syncope, irritability, and fullness. If syncope persists, death ensues.
If failed blood attacks the Liver, causing Liver Qi deficiency, symptoms include visual disturbances, irritability, and near-death states.
It is also stated that postpartum blood syncope with irritability and impending Qi cessation is due to excessive postpartum hemorrhage leading to extreme Blood and Qi deficiency, causing syncope.
Alternatively, insufficient bleeding with extreme Qi rebellion causes Blood to rush upward to the Heart, leading to irritability and fullness. One must examine the amount of postpartum bleeding to predict syncope.
Persistent syncope leads to death. During delivery, one should face the sitting/lying area and adhere to seasonal regulations. Violating taboos often causes syncope. Thus, regardless of bleeding volume, violations during childbirth often lead to disaster.
Blood syncope vertigo requires assessing deficiency and excess. Pre-existing fluid retention, if not resolved, often causes postpartum vertigo, along with Blood and Qi weakness leading to rebellious Blood attacking upward. Qing Hun San (Clear Soul Powder) is ineffective here.
Cui Shi stated: All syncope is due to deficient heat, reckless Qi and Blood, and abdominal emptiness. Before delivery, apply sour vinegar to the mouth and nose, keep vinegar fire nearby for inhalation, and drink it slowly. This is the superior method to prevent syncope.
There are three types of blood syncope syndromes; medication should be chosen based on careful differentiation.
For syncope due to low blood volume (lochia not descending, rushing to Heart, subcardiac fullness, coma, locked jaw), use blood-breaking and circulating herbs. Hei Shen San (Black Divine Powder) is highly effective.
For syncope due to high blood volume (only coma and irritability), use blood-tonifying and Heart-clearing herbs. Xiong Gui Tang (Ligusticum and Angelica Decoction) is highly effective.
For insufficient bleeding with extreme deficiency, Yang Rou Tang (Mutton Decoction) or Dang Gui Jian Zhong Tang (Angelica Decoction to Construct the Middle) may be used.
For those with chronic wind diseases, where production injury exacerbates stagnation, and Qi/Blood attack due to deficiency, Si Wu Tang (Four Substances Decoction) added with Fang Feng (Saposhnikovia Root) and Qiang Huo (Notopterygium Rhizome) is suitable.
For syncope with significant heat, Du Huo Chai Hu Tang (Angelicae Pubescentis and Bupleuri Decoction) treats wind-phlegm.
For excessive bleeding with extreme Qi deficiency, coma, weak respiration, and persistent syncope, death is inevitable.
