In cases of Taiyang disease (Taiyang Disease) where urination is obstructed, it may be due to excessive sweating, leading to a deficiency and scarcity of body fluids (Jinye), causing dryness in the Stomach; or it may be due to latent heat in the Small Intestine, resulting in the obstruction of Qi mechanisms. All these conditions lead to urinary obstruction. The commentary on formulas states: When the Stomach is dry, there is no urine production, and one must absolutely not use diuretics. This refers to the loss of body fluids after sweating. If there is heat in the Lower Jiao (Lower Burner) and urination is difficult, one should treat according to the syndrome by promoting urination.
The *Huoren Ge Kuo* says: After Taiyang disease undergoes sweating or purging, body fluids are lost, causing extreme dryness in the Stomach. According to principles, this often results in urinary obstruction; if physicians mistakenly use diuretics, it is contrary to treatment. If heat is in the Lower Jiao and the patient desires water but urination remains obstructed, a floating pulse indicates Wu Ling San (Five-Ingredient Powder with Poria), while a deep pulse suggests Zhu Ling San (Poria Powder) is also suitable. Difficulty in urination due to dryness and exhaustion of the Small Intestine, or dryness of the Stomach with excessive sweating, both belong to Yangming disease and require caution against promoting urination or guiding fluid intake. Yellowing caused by dampness must be distinguished from difficulty in urination due to stagnation. Deficient heat lodging in the Bladder, often occurring after sweating or purging that depletes fluids and causes Stomach dryness, leads to urinary difficulty. Physicians seeing urinary difficulty often erroneously use diuretics. If urination becomes free, the illness will not resolve spontaneously. It is merely heat in the Lower Jiao causing urinary difficulty; a floating pulse warrants Wu Ling San, and a deep pulse warrants Zhu Ling San. If the exterior syndrome is unresolved, with fluid retention in the Heart region, fever, cough, abdominal fullness, and urinary difficulty, Xiao Qing Long Tang (Minor Green Dragon Decoction) with Ephedra removed and Poria added is used. For symptoms like mild sweating, headache at the vertex, and urinary difficulty, Gui Zhi Qu Gui Jia Fu Ling Bai Zhu Tang (Cinnamon Twig Decoction with Cinnamon Removed plus Poria and Atractylodes Macrocephala Decoction) is indicated. For vomiting with fever, chest and hypochondriac fullness, palpitations, and urinary difficulty, Xiao Chai Hu Tang (Minor Bupleurum Decoction) with Scutellaria removed and Poria added is used. For Shaoyin disease with urinary difficulty, Si Ni San (Frigid Extremities Powder) with Poria added is used. Sweating externally depletes internal fluids, causing urinary obstruction. Alternatively, Yin deficiency allows Yang to congregate, creating latent heat in the Small Intestine, leading to frequent, hot, red, and yellow urine that is difficult to stop. The *Su Wen* states: "When Yang enters the Yin division, the Bladder becomes hot and urination becomes difficult," which describes this situation. Those who sweat excessively should avoid diuretics. Those with latent heat should use cooling agents to clear it. In cases of dampness-induced jaundice, failing to use diuretics is not the correct treatment. For Taiyang disease with urinary difficulty, using diuretics aims at leaching out dampness and draining heat. After a major illness resolves, if there is water qi below the waist, Mu Li Ze Xie San (Oyster Shell and Alisma Powder) is used, following the principle of promoting water passages and leaching dampness. Difficulty in urination with yellow color indicates Yin deficiency attacked by Yang, with heat inside; thus, difficulty and yellow urine occur, treated with Wan Quan Mu Tong San. Difficulty in urination can result from Taiyang disease where sweating leads to continuous leakage, aversion to wind, stiffness and cramping of the four limbs making flexion and extension difficult, causing urinary difficulty; Gui Zhi Jia Fu Zi Tang (Cinnamon Twig Decoction plus Aconite Root) is used. In Yangming Wind Strike, with a stringy, floating, and large pulse, shortness of breath, abdominal fullness, pain in the hypochondrium and heart region, dry nose, inability to sweat, drowsiness, yellow skin, urinary difficulty, tidal fever, and hiccups, Xiao Chai Hu Jia Fu Ling Tang (Minor Bupleurum Decoction plus Poria) is used. It is also said that Kidney and Bladder deficiency allows pathogenic heat to enter through the deficiency. Deficiency causes frequent urination; heat in urine causes涩 (astringency/difficulty) in the urinary pathway. A choppy Diagonal pulse combined with a floating pulse indicates that choppy signifies Kidney deficiency leading to frequent urination, while floating signifies Stomach excess leading to hard stool. In Taiyang disease with spontaneous sweating, stiff limbs, difficulty moving, irritability, slight aversion to cold, and leg cramps, if urination is frequent, Gui Zhi Tang must not be used. Instead, Gan Cao Gan Jiang Tang (Licorice and Ginger Decoction) and Shao Yao Gan Cao Tang (Peony and Licorice Decoction) are appropriate. After vomiting and purging in Taiyang disease, if urination is frequent and stool is hard and dry, Xiao Cheng Qi Tang (Minor Order the Qi Decoction) or similar purgatives are used. In Taiyang disease with spontaneous sweating, floating pulse, frequent urination, Stomach disharmony, and delirium, a small amount of Tiao Wei Cheng Qi Tang (Regulate the Stomach and Order the Qi Decoction) is given. For hematuria, Yan Hu Suo San (Corydalis Powder) is discussed in treatment theories.
