Lymphadenitis (Luoli) is a disease belonging to the Sanjiao and Liver-Gallbladder meridians, caused by wind-heat pathogenic factors leading to blood dryness, or by deficiency of essence and blood in the Liver and Kidney meridians with internal stirring of deficient fire. It may also result from anger and sorrowful overthinking, causing qi rebellion in the Liver and Gallbladder meridians. These two meridians typically possess abundant qi but scarce blood; thus, anger injures the Liver, causing Wood-fire to flare up and blood to dry out; Kidney yin deficiency fails to nourish Liver Wood, also resulting in blood dryness.
Blood dryness leads to pathology of the sinews, as the Liver governs the sinews; therefore, lymph nodes cluster together like beads on a string. This condition mostly occurs around the ears, extending to the cheeks and mandible, downward to the supraclavicular fossa (Quepen), and the sides of the chest and axilla, also known as "Madao" (Horse Knife). Initially, it resembles a bean grain, gradually growing to the size of a plum or walnut kernel, appearing as one nodule or three to five nodules. Palpation reveals mobility and slight pain, with minimal heat sensation; over time, it worsens daily. Symptoms may include stiffness and pain in the neck, mild afternoon fever, night-time dry mouth, reduced appetite, limb fatigue, hard nodules that do not ulcerate, or ulcers that fail to heal. All these are due to insufficiency of qi and blood, often transforming into consumptive tuberculosis (Laozai). *Waitai Miyao* states: Deficient heat in the Liver and Kidneys generates lymphadenitis. *Bingji* states: Lymphadenitis is not caused by rich food, dan-du (toxic sores), or fire-heat transformations, but is entirely due to consumptive weakness and qi stagnation. Treatment should solely rely on herbs that tonify qi and nourish the nutritive level (Ying); the sores will resolve spontaneously without requiring diaphoresis or purgation. If the differentiation of pulse and syndrome excess/deficiency is not carefully examined, and agents for cauterization/purging such as Liqi Yin or Shixuan San are indiscriminately used, one will violate meridian and disease contraindications, further damaging qi and blood, inevitably leading to a critical state. A surging pulse indicates exhaustion of primal qi and is incurable. A sallow white complexion indicates Metal overcoming Wood, which is also incurable. Red vessels penetrating the pupil indicate death within the number of years corresponding to the visible vessels. Failing to seek the root cause and recklessly using herbs that suppress the Liver is erroneous, as suppressing the Liver first injures Spleen Earth; injury to the Spleen damages the source of the Five Zang organs. How can one not be cautious?
Qi Shi stated: When lymphadenitis nodules are first perceived, internal resolution should be employed. If persistent, leading to gradual decline of qi and blood, cold muscles and flesh, thin pus, failure of toxic matter to exit, non-healing wounds, swelling without redness, nodules without pus, and unclear external signs, interior-supporting methods (Tuoli) are appropriate. For unformed pus, accelerate formation; for ulcerated sores, promote early granulation. For qi-blood deficiency, support the interior and tonify; for yin-yang disharmony, support the interior and harmonize. Generally, interior-supporting methods prevent deterioration of the sore, hence their suitability.
Danxi stated: Lymphadenitis originates in the Foot-Shaoyang meridian; ignoring taboos allows it to spread to the Foot-Yangming meridian. Rich diet and long-standing qi stagnation generate toxicity, wind, and heat. Progression requires differentiation of excess and deficiency; excess is easier to treat, while deficiency is concerning. This meridian governs decision-making and houses ministerial fire, possessing abundant qi and scarce blood. In women, if accompanied by irregular menstruation and alternating chills and fever progressing to tidal fever, only abstinence and bland diet can allow even a divine physician to cure it.
Lizhai stated: Swelling with a deep, rapid pulse indicates excess pathogenic factors, requiring drainage. Swelling, pain, aversion to cold, fever, or spasms indicate pathogenic factors at the exterior, requiring dispersal. Nodules caused by anger, with swelling, pain, or fever, require soothing the Liver and moving qi. Swelling and pain with a floating, rapid pulse require dispelling wind and clearing heat. A choppy pulse indicates primary blood tonification; a weak pulse indicates primary qi tonification. Hard, non-ulcerated swelling requires tonifying qi and blood. Depression-induced cases require resolving stagnation and regulating qi and blood. Non-healing post-ulceration indicates dual qi-blood deficiency, requiring strong tonification. Consumptive causes require tonification. Nodules preventing healing require removing necrotic tissue followed by tonification. Excess pulse with non-healing or non-resolving nodules requires purgation.
Furthermore, *Zhifa* states: Cases with chills, fever, and pain involve Liver fire and wind-heat affecting qi; use Xiao Chai Hu Tang to clear Liver fire and Jiawei Siwu Tang to nourish Liver blood. If chills and fever cease but nodules persist, this involves Liver meridian fire-dryness affecting blood; use Jiawei Xiaoyao San to clear Liver fire and Liuwei Dihuang Wan to generate Kidney water. High, soft swelling with sallow complexion and hot skin indicates formed pus; puncture with a needle and administer interior-supporting formulas. Chronic non-healing or recurrent cases with dripping pus and emaciation require pure tonics to achieve closure, otherwise evolving into nine types of fistula (Jiulou). *Neijing* states: Depressed meridians lead to fistulas lingering in muscle interstices, describing this disease. Externally apply Fermented Soybean Cake (Douchi Bing) and Amber Ointment (Hupo Gao) to disperse cold evil and restore yang; internally take Buzhong Yiqi Tang and Liuwei Wan to nourish Kidney water, cultivate Liver wood, and strengthen Spleen earth, offering potential for cure.
