Taiyang disease (diseases caused by pathogenic factors invading the Taiyang meridian) that has passed through its transmission cycle but remains unhealed, with a pulse that is instead deep and slow, and cold extremities, indicates that the pulses at the lower part are absent or imperceptible. This signifies an interruption of communication between Yin and Yang, where the pathogenic factor invades the Foot Shaoyin (Kidney) meridian. Toxic heat steams upward, causing sore throat or even ulceration.
Sore throat due to Taiyang disease occurs when the Shaoyin meridian is invaded by pathogens, and toxic heat obstructs the Upper Jiao (Upper Burner). Mild cases present only as sore throat; severe cases involve pain accompanied by ulceration. Furthermore, if internal pathogenic heat becomes excessive after the transmission cycle, steaming the Upper Jiao, it causes redness, swelling, and pain in the throat. Treatment should be based on the specific syndrome pattern.
The verse states: Eruption and spitting blood indicate Yang Du (Toxic Heat); Yin deficiency inevitably leads to ulcers in the throat. If sweating occurs with both Yin and Yang pulses being tight and choppy, the rule dictates this is a sore throat known as Wang Yang (Loss of Yang).
Another discussion states: Yin syndrome refers to Shaoyin disease. If both Yin and Yang pulses are tight, it primarily indicates absence of sweating; if sweating occurs, it is called Wang Yang (Loss of Yang) and belongs to Shaoyin. The rule dictates sore throat. Treatment for such cases can refer to the "Spontaneous Sweating and Loss of Yang" section. Generally, for sore throat, Ganjie Tang (Licorice and Platycodon Decoction) and Zhufu Tang (Pig Skin Decoction) can be used for both Yin and Yang syndromes.
Regarding syndrome treatment: For mouth sores with redness and erosion, soak Huangbai (Phellodendron Bark) in honey and swallow the juice. Shengma Liuwu Tang (Ephedra and Six-Ingredient Decoction) may also be used. Additionally, if sudden cold attacks outside the proper season, hiding in the Shaoyin meridian, resulting in a weak pulse and sore throat followed by diarrhea, ancient formulas call this "Kidney Cold Damage." Treatment should first use Banxia Guigan Tang (Pinellia, Cinnamon Twig, and Licorice Decoction), then Sini Tang (Frigid Extremities Decoction). It should heal within one to two days. All Shaoyin meridian cold damage is referred to as Kidney Cold Damage. These conditions usually lack exterior-interior heat, but patients feel vexed, silent, extremely averse to light, sometimes have abdominal pain, or a deep-thin pulse. Old formulas use Sishun Tang (Four-Smooth Decoction). Ancients used it knowing its principle. If Kidney disease presents with residual body heat, Huanglian Longgu Tang (Coptis and Dragon Bone Decoction) can be taken. If after ten-plus days, diarrhea persists and extremities are thoroughly cold, for heat syndromes, use Sishun Tang. Details on this formula are scarce in other texts. Huanglian Longgu Tang is derived from Huanglian Jizi Tang (Coptis and Egg Yolk Decoction) by removing the egg yolk and adding half tael of Longgu (Dragon Bone). Sishun Tang Wan is derived from Lizhong Tang Wan (Regulate the Middle Decoction Pill) by doubling the amount of Gancao (Licorice Root).
For sore throat, if caused by Yin Du (Yin Toxin) leading to eruption, cold limbs, throat ulcers, Renzhuo Syndrome (Kidney Dampness) with tight pulse and absence of Yang, sore throat, weak pulse, and diarrhea:
Another discussion states: Throat discomfort, pain, or ulcers, inability to eat, and vomiting are caused by toxic heat rushing upward. Yang Du inevitably causes eruption; Shaoyin disease causes cold limbs. For throat discomfort, provide Ganjie Tang or Banxia San (Pinellia Powder) with Gancao and Shengjiang (Fresh Ginger). If there is no diarrhea, or if urination is red and scanty causing a desire to remove clothes, provide Jiegeng Tang (Platycodon Decoction) with Xuanshen (Scrophularia Root) and Zhike (Aurantium Fruit) to promote bowel movement, thereby releasing deep-seated toxicity. Follow up with a decoction of Gancao and Shengjiang to soothe, as they resolve toxicity.
