I. Whenever there is swelling, redness, and pain caused by wind-heat, this must be a condition of mumps (swelling of the parotid glands), epidemic toxic heat (warm-toxic pathogens with contagious nature), or carbuncles and sores. The discussion on diagnosis and treatment is detailed in the Surgery section; one should refer to it for management. If the condition is severe, Fangfeng Tongsheng San (Saposhnikovia and Gardenia Powder) should be used as the primary formula.
II. Flaccid edema of the face and eyes may result from excessive sexual activity, leading to Yin deficiency and floating of Qi. In such cases, Liuwei Dihuang Tang (Six-Ingredient Rehmannia Decoction) or Bawei Dihuang Tang (Eight-Ingredient Rehmannia Decoction) or Jiawei Bawei Wan (Modified Eight-Ingredient Rehmannia Pill) are appropriate. If the flaccid edema is caused by fatigue and overwork damaging the Spleen, resulting in Qi deficiency failing to contain fluids, Shenling Baizhu San (Ginseng, Poria, and Atractylodes Macrocephala Powder) or Guipi Tang (Restore the Spleen Decoction) or Shiquan Dabu Tang (Ten Complete Tonifying Decoction) are suitable. If the edema is due to excessive alcohol consumption causing damp-heat to accumulate upward, Gegua Jiecheng Tang (Pueraria Flower Decoction to Relieve Intoxication) or Qiwei Baizhu San (Seven-Ingredient Atractylodes Macrocephala Powder) are indicated. If persistent diarrhea or dysentery leads to Spleen and Kidney Qi deficiency causing facial edema, Weiguan Jian (Stomach Gate Decoction) or Wenwei Yin (Warming Stomach Infusion) should be used. If improper diet causes Yangming channel stagnation and excess, with constipation of both stool and urine leading to fullness and distension of the head and face, Kuqing Yin (Clearing Purge Infusion) is appropriate. This condition is common in children; if severe, Muxiang Binglang Wan (Aucklandia and Areca Nut Pill) should be used to purge. If Yangming excess heat and Stomach fire float upward, accompanied by irritability, fever, dry mouth, and thirst, along with facial edema, Chouxin Yin (Drawing Out the Firewood Infusion), Baihu Tang (White Tiger Decoction), or Dali Qingyin (Great Clearing Purification Infusion) should be used to clear heat and promote urination.
III. Edema that is visibly swollen, or where the area under the eyes resembles a silkworm lying down, is caused by water qi pathology. The discussion and treatment are detailed in the "Swelling" chapter.
IV. Various formulas for acne and freckles on the face and nose are listed after Volume 8, Chapter 7.
