Women's milk is transformed from the Qi and Blood of the Chong Mai (Penetrating Vessel) and Ren Mai (Conception Vessel); therefore, when it flows downward, it becomes menstruation, and when it flows upward, it becomes milk. If there is a delay or scarcity of milk postpartum, it is due to insufficiency of Qi and Blood; if there is no milk at all, this is undoubtedly due to weakness of the Chong and Ren vessels. Treatment should tonify the source of transformation while simultaneously promoting unblocking and benefiting, making Zhu Ti Tang (Pig Trotter Decoction) appropriate. If the breasts are about to produce milk but flow is not yet smooth, Yong Quan San (Surging Spring Powder) is appropriate.
The failure of milk secretion in postpartum women has two primary causes: First, due to insufficiency of Qi and Blood, causing lack of milk; Zhu Ti Tang (Pig Trotter Decoction) is appropriate here, which represents the method of tonifying deficiency. Second, due to excessive phlegm-Qi stagnation in obese women, causing obstruction of the milk channels and lack of milk; formulas such as Lou Lu Tang (Euphorbia Fissurata Decoction) are appropriate here, which represents the method of moving stagnation.
