On the fifth day of cold damage (Shanghan), the Shaoyin meridian is affected. The Shaoyin meridian belongs to the Kidney. Its pulse penetrates the Kidney and connects with the Lung. Therefore, after five days of illness, symptoms such as dry mouth and dry tongue may appear; or if the mouth feels normal but there is aversion to cold, this indicates that the Shaoyin Kidney is affected. In Shaoyin disease, if dry mouth and dry tongue occur, urgent purgation should be applied, and Dachengqi Tang (Great Order the Qi Decoction) is suitable. If there is no thirst, no dry mouth or tongue, and the pulse is deep, urgent warming should be applied, and Sini Tang (Frigid Extremities Decoction) is suitable. Both Taiyin disease and Jueyin disease do not present with aversion to cold; only Shaoyin disease presents with aversion to cold. This must not be overlooked. In Shaoyin disease, occurring two or three days after onset, if the mouth feels peaceful but the back feels cold, moxibustion should be applied, and Sini Tang (Frigid Extremities Decoction) should be taken. Generally speaking, Yangming syndrome in cold damage is suitable for purgation, while Shaoyin syndrome is suitable for warming. Zhang Zhongjing, when discussing Shaoyin syndrome, stated that if dry mouth and dry throat occur, urgent purgation should be used. This is because Shaoyin governs the Kidney and connects to the root of the tongue. When heat from cold damage enters the zang organs and flows into the Shaoyin meridian, kidney fluids dry up, and the throat becomes scorched, leading to dry mouth, dry throat, and thirst. Urgent purgation is necessary, unlike Yangming syndrome where purgation can be delayed. Although so, there is one case in Yangming syndrome: fever with profuse sweating requires urgent purgation. Because Yangming belongs to the Stomach, excessive sweating depletes stomach fluids, requiring urgent purgation.

Treating cold damage on the fourth, fifth, or sixth day without sweat, with headache and high fever, unknown pain location in the body, restless sleep, irritability, surging and tight pulse, and difficult constipation. Baijie San (Hundred Release Powder) is suitable. For ordinary people suffering from cold damage, sudden febrile disease, several days of no appetite, and difficult constipation, doctors often attribute the lack of appetite to weak stomach qi and refuse to use purgatives, leading to fatal consequences. This is because heat-toxin qi steams and stifles the zang organs, damaging righteous qi, hence the inability to eat. For those suffering from cold damage, after four to five days without sweat, with headache, high fever, irritability, pain in all bones and joints, and difficult hot constipation, even if they cannot eat, purgation should still be used. If the physical form is not severely ill, qi will naturally become safe, and appetite will increase. When using purgatives, first examine the pulse qi of the five zang organs to see which organ is diseased, then apply the medicine accordingly to ensure safety. If among the three positions (cun, guan, chi), one position is particularly large and tight, that organ is diseased. If pulse diagnosis is not precise, simply taking Baijie San will never cause error, as this formula treats diseases of the five zang organs. If there is delirious speech, urgently take the following medicine; purgation will bring relief. Effective. (from *Hu Ming Fang*)
