Yin and Yang qi fail to connect smoothly, the clear and turbid qi interfere with each other, the zang-fu organs lose harmony, and healthy qi (Zheng Qi) and pathogenic factors (Xie Qi) intermingle chaotically, causing dysfunction of the intestines and stomach, thereby leading to vomiting and diarrhea. If Huoluan is caused by Shanghan (Cold Damage), the Spleen qi is already deficient, there is vexation and heat in the heart and diaphragm, abdominal distension, headache, aversion to cold in the body, indigestion of retained food, stagnation in the Middle Jiao (Middle Burner), rebellion of Ying (Nutritive) and Wei (Defensive) qi, mutual attack of cold and heat pathogens, cold limbs, occasional vomiting or diarrhea. Hence it is called Huoluan. The name "Huoluan" refers to the condition where one vomits above and has diarrhea below, occurring suddenly and causing chaos. If the pathogen is in the Upper Jiao (Upper Burner), there is only vomiting without diarrhea; (if in the Lower Jiao, the reverse applies). Vomiting and diarrhea caused by Shanghan are due to injury by pathogenic factors; vomiting and diarrhea caused by Huoluan are due to injury by diet. If there is a concurrent invasion of Shanghan pathogens leading to disharmony between interior and exterior, presenting with headache, fever, and accompanied by vomiting and diarrhea: The Classics state, "Fever, headache, body ache, aversion to cold, vomiting and diarrhea—what disease is this?" The answer is: This is named Huoluan. In Huoluan with headache, fever, and body pain, if heat signs predominate and there is thirst for water, Wuling San (Five-Ingredient Powder with Poria) is the primary formula; if cold signs predominate and there is no desire for water, Lizhong Wan (Regulate the Middle Pill) is the primary formula. Because of mismanagement of the Middle Jiao, Yin and Yang qi become separated and blocked, inevitably leading to bias. Bias toward Yang results in excess heat; bias toward Yin results in excess cold. Xu Renze stated: There are two types of Huoluan: Gan Huoluan (Dry Huoluan) and Shi Huoluan (Wet Huoluan). Gan Huoluan has a high mortality rate, while Shi Huoluan has a low mortality rate. This is because in Shi Huoluan, the ingested harmful substances are expelled through vomiting and diarrhea; even if Huoluan is severe, once the water and grain in the stomach are exhausted, it stops, so few die. As for Gan Huoluan having a high mortality rate, it is because one cannot vomit from above nor defecate from below, so the ingested harmful substances cannot be expelled, blocking healthy qi and isolating Yin and Yang, causing vexation, agitation, and stifling discomfort with no relief, ultimately leading to death from wheezing, distension, and Huoluan. Alas! Those who eat and drink in moderation and maintain regular daily routines, how could they contract Huoluan? When food intake doubles, the intestines and stomach are injured; the cause of death is truly self-inflicted.
Vomiting accompanied by diarrhea is named Huoluan. It is caused by pathogenic factors in the Middle Jiao, preventing the normal ascending and descending of Yin and Yang qi, resulting in drum-like pain in the heart and abdomen, and inducing vomiting and diarrhea. The symptoms manifest such that if heart pain occurs first, vomiting precedes; if abdominal pain occurs first, diarrhea precedes; if both heart and abdominal pain occur, vomiting and diarrhea happen simultaneously. The ancients named it Huoluan because the disease strikes swiftly and violently, causing chaos within the time it takes to wave a hand. Shanghan Huoluan is also caused by disharmony of Yin and Yang in the Middle Jiao. Thus, some present with high fever and thirst, some with chills and thirst, some with fever, aversion to cold, sweating, and cold limbs, and some with lingering body pain after the acute phase has passed. Treatment cannot be uniform. A verse states: "If limbs are cramped and rigid with cold limbs, and fever with aversion to cold, use Sini Tang (Four-Reversal Decoction). If vomiting and diarrhea stop but body pain persists, Guizhi Tang (Cinnamon Twig Decoction) is best for harmonizing."