For Kidney disease with sore throat and discomfort, if the body still has heat, do not use Sishun Tang yet; instead, use Huanglian Longgu Tang (Huanglian Jizi Tang minus egg yolk plus Dragon Bone). For Yang Du sore throat with eruption and purulent bloody sputum, with a surging-rapid pulse, refer to the specific section. For various Shaoyin sore throats with ulcers, deep-slow pulse, and cold limbs, emetic or purgative methods may be considered. If Yin-Yang pulses are tight but with spontaneous sweating, indicating absence of Yang, expect sore throat with vomiting and diarrhea; this also belongs to Shaoyin. In these cases, sweating, purging, and heavy moxibustion/ironing are contraindicated. If sweating occurs, apply rice powder to the area. For sore throat, use Ganjie Tang or Zhufu Tang. For severe deep-cold, use Banxia San or Tongmai Sini Tang (Revive the Pulse Sini Decoction, removing Shaoyao and adding Jiegeng). Ganjie Tang is general for sore throat. If Upper Jiao heat obstruction exists, add Zhike and Qianhu (Peucedanum Root).
Taiyang disease is caused by non-seasonal violent cold attacking the person, hiding in the Shaoyin meridian, causing headache, lower back pain, and weak pulse. Initial sore throat resembles Taiyang disease but is followed by diarrhea; it differs from Hou Bi (Throat Obstruction). Use Banxia Guigan Tang (Banxia San) for sore throat, and Sini Tang or Sishun Tang for diarrhea.
Regarding syndrome treatment: For mouth sores with redness and erosion, soak Huangbai in honey overnight and gargle. Use Shengma Liuwu Tang for heat. For throat closure, use Wushan (Iris Tectorum).
If Taiyang disease lasts six to seven days and is mistakenly treated with harsh purgatives, resulting in a deep-slow cun pulse and absent chi pulse, throat discomfort, purulent bloody sputum, cold limbs, and persistent diarrhea, it is difficult to treat. Use Mahuang Shengma Tang (Ephedra and Ephedra Decoction).
Sore throat is divided into Yin and Yang syndromes. Floating-rapid pulse, red face like brocade patterns, sore throat, and purulent bloody sputum indicate Yang syndrome. Deep-slow pulse, cold limbs, or vomiting/diarrhea with sore throat indicate Shaoyin syndrome. *Bing Yuan* states: Absent lower pulses signify interrupted Yin-Yang communication; pathogenic factors invade the Foot Shaoyin collaterals, toxic heat rushes upward, causing throat discomfort, pain, or ulcers.
If Taiyang disease pulses are tight-yin-yang but with sweating, it indicates Wang Yang (Loss of Yang), belonging to Shaoyin. Expect sore throat with vomiting and diarrhea. For this condition, sweating, purging, and heavy ironing are all contraindicated. If sweating occurs, apply rice powder.
For sore throat, select from: Gancao Tang (Licorice Decoction), Jiegeng Tang, Zhufu Tang, Banxia San, Tongmai Sini Tang (minus Shaoyao plus Jiegeng), or Mahuang Shengma Tang.
There is also Fu Qi Bing (Latent Qi Disease), where non-seasonal violent cold hides in the Shaoyin meridian. Initially asymptomatic, it manifests after a month with a weak pulse. Rule: First sore throat resembling Taiyang disease but not Hou Bi, followed by diarrhea. Start with Banxia Guigan Tang, then Sini San (Frigid Extremities Powder). It heals in one to two days, called Kidney Cold Damage in ancient texts.
