Biao Ju (Biao Abscess), as a disease, has toxic pathogens that erupt rapidly, harming people within the blink of an eye. Therefore, its character is pronounced from "Piao" (fleeing), similar to the drifting of a gusty wind. Treatment must not be delayed in the slightest. The ancients stated that humans are afflicted by evil winds entering the muscles and vessels, transforming into this disease. It is likely that severe wind pathogens lie dormant and hidden, initially imperceptible; once the toxic qi stagnates and erupts, it becomes fierce like raging fire, unbearable to approach. Thus, its concealment is deep, appearing solid, dark, and obscure. The pain corresponds to the Heart, as the toxic qi has penetrated deeply, hence such symptoms.
Biao Ju manifests as sudden nodules in the flesh, sized like bean grains, small ones like millet seeds, severe cases like plums or peaches. Colors may be red, black, green, or white, with indeterminate shapes. They have roots but do not swell edematically; touching them causes pain that resonates with the Heart. The root extends deep into the muscle; after a long time, swelling spreads in all directions, turning dark purple-black, capable of destroying bones and tendons. If the toxin scatters along the meridians and enters the Zang organs, it kills. People in the South call it "Tuo Zhuo." Where the toxin concentrates in the flesh, it is surgically excised, or cauterized with a hot iron until charred like charcoal. Alternatively, moxibustion of one hundred burns is applied, or decoctions of Abelmoschus esculentus root juice, Indigofera tinctoria juice, Rhinoceros Horn juice, Cimicifuga foetida juice, Phyllostachys heterocycle bamboo sap, or Huanglong Tang are drunk. These single-herb treatments focus on clearing heat to achieve recovery. This disease favors the ten fingers, resembling Dai Zhi (Paronychia); because people do not recognize it, they call it Dai Zhi. If not treated urgently, it travels up the meridians into the Zang organs, killing. Southerners who contract this often amputate the finger. Initially, the fingertip turns dark, then swells red, black, and dark; pain piercing the Heart indicates this condition.
There is also a type called "E Rou" (Malignant Flesh), where flesh suddenly grows on the body like black bean grains, protruding and elongating, resembling cow or horse teats, with a top like a rooster's comb. Untreated, it grows continuously without pain or itching. This results from evil winds invading the muscles and vessels during spring and winter. Treatment should involve taking Loulu Tang externally cauterized with a hot iron daily until charred, then applying Shengma Ointment. Recovery takes accumulated days.
The eruption colors—red, black, green, white—change unpredictably. It may appear on arms, buttocks, mouth/teeth, or umbilicus, with no fixed location. Generally, it is more common between fingers. Roots penetrate deep into muscles, swelling inversely towards the arm; toxic blood flows and injects, penetrating sinews and vessels, rotting flesh to expose bone and bleeding. It often causes radiating pain, delirious speech, and death if pain pierces the Heart. Protrusions on the external genitalia also result in death. This sore most fears attracting Wind pathogens. Treatment methods should promote detoxification and blood circulation, paying special attention to the Heart and Kidney. The Heart is the gateway for all diseases; stagnant敗 blood, irritability, and chest oppression are due to toxic qi attacking the Heart. When the Heart is attacked by toxins, promoting urination might weaken the external genitalia and make one susceptible to wind; thus, it is necessary to guide toxins out through the stool, such as using Gualou Wine with Myrrha, Trogopterus Dung, and Gleditsia Sinensis thorn (with ash). Although this guides toxins via the stool, formulas that clear the Heart, circulate blood, and dispel wind cannot be discarded. With a clear Heart and circulating blood, wind pathogens disperse, leading to potential recovery. Classics state: if on the finger, amputate; if in the flesh, excise. This is to fear toxins traveling with wind through muscles and entering the abdomen via meridians, harming people suddenly. This disease initially arises from contracting evil winds entering the vessel network, or following Typhoid fever/Miasmic Malaria, generated by toxic transformation. For every inch an abscess opens, wind is drawn in; it is not necessarily that wind enters, but deficient areas naturally attract wind. Only when wind pathogens attack deficiency does ulceration penetrate bone and malignant blood flow freely (Arisaema cum Bile, Pinellia Ternata, Angelica Dahurica tip powder can dispel wind and be frequently applied; the therapeutic principle expands, applicable to carbuncles and sores).
Biao Ju is also known as She Zhang (Snake Miasma). In southern regions, snake venom is prevalent, excreted in misty, miasmic lands. Humans exposed to water, soil, and wind patterns may contract it; months later, it invariably develops into Snake Miasma. Red Centipede is the best herb for subduing snakes; Realgar and Angelica Dahurica are secondary. Various snake antidotes can also control its toxicity (Realgar is toxic and damages flesh, but should be used with snake herbs).
Chest pain with shortness of breath, rushing into darkness, covering the left eye with hand; if light is seen with the right eye, it is a chest carbuncle. If no light is seen, Biao Ju erupts internally. If pus and blood are vomited, it is incurable; ashes should cover the pus/blood, otherwise, it infects others.
