The Ying (Nutrient) Blood is originally deficient, and the Yin Fire is hyperactive, with the responsibility lying in congenital insufficiency. The Spleen Qi of the acquired constitution is not robust; Liver Wood overacts upon it. The ingested food generates Phlegm and Fluid Retention, which are stored in the Stomach. If they can still be expelled through vomiting, peace may be maintained without incident. However, if delayed for a long time, they invade outside the Membranous Origin (Mo Yuan), the Qi mechanism becomes unclear, and distension arises spontaneously. The Spleen is the source of Phlegm production, and the Stomach is the vessel storing Phlegm. Unless the central Middle Jiao is transformed and transported, and the exterior of the membranous origin is penetrated, the pain will only increase, leading to fullness and distension, which is easily achieved. Nevertheless, the Spleen governs the Blood, and the Liver stores the Blood; after a long illness, the Blood becomes even more depleted. Treatment focuses on resolving Phlegm but must be assisted by harmonizing and nourishing methods. The ancients' use of moistening and drying agents together was precisely designed for such conditions.

When urination and defecation exchange their positions, it is named "Jiao Chang" (Crossed Intestines Syndrome). This is caused by sudden Qi chaos in the middle, constituting an acute disease. If delayed for a long time, filth from the intestines merges into the Bladder, and all drinks and foods exit via urination. Cases that seem similar to Jiao Chang syndrome but are actually different are mostly due to internal obstruction by Blood Stasis. The Large Intestine becomes dysfunctional and unused; the You Men (Pylorus) becomes a smooth path; the Lan Men (Ileocecal Valve) fails to perform its function of separating clear from turbid. Abandoning the old for the new, abandoning the wide for the narrow, the condition becomes increasingly obstructed day by day. One fears that the Yuan Qi (Original Qi) will fail and collapse. At this stage, treatment must restore the original pathway, allowing Blood Stasis to gradually dissipate, which would be close to reason.

The patient's illness stems from a constitutional predisposition to Damp-Heat, complicated by Yin Deficiency symptoms. Where does Dampness come from? It is the Qi of the Earth. Where does Heat come from? It is the Qi of Heaven. When Heaven and Earth are stagnant and steaming, Damp-Heat arises; when Heaven and Earth interact harmoniously, vitality is nurtured. The miraculous mechanism of endless generation blends within this, which humans inherit and receive. Damp-Heat and Original Qi mix as one family, originating from congenital sources, firm and unbreakable. Compared to Damp-Heat arising from external pathogens or internal injuries, which belong to the acquired constitution, this is far more profound and cannot be compared. If one eats bland food and avoids sexual excess, one would merely suffer from sores or bleeding, similar to minor illnesses in youth. However, indulging in rich, greasy foods and excessive desires increases external Damp-Heat and consumes internal Yin Essence. Consequently, the Zang-Fu organs and Ying-Wei (Nutrient-Defensive) systems possess the qualities of Spring and Summer (expansive) but lack those of Autumn and Winter (contractive). No wonder, in the year of Hai, Wind and Fire fan each other, causing ear tinnitus. Is this not a verification of Yang Qi surging upward like all things in abundance? At such a time, if one rests and nourishes quietly, aside from symptoms like cold umbilicus and toothache, one could remain peaceful. Why did the Fire add oil to the flame, seeking merit and trouble, suddenly causing dizziness and facial redness, presenting a blazing trend? Even to the point where extreme Fire resembles Water, Yang ceases to be Yang, and Heat ceases to be Heat. The Fire of the Liver Meridian and the Yang of the Du Mai (Governing Vessel) also attack upwards, losing their original nature. Recently, I heard of "Guiding Fire back to its Source," believing that sweet-warm herbs can treat Fire-Heat disorders. Alas! This has not heard the true Dao. Sweet-warm herbs can treat great heat only when there is extreme Yin cold at the lower extremity, causing True Yang to float upward; guiding the fire back to its source allows Kan and Li to interact, and Taiji remains at peace. But if Taiyin Damp-Heat steams upward, using clearing and moistening methods is already difficult to manage; how dare one use Fire to feed Fire, clearly violating the warning against repeating mistakes? Although there are methods for clearing and many for nourishing, allowing diet to increase and body weight to gain, appearing sufficient externally. Yet, the inability to stand or sit for long indicates that in the pathology, "the Yin and Yang of all things are unsettled and have no master." Facing this situation of external excess with internal emptiness, lower deficiency with upper excess, it is truly difficult to find effective medication.

Li Dongyuan's Qing Zao Tang (Clear Dryness Decoction) and Danxi's Hu Qian Wan (Tiger Crouching Pill) combine moistening and drying appropriately, coordinating rigid and soft properties. Zhang Shi often praised these two formulas, relying on them. Why not use them immediately? Unfortunately, the carrier of medicinal power is Stomach Qi. The Stomach belongs to Yangming, the Central Earth where all things return. Damp-Heat has occupied it for a long time, steaming and transmitting into hematemesis, coughing with phlegm, nasal congestion, belching, and irregular Qi and bowel movements. This is exactly "Nine Orifices Disharmony, all belonging to Stomach Disease." To calm the internal Zang organs, one must first clear the external Fu organs, which is a crucial step. Regarding the worsening of illness in late autumn and early winter: in the tenth month, Kun Gua (Earth Hexagram) is purely Yin, and Heaven is quiet, yet Damp-Heat moves contrary to this; the Kidney desires to store, yet Damp-Heat exposes it contrary to this. Thus, evil causes Zheng (Righteous Qi) to lose control, inevitably pushing the patient's condition to a deeper level. Attached formula is respectfully presented.
