In the early stage of this condition, when pathogenic factors are clustered in the Lung, Jiegeng Xingren Jian (Platycodon and Apricot Seed Decoction) is the primary formula for treatment. Regarding new-onset etiology, see Article 33.
Qi Dezhi stated: The Lung is the "Huagai" (Canopy, covering the five Zang organs), located in the chest, governing climate regulation, and corresponding to the skin and hair. If labor and injury deplete Qi and Blood, causing the Couli (skin texture and pores) to become deficient, Wind pathogens may invade and affect the Lung internally. Consequently, symptoms such as sweating, aversion to wind, cough, shortness of breath, nasal congestion, neck stiffness, and distension/fullness in the chest and hypochondrium occur. If these persist without resolution, Fei Wei (Lung Flaccidity) develops. When Wind affects the Wei (Defensive) level, exhalation fails to enter deeply; when Heat reaches the Ying (Nutritive) level, inhalation fails to exit fully. Thus, Wind damages the skin and hair, while Heat damages the blood vessels. Wind and Heat struggle with each other, causing Qi and Blood to stagnate and accumulate in the Lung, transforming into sores and abscesses. Upon pulse diagnosis, a rapid but deficient pulse at the Cun Kou (Radial Artery) indicates Fei Wei (Lung Flaccidity); a rapid and excess pulse indicates Fei Yong (Lung Abscess). To determine if pus has formed: a slightly tight and rapid pulse suggests no pus yet; a significantly tight and rapid pulse indicates pus has formed. Symptoms of Fei Wei (Lung Flaccidity) include prolonged coughing due to excessive sweating leading to severe fluid depletion; sputum resembling rotten melon; discharge like pig fat; frequent urination without thirst. Thirst indicates spontaneous recovery; desire to drink indicates impending improvement. This presentation of profuse salivation without pus characterizes Fei Wei (Lung Flaccidity). Symptoms of Fei Chuang (Lung Sore) include dry mouth, panting, chest fullness, dry throat with thirst, slight swelling of the four limbs, coughing up pus and blood, or foul-smelling turbid froth, and dull pain in the chest, known as Fei Ju (Lung Ulcer). Additionally, *Shenghui Fang* states that dull pain at the Zhongfu (LU-1) acupoint indicates Fei Ju (Lung Ulcer); slight elevation of the flesh above indicates Fei Chuang (Lung Sore). Zhongfu is an acupoint name. Therefore, intervention at the initial stage can save the patient, but once pus forms, mortality is high. Furthermore, *Neijing* states: "Blood heat causes tissue decay and impedes Ying and Wei circulation, inevitably leading to pus." Generally, Fei Chuang (Lung Sore) presents with cough, shortness of breath, chest fullness, and expectoration of pus and blood resembling rice gruel over time, which is difficult to treat. Continuous vomiting of pus is incurable; spontaneous cessation of pus vomiting indicates recovery. A short and choppy pulse indicates self-healing; a floating, large, and洪 (surging) pulse indicates difficulty in treatment. A complexion that should be white but appears red indicates Fire overcoming Metal, which is incurable. Zhang Zhongjing stated: "Upper Qi with facial edema, shoulder breathing, and a floating-large pulse is untreatable. Diarrhea exacerbates the condition."
Ma Yiqing stated: The general principles for treating Fei Yong (Lung Abscess) involve first administering Xiao Qinglong Tang (Minor Blue Dragon Decoction) to resolve Wind-Cold pathogens, followed by Tingli Dazao Xiefei Tang (Descurainia Seed and Jujube Decoction to Drain the Lung), Jiegeng Tang (Platycodon Decoction), or Weijing Tang (Reed Rhizome Decoction) [see *Jingui Yaolue*] according to the syndrome to expel pus. This is the protocol for treating Swelling Sores (uncracked abscesses). Finally, Neibu Huangqi Tang (Internal Tonifying Astragalus Decoction) is used to tonify internal Yin Qi, which is the protocol for treating Ulcers (cracked abscesses). He also stated: If a swollen abscess ruptures and is re-infected by Wind, it is untreatable. Alternatively, Taiyi Gao Wan (Taiyi Paste Pills) may be taken, combined with Soufeng Tang (Search Wind Decoction) to induce vomiting. If pus and blood resembling lung abscess expectoration with foul odor are vomited and other formulas fail, Xiaofeng San (Eliminate Wind Powder) with male hair ash, mixed with clear water, can remove the condition after two doses.