If vomiting and diarrhea cease, followed by sweating, cold limbs, cramping rigidity of limbs, and a faint pulse nearly imperceptible, Tongmai Sini Tang (Pulse-Reviving Four-Reversal Decoction) with Pig Bile is used. For Huoluan after sweating, with body pain, Guizhi Tang is used. For excessive heat with thirst, Wuling San is used; for excessive cold without thirst, Lizhong Tang is used, optionally adding Shigao (Gypsum Fibrosum).
Another verse states: "For Huoluan with excessive heat, use Wuling San; for excessive cold without water craving, seek Lizhong. For aversion to cold, cold limbs, and alternating heat, use Ginger, Licorice, and Aconite. For summerheat stroke with chest irritability and abdominal pain, use another prescription." Zhang Shi claimed that mere vomiting and diarrhea do not constitute Huoluan. Huoluan involves pathogens in the Middle Jiao, caused by irregular diet and lifestyle. Combined with food retention and Shanghan, Yin and Yang are disrupted, leading to upper vomiting, lower diarrhea, irritability, abdominal pain, bias toward Yang causing heat, bias toward Yin causing cold. It strikes suddenly, more dangerous than a candle in the wind. Xu Renze noted that Shi Huoluan has fewer deaths, while Gan Huoluan has more. This is because in Shi Huoluan, harmful substances are expelled via vomiting/diarrhea and stop when exhausted, hence fewer deaths. In Gan Huoluan, inability to vomit or defecate blocks healthy qi and isolates Yin/Yang, causing irritability and flank distension, hence more deaths. Diet and lifestyle should heed this warning. For Huoluan with vomiting/diarrhea, excessive heat with thirst: Wuling San. Excessive cold without water craving: Lizhong Tang/Pill. For cold syndrome with abdominal fullness/pain, cramping rigidity, convulsive diarrhea: Cuo Lizhong Tang with Sheng Fuzi (Prepared Aconiti Lateralis Radix) and Guixin (Cinnamomi Cortex). For vomiting/diarrhea with sweating, fever, aversion to cold, cold hands/feet, cramping, deep/thin pulse: Sini Tang. For summerheat Huoluan with irritability, gripping pain in large intestine/heart/abdomen, cold limbs, cold sweat, foot convulsions: Xiangling San. Qianjin Fang states: For convulsions, use Lizhong Tang with Shigao. For Huoluan after sweating, though vomiting/diarrhea stopped, body pain persists: give Guizhi Tang. For stopped vomiting/diarrhea, sweating, cold limbs, cramping, faint pulse: Tongyong Sini Tang with Pig Bile. Huoluan generally uses Huoxiang Zhengqi San (Agastache Powder to Rectify the Qi); add Guizhi for abdominal pain, add Zhuyu (Evodiae Fructus) and Huoxiang for severe pain, add Fuling for difficult urination, use Chushi Tang (Dampness-Removing Decoction) removing Cangzhu and adding Guizhi/Dingxiang for persistent diarrhea. For Gan Huoluan with heart/abdominal pain, desire to vomit but unable, desire to defecate but unable: first drink one bowl of salt water at once, wait for thorough vomiting, then administer Cuo Lizhong Tang with doubled Juhong (Tangerine Peel). For Qi stagnation in the middle, neither vomiting nor diarrhea, which is urgently harmful: use Suhexiang Wan (Styrax Pill) administered with Zhike San (Bitter Orange Powder). Or use Huoxiang Zhengqi San with Guixin, Fuling, and doubled Zhike. Generally, wind/rain/cold/dampness enter from outside; Huoxiang Zhengqi San and Chushi Tang are key. Dietary/lifestyle evils arise from within; Zhizhong Tang treats them. For unceasing vomiting/diarrhea with cyanotic face and cold limbs: Zhenling Dan (Shaking Spirit Elixir) ground and taken with Muxiang/Ganjiang decoction. Regarding Da Chaihu Tang syndrome: "Fever, sweating, unresolved illness, vomiting, diarrhea, epigastric stuffiness: Da Chaihu Tang treats this." This is distinct from Huoluan.
