Xiao Banxia Tang treats Huoluan (cholera-like syndrome) with vomiting of saliva/phlegm, where the physician mistakenly used purgatives, resulting in epigastric stagnation. (Same source) (Note: *Qianjin Fang* uses Gancao Xiexin Tang for this condition, which originates from *Jingui Yaolue · Women's Diseases* chapter.)
Sipian Jin treats exposure to summer heat and latent heat, abdominal pain with diarrhea or dysentery, and excessive drinking. It treats Huoluan (cholera-like syndrome) caused by drinking cold liquids, exposure to cold, hunger, anger, or motion sickness damaging stomach Qi, leading to upper vomiting and lower diarrhea simultaneously, causing dizziness, vertigo, muscle cramps in hands/feet, and cold limbs. Delayed treatment may lead to death within moments.
Wuzhuyu (Evodiae Fructus, half liang), Gan Mugua (Chaenomelis Fructus, half liang), Yanshi (Sal, half liang).
Stir-fry the above three ingredients together until scorched. Boil three sheng of water in a porcelain bottle until boiling vigorously. Add the fried herbs and simmer until reduced to less than two sheng. Pour out one zhan and administer according to patient's preference for temperature. Stop immediately upon reaching the throat. In emergencies without these herbs, use only Kubaifan (Alumen) powdered, one daqian with boiling water. Without Alumen, use a pinch of salt and one zhan of vinegar, simmer to eight parts, take warm. Salt plums or other sour/salty items can also be decocted and taken. (*Jiabao Fang*) (Note: This formula is named Wuzhuyu Tang in *Qixiao Liangfang*. Also, *Taiping Shenghui Fang* uses Mugua (Chaenomelis Fructus), Shengjiang (Zingiberis Rhizoma Recens), and Wuzhuyu (Evodiae Fructus) for post-Huoluan (cholera-like syndrome) leg cramps, likely a variation of this formula.)
Huoxiang Zhengqi San is generally used for Huoluan (cholera-like syndrome) with vomiting and diarrhea. Add Rougui (Cinnamomi Cortex) for abdominal pain, add Fuling (Poria) for difficult urination. This formula warms and disperses wind-cold-dampness. Add Xiangru (Moslae Herba) for summerheat conditions. (*Zhizhi Fang*) *Xu Yijian Fang*'s Huoxiang Zhengqi San removes Cangzhu (Atractylodis Rhizoma) from Buhuanjin Zhengqi San. It states: For Huoluan (cholera-like syndrome) with vomiting and diarrhea, add Baizhu (Atractylodis Macrocephalae Rhizoma). *Huichun Fang* suggests Jiajian Zhengqi San for travelers experiencing水土不服 (acclimatization issues) with vomiting/diarrhea, chest/abdominal fullness, or swelling without vomiting/diarrhea, adding Shanren (Amomi Fructus) and Xiangfu (Cyperi Rhizoma) to Buhuanjin Zhengqi San.
Draining and Promoting Urination Methods
Upon feeling this syndrome, first consider Wuling San (Five-Ingredient Powder with Poria), Yiyuan San (Benefit Origin Powder), or Guiling Ganlu Yin (Cassia-Gastrodia Sweet Dew Decoction), which are holy medicines for vomiting and diarrhea. (*Baoming Ji*)
Fuling Zexie Tang treats post-Huoluan (cholera-like syndrome) vomiting/diarrhea with irritability, thirst, and desire to drink. (*Sanyan Jingyi Bingzheng Fanglun*) (Note: This is the original formula from *Jingui Yaolue*. However, Wuling San (Five-Ingredient Powder with Poria) is still considered superior.)