Xue Lizhai stated: Any condition where labor injures Qi and Blood, making Couli (skin texture and pores) loose, allowing external pathogens to invade and affect the Lung internally; or excessive sexual activity depleting Kidney Water, causing Deficient Fire to flare upward; or consumption of strong wine, roasted/fried foods, and spicy/concentrated flavors steaming the Lung; or excessive coughing/salivation and improper sweating/purging leading to severe fluid depletion, can all cause this disease. Symptoms include aversion to wind, cough, nasal congestion, neck stiffness, chest/hypochondriac distension, breathing difficulty, dry throat with thirst, slight limb swelling, and coughing up pus and blood. Expectoration of foul/turbid sputum, bloody/foul pus, and dull chest pain with a rapid/excess pulse at the right Cun Kou indicates Fei Ju (Lung Ulcer). Salivating froth without pus and a rapid/deficient pulse indicates Fei Wei (Lung Flaccidity).
Additionally, *Zhifa* states: Generally, labor injuring Qi and Blood makes Couli loose; Wind pathogens attack the Lung; Wind and Heat struggle and accumulate, inevitably causing cough. Improper excessive sweating or purging leads to severe fluid loss, resulting in this syndrome. For panting/cough with urgency and chest fullness, use exterior-releasing methods. For cough with fever, use harmonizing methods. For cough with dull chest/diaphragm pain and foul sputum, use Paimo San (Pus-Expelling Powder). For urgent panting with mental confusion and profuse phlegm, soothe the Lung. For coughing pus with a short/choppy pulse, use tonification.
Furthermore, *Zhifa* states: For cough with urgent panting, use Xiao Qinglong Tang (Minor Blue Dragon Decoction). For cough with chest distension, use Tingli Dazao Xiefei Tang (Descurainia Seed and Jujube Decoction to Drain the Lung). For coughing foul/turbid pus, use Jiegeng Tang (Platycodon Decoction). For cough/panting with shortness of breath or scanty urine, supplement with Renshen Bufei Tang (Ginseng Lung-Tonifying Decoction). For fatigue and poor appetite, supplement with Shenzhu Bupi Tang (Ginseng and Atractylodes Spleen-Tonifying Decoction). For coughing with obstructive phlegm due to Kidney Deficiency causing Water Flooding, use Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia). For dry mouth/throat due to Deficient Fire flaring upward, use Jiaban Bawei Wan (Modified Eight-Ingredient Pill). This condition arises because Spleen Earth deficiency fails to generate Lung Metal, and Lung Metal fails to generate Kidney Water. Thus, early stages are curable, but post-pus formation is often fatal. Tonifying Spleen/Lung and nourishing Kidney Water offers hope. Specifically attacking the sore further weakens Spleen/Stomach, rarely avoiding failure.
The son of Lu Siting, in spring, presented with coughing foul sputum, chest fullness, shortness of breath, lusterless skin, neck stiffness, and rapid pulse, indicating Fei Ju (Lung Ulcer). Since the Lung meridian connects to the neck, Lung injury damages the connection, causing牵引 (traction) preventing turning. As the Lung is the root of Qi, its brilliance manifests in hair, and it fills the skin, treatment used Huangqi (Astragali Radix), Danggui (Angelicae Sinensis Radix), Chuanxiong (Chuanxiong Rhizoma), Baizhi (Angelicae Dahuricae Radix), Beimu (Fritillariae Bulbus), Zhimu (Anemarrhenae Rhizoma), Maidong (Ophiopogonis Radix), Gualou Ren (Trichosanthis Semen), Jiegeng (Platcodonis Radix), Fangfeng (Saposhnikoviae Radix), and Gancao (Glycyrrhizae Radix), combined with Lafan Wan (Beeswax and Alum Pill) and Taiyi Gao (Taiyi Paste). Recovery occurred when pus was exhausted and the pulse became choppy.
