1. Transverse presentation refers to the situation where the fetus is turning, but the mother exerts force too early, causing the fetal arms to protrude before the body aligns properly. The mother should lie supine comfortably. The midwife should gently push the fetal arm and body back into alignment using her hand. Then, use the middle finger to palpate the shoulder to ensure the umbilical cord is not entangled or tethered, allowing for delivery.
2. Breech presentation occurs when the fetus has not yet turned, and the mother's exertion causes the feet to present first. The mother should lie in a correct position. Gently push the feet back inside with the hand. After some time, push the fetal body again, waiting slowly until it turns upright and approaches the birth canal for delivery.
3. Oblique presentation happens because the fetus does not follow the correct birth path, and the mother's exertion forces the fetal head to one side. Although it may appear that the vertex is presenting, it is actually the frontal angle. Follow the previous method to push it back into alignment for delivery. If the occipital bone of the fetal head presses against the side of the anus, gently lift and correct it from the outside rear with the hand for delivery.
4. Impeded labor occurs when the fetal body is aligned and the birth canal is straight, yet descent cannot occur. This is due to the umbilical cord becoming entangled around the shoulder during fetal rotation. Have the mother lie supine. Gently push the fetus upward with the hand, then press on both shoulders with the middle finger to straighten and untangle the umbilical cord, allowing for delivery.
5. Seated labor results from maternal fatigue near the time of delivery, causing the mother to sit for a long time on a chair or cushion, obstructing the birth path. Tie a towel securely at a high place. Instruct the mother to hold onto it, gently flexing and extending her legs to open the birth path, facilitating normal delivery.
6. Enteroptosis during labor (Pan Chang Chan) refers to the prolapse of the intestines before the fetus is delivered, which fails to retract after the fetus is born. Ancient methods suggest using half a cup of vinegar and half a cup of water, silently blowing air towards the mother's face and back to induce retraction via the acetic acid vapor. Another method involves grinding forty-nine seeds of Ricinus communis (Castor Bean), applying the paste to the vertex of the mother's head, and washing it off immediately once the intestines have retracted. Folk practices involve startling the mother by blowing water on her face and back to retract the intestines; however, this is discouraged as shock may scatter Qi and cause other ailments.
Another prescription: For transverse or difficult labor, have the mother lie supine. Prick the soles of the fetal hands and feet three to five times with a small needle, rub with salt, causing the limbs to retract. The fetus will turn and deliver.
Another prescription: For enteroptosis during labor, powderize Banxia (Pinelliae Rhizoma), take a small amount, insufflate it into the nose, causing the intestines to ascend naturally.
Another prescription: Use a large paper wick soaked in Sesame Oil (Sesami Oleum). Ignite it and blow it out, then smoke the fumes into the nostrils of the parturient woman, causing the intestines to rise.
Another prescription: If the intestines are prolapsed, place them in a clean lacquerware vessel. Immerse them in a strong decoction of Huangqi (Astragali Radix); the intestines will ascend.