Furthermore, *Zhifa* states: Generally, this condition belongs to deficiency. Ignoring excess/deficiency differentiation violates contraindications and leads to error. Standard treatment begins with regulating menstruation and resolving stagnation, followed by Garlic-Separated Moxibustion, often resulting in spontaneous resolution. If unresolved, apply Amber Ointment. Once pus forms, puncture to prevent spread. If complicated by phlegm or yin deficiency, add corresponding herbs without disturbing other meridians. If qi and blood recover but nodules persist despite a month of scrofula-resolving herbs without effect and no qi-blood damage, then administer Bixiao San or Yuxian Wubi Wan, stopping after one dose to avoid toxicity. Follow with Yiqi Yangying Tang for regulation. For non-healing ulcers, apply Fermented Soybean Cake moxibustion and Amber Ointment. Cases with dual qi-blood deficiency or negligence in diet/lifestyle/emotions are incurable. However, this disease centers on qi and blood; robust individuals need not use cauterizing agents, as necrosis occurs naturally and removal facilitates healing. Weak individuals failing to use tonics first and repeatedly using cauterizing agents invite disaster. Cases with chills/fever and red vessels penetrating the pupil are incurable.
Moxibustion for Lymphadenitis: Select one point each at the junction of the acromion and olecranon processes, specifically Jianyu (LI15) and Quchi (LI11) of the Hand-Yangming Meridian. Moxa 7 cones per point. Treat left side for left lesions, right for right; bilateral lesions require bilateral treatment. I find this highly effective. Xue’s identification of Quchi as "Elbow Tip" is imprecise.
*Xue Shi Jingyan Fang* states: For formed/unformed or ulcerated/non-ulcerated lymphadenitis, place hand on shoulder, lift slightly to expose the olecranon tip, the moxa site. Moxa 30-40 cones, then take Yiqi Yangying Tang. Three sessions remove the sore. For chronic cases (3-4 years), moxa from Chen hour to Shen hour; three sessions cure, followed by tonics. This method emphasizes single-point Quchi moxa with high cone count. While prior methods are excellent, refractory cases require this. It is a secret method for men and women with Liver injury from anger and qi-blood stagnation, to unblock meridians. Locate points by pressing elbows and shoulders until numbness/aching is felt at the lesion.
Another method for non-purulent lymphadenitis: Apply 3-qian thick garlic slices to the lesion, moxa on top. Replace garlic every 3-5 cones. Moxa dozens of slices daily to extract stagnant toxin. For chronic non-healing ulcers, use Jiangxi Fermented Soybean powder mixed with saliva as a cake for moxa to boost yang. Internally take tonics, externally apply Amber Ointment or Taiyi Gao for closure. If the ulcer opens but the nodule does not necrose, hindering healing, or if Amber Ointment fails, apply Zhentou San to remove necrotic tissue, then Rushen San, followed by Yiqi Yangying Tang. For qi-blood deficiency, start with Yiqi Yangying Tang; once blood/qi improves, use Zhentou San while continuing the decoction.
Case 1: Male with hard, non-resolving, non-purulent swelling unresponsive to scrofula-resolving toxins. Moxa Jianjian and Zhoujian points, plus Yiqi Yangying Tang; cured in one month.
Case 2: Female with chronic ulceration, fever, delayed scanty menses. Used Xiaoyao San combined with previous decoction for two months; qi-blood restored, sore healed. One opening remained open; applied Zhentou San and moxa anterior points; cured. Chronic cases (2-3 years) cured by three consecutive moxa sessions plus interior-supporting herbs.
Case 3: Female with tender/swollen nodules from anger, presenting excess qi-blood. Initially purged with Bixiao San, then resolved with 30+ doses of Yiqi Yangying Tang. General rule: Tonify weak patients first with Yiqi Yangying Tang until qi-blood improves, then purge with Bixiao San to remove toxin, then resume tonics. Always effective.
Case 4: Mrs. Tian, >30 years old, chronic ulcerated lymphadenitis. Treated with Bazhen Tang plus Chaihu, Digupi, Xiakucao, Xiangfu, Beimu for 50+ doses; constitution improved. Given Bixiao San twice; wound closed. Qi-blood still unstable; resumed previous formula for 30+ doses; cured. Later, Mr. Tian used this formula indiscriminately without differentiation. He failed to realize Banmao (Mylabris) in the powder is toxic; excessive use damages primal qi. Excess cases may benefit from initial purgation followed by tonics. Deficient cases treated with purgatives/cauterizers retain stasis but fail to heal, becoming difficult. (All noted by Xue Ji).
Formula for Lymphadenitis Phlegm-Nodules: For early, mild lymphadenitis, take this formula; adding Xiakucao is beneficial.
Use Jin Yin Hua (Lonicerae Japonicae Flos) and Pu Gong Ying (Taraxaci Herba) 4-5 qian each. Decoct in 2 bowls of water. Drink morning and evening as tea. Gradually resolves in ~10 days. This treats symptoms; for root cure, moxa Jianyu and Quchi points.