A man with a pale complexion and mental fatigue, coughing with dull chest/diaphragm pain and a slippery-rapid pulse, was diagnosed with Fei Yong (Lung Abscess). I proposed Jiegeng Tang (Platycodon Decoction), but he refused, continuing with exterior-releasing herbs, worsening the cough and producing foul sputum. Realizing his error, he took the previous formula for four doses, calming the cough. Four doses of Sishun San (Four-Smooth Powder) stabilized the pulse, followed by several doses of Tuoli herbs (Interior-Supporting Herbs) for recovery.
A man with cough, urgent panting, fever, irritability, red face, sore throat, and surging-large pulse improved slightly after two doses of Huanglian Jiedu Tang (Coptis Decoction to Resolve Toxicity) and settled after four doses of Zhizi Tang (Gardenia Decoction).
A man with Fei Wei (Lung Flaccidity) presented with cough, urgent panting, foul sputum, chest fullness, dry throat, and surging-rapid pulse. Six doses of Renshen Pingfei San (Ginseng Lung-Calming Powder) and drinking Tongbian (Child Urine) resolved most symptoms, followed by Ziwan Rong Tang (Aster Bud Decoction) for complete recovery. Tongbian (Child Urine) is specific for Deficient Fire and effective for sores/swelling, fever/thirst, and Fei Wei (Lung Flaccidity)/Fei Yong (Lung Abscess) with fever/thirst.
A man with red face, pus expectoration, fever/thirst, irritability, and chaotic surging-rapid pulse received one large dose of Jiaban Bawei Wan (Modified Eight-Ingredient Pill) with Maidong (Ophiopogonis Radix), instantly stopping fever/thirst. After deep sleep, he woke refreshed and hungry. Two doses reduced symptoms significantly. Maintenance involved the previous formula plus Renshen (Ginseng Root) 5 qian, Maidong (Ophiopogonis Radix) 2.5 qian, and Wuweizi (Schisandrae Fructus) 2 qian, decocted as tea, one dose daily, resolving the condition within a month. Red face requires tonifying Lung/Kidney; pale face requires tonifying Spleen/Lung. Physicians must discern this.
A woman with inherent Blood deficiency presented with fever/cough. After using Phlegm-Fire formulas, she vomited pus/blood, had a red face and rapid pulse, indicating critical status due to pus formation and Qi/Blood deficiency. Bazhen Tang (Eight-Treasure Decoction) tonified Yuan Qi, and Jiegeng Tang (Platycodon Decoction) treated the Lung condition, leading to gradual recovery.
A scholar with Fei Yong (Lung Abscess) had clear nasal discharge, coughing pus/blood, and chest distension, caused by external Wind pathogens. Xiaofeng San (Eliminate Wind Powder) with human hair ash cleared the nose in two doses. Sijunzi Tang (Four Gentlemen Decoction) plus Chuanxiong (Chuanxiong Rhizoma) and Danggui (Angelicae Sinensis Radix), along with Jiegeng Tang (Platycodon Decoction), cured him. Later, due to labor, he coughed pus, had dribbling urine, and yellow-white complexion, indicating Spleen Earth failing to generate Lung Metal, and Lung Metal failing to generate Kidney Water. Buzhong Yiqi Tang (Middle-Boosting Qi-Invigorating Decoction) and Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) cured him.
A servant over thirty years old had chronic cough, Qi obstruction, restless sleep, hemoptysis, and extreme weakness, abandoned by his physician. Ningfei San (Lung-Calming Powder) provided slight relief in one dose and significant reduction in the next. Several doses of Ningfei Tang (Lung-Calming Decoction) achieved recovery. As the saying goes, "For this disease, use this medicine." If one hesitates due to the astringent nature of the powder, recovery would not have been possible. (Commentary by Xue Ji)
