一、命门火衰，精气虚寒而阳痿者，宜右归丸、赞育丹、石刻安肾丸之类主之。若火不
甚衰，而止因血气薄弱者，宜左归丸、斑龙丸、全鹿丸之类主之。
一、凡因思虑惊恐，以致脾肾亏损而阳道痿者，必须培养心脾，使胃气渐充，则冲任始
振，而元可复也。宜七福饮、归脾汤之类主之。然必大释怀抱，以舒神气，庶能奏效。否则
徒资药力无益也。其有忧思恐惧太过者，每多损抑阳气，若不益火，终无生意，宜七福饮加
桂附枸杞之类主之。
一、凡肝肾湿热，以致宗筋弛纵者，亦为阳痿。治宜清火以坚肾，然必有火证火脉，内
外相符者，方是其证。宜滋阴八味丸，或丹溪大补阴丸、虎潜丸之类主之。火之甚者，如滋
肾丸、大补丸之类俱可用。
薛立斋曰∶按阴茎属肝之经络。盖肝者，木也。如木得湛露则森立，遇酷暑则萎悴。若
因肝经湿热而患者，用龙胆泻肝汤以清肝火，导湿热；若因肝经燥热而患者，用六味丸以滋
肾水、养肝血而自安。又曰∶琼玉膏、固本丸、坎离丸，此辈俱是沉寒泻火之剂，非肠胃有
燥热者不宜服。若足三阴经阴虚发热者，久而服之，令人无子。盖损其阳气，则阴血无所生
故也，屡验。
一、命门火衰（肾阳不足），精气虚寒导致阳痿的，适宜用右归丸、赞育丹、石刻安肾丸这类方剂为主进行治疗。如果命门之火并不十分衰弱，仅仅是因为血气薄弱导致的，适宜用左归丸、斑龙丸、全鹿丸这类方剂为主进行治疗。
一、凡是因思虑过度或惊恐，导致脾肾亏损而引发阳痿的，必须培补心脾，使胃气逐渐充盛，那么冲任二脉才能振奋，阳气才能恢复。适宜用七福饮、归脾汤这类方剂为主进行治疗。然而必须大大开阔胸怀，以舒展神气，这样才能奏效。否则仅仅依靠药力是没有益处的。那些忧愁思虑恐惧太过的人，往往多有损伤压抑阳气的情况，如果不温补命门之火，终究没有生机，适宜用七福饮加上肉桂、附子、枸杞等药物为主进行治疗。
一、凡是肝肾湿热，导致宗筋（男性生殖器）弛纵无力的，也属于阳痿。治疗应当清泻火热以坚固肾脏，但必须有实火的症状和脉象，内外相符的，这才是该证候。适宜用滋阴八味丸，或者丹溪大补阴丸、虎潜丸这类方剂为主进行治疗。火邪严重的，如滋肾丸、大补丸这类方剂都可以使用。
薛立斋说：按阴茎归属于肝的经络。因为肝属木。就像树木得到露水滋润就会挺拔直立，遇到酷暑就会枯萎憔悴。如果是因肝经湿热而患病的，用龙胆泻肝汤来清泻肝火，引导湿热排出；如果是因肝经燥热而患病的，用六味地黄丸来滋补肾水、滋养肝血从而使身体安宁。又说：琼玉膏、固本丸、坎离丸，这些都属于性质沉寒、泻火的药剂，如果不是肠胃有燥热的人不宜服用。如果是足三阴经阴虚发热的人，长期服用的话，会导致人无法生育。这是因为损伤了阳气，阴血就无法生成的缘故，多次验证如此。
一方：治疗阳事不能勃起，用蛇床子、五味子、菟丝子等量研为细末，蜂蜜炼制成丸，梧桐子大小，每次服用三五十丸，温酒送下，每天服用三次。

When Mingmen Fire (Fire of the Gate of Life, referring to Kidney Yang) is deficient, and Essence and Qi are cold and deficient leading to impotence, formulas such as Yougui Wan (Right Restoring Pill), Zanyu Dan (Nourishing Fertility Elixir), and Shike Anshen Wan (Stone-Carved Kidney-Calming Pill) should be primarily used. If Mingmen Fire is not significantly deficient, but the condition arises solely from thinness and weakness of Blood and Qi, formulas such as Zuogui Wan (Left Restoring Pill), Banlong Wan (Spotted Dragon Pill), and Quanlu Wan (Whole Deer Pill) should be primarily used.
1. In cases where excessive worry or fright causes deficiency and decline of the Spleen and Kidney, resulting in impotence, it is essential to nourish the Heart and Spleen so that Stomach Qi gradually becomes sufficient; only then can the Chong and Ren vessels (Penetrating and Conception Vessels) be revitalized, and Yuan Qi (Original Qi) restored. Formulas such as Qifu Yin (Seven Blessments Decoction) and Guipi Tang (Restore the Spleen Decoction) should be primarily used. However, one must greatly broaden their outlook to soothe the Spirit and Qi; otherwise, relying solely on medicinal power will be of no benefit. For those with excessive sorrow, worry, or fear, who often suffer from suppression of Yang Qi, if Mingmen Fire is not supplemented, there will ultimately be no vitality. In such cases, Qifu Yin added with Rougui (Cortex Cinnamomi), Fuzi (Radix Aconiti Lateralis Preparata), and Gouqi (Fructus Lycii) should be primarily used.
1. In cases where Damp-Heat in the Liver and Kidney leads to flaccidity and relaxation of the Zongjin (Tendons of the Genitals, referring to the penis), this also constitutes impotence. Treatment should focus on clearing Heat to firm the Kidney. However, this diagnosis applies only when there are clear signs and pulses of Excess Heat, consistent internally and externally. Formulas such as Ziyin Bawei Wan (Yin-Nourishing Eight-Ingredient Pill), or Danxi Dabu Yin Wan (Danxi Great Tonifying Yin Pill) and Huqian Wan (Tiger-Hiding Pill) should be primarily used. For severe Heat, formulas such as Zishen Wan (Kidney-Nourishing Pill) and Dabu Wan (Great Tonifying Pill) may all be used.
Xue Lizhai stated: The penis belongs to the meridians of the Liver. Since the Liver corresponds to Wood, just as trees stand erect when bathed in dew, they wither under scorching heat. If the disease is caused by Damp-Heat in the Liver meridian, Longdan Xiegan Tang (Gentian Drain the Liver Decoction) should be used to clear Liver Heat and guide out Damp-Heat. If the disease is caused by Dry-Heat in the Liver meridian, Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) should be used to nourish Kidney Water and nourish Liver Blood, thereby bringing peace to the body. He further stated: Qiongyu Gao (Jade Pearl Paste), Guben Wan (Root-Firming Pill), and Kanli Wan (Kan-Li Pill) are all agents of deep cold that drain Fire; they are not suitable for those without Dry-Heat in the intestines and stomach. If patients with Deficiency-Heat due to Yin deficiency of the three Yin meridians of the foot take these for a long time, it will render them infertile. This is because damaging Yang Qi prevents the generation of Yin Blood. This has been verified repeatedly.
A prescription for treating inability to achieve erection: Use equal parts of Chuanshangzi (Semen Cnidii), Wuweizi (Fructus Schisandrae), and Tusizi (Semen Cuscutae), grind into fine powder, make pills with honey, sized like wutong seeds. Take 30–50 pills per dose, washed down with warm wine, three times daily.