Official Shiqi, aged 79, suffered from summerheat toxicity on the 4th day of the 6th month in Bingyin year of Zhiyuan. He had Huoluan (cholera-like syndrome) with vomiting and diarrhea, unconsciousness all day, unresponsive to stimuli. At midnight, he requested my treatment. His pulse was surging, large, and forceful, beating 7-8 times per breath. His head was hot as fire, feet cold as ice, hemiplegia, and lockjaw. I recalled *Neijing · Wuluan Pian*: "Clear Qi in Yin, Turbid Qi in Yang, Ying Qi follows vessels, Wei Qi moves counter-current. Chaos in the chest is called Great Chaos... Chaos in intestines/stomach becomes Huoluan." Thus the name Huoluan originated. Due to old age and weak Qi, he could not withstand summerheat. Yang failing to anchor Yin causes diarrhea; Yin failing to anchor Yang causes vomiting. When Yin and Yang fail to anchor each other, both vomiting and diarrhea occur. Former sages treated cold patterns with Lizhong Wan and heat patterns with Wuling San (Five-Ingredient Powder with Poria) as standard methods. Now, summerheat is extreme, Yangming is in season, and since it was acquired through activity, internal heat is clear. Only sweet-acrid great-cold formulas can drain summerheat, subdue floating fire, and calm the Spirit. I used Ganlu San (Sweet Dew Powder), sweet-acrid and great-cold, to drain heat and tonify Qi, added Baifuling (White Poria) to differentiate Yin and Yang, weighing about one liang. Administered with ice water, he gradually regained consciousness and symptoms resolved. After three days of restricting speech and diet, I adjusted Shenzhu Tiaozhong Tang to regulate Qi and restore health. Over ten days later, he fully recovered. (*Baojian*)
Guiling Ganlu Yin moistens dryness, treats phlegm-saliva, stops cough, regulates organs, treats cold-heat vomiting, promoting smooth Qi flow throughout the body, and treats edema and diarrhea.
Guigui (Cinnamomi Ramulus), Huoxiang (Agastaches Herba), Ren Shen (Ginseng Radix et Rhizoma) (each half liang), Muxiang (Aucklandiae Radix, one fen), Fuling (Poria), Baizhu (Atractylodis Macrocephalae Rhizoma), Gancao (Glycyrrhizae Radix et Rhizoma), Zexie (Alismatis Rhizoma), Gegen (Puerariae Lobatae Radix), Shigao (Gypsum Fibrosum), Hanshuishi (Halloysitum Rubrum) (each one liang), Huashi (Talcum, two liang).
Grind the above twelve ingredients into powder. Take three qian per dose, mixed with white water, new water, or ginger soup. (Same source) (Note: This formula originates from *Sanfa Liu Men*.)
Jiang Yingsu treated a woman in mid-June with Huoluan (cholera-like syndrome), vomiting, diarrhea, and muscle cramps. A physician prescribed Huoxiang Zhengqi San, worsening her condition with irritability, red face, and restlessness (pulling clothes off, lying on ground). My diagnosis showed a deficient, powerless pulse, body heat, and thirst. This was caused by summerheat damage. I used sweet-acrid great-cold formulas to drain fire, prescribing Wuling San (Five-Ingredient Powder with Poria) plus Huashi (Talcum) and Shigao (Gypsum Fibrosum). Once vomiting/diarrhea stopped, I administered Guiling Ganlu Yin (Cassia-Gastrodia Sweet Dew Decoction) and she recovered. (*Mingyi Leian*)
Jiawei Wuling Tang treats thirst and body heat due to summerheat damaging the middle.
Zhuling (Polypori Umbilicati Sclerotium, seven fen), Zexie (Alismatis Rhizoma, seven fen), Baizhu (Atractylodis Macrocephalae Rhizoma, five fen), Chi Fuling (Red Poria, one qian), Tianhuafen (Trichosanthis Radix, two qian), Gan Ge (Puerariae Lobatae Radix, one qian), Xiangru (Moslae Herba), Huanglian (Coptidis Rhizoma), Gancao (Glycyrrhizae Radix et Rhizoma) (equal parts).
If heat is extreme, add Shigao (Gypsum Fibrosum) and Zhimu (Anemarrhenae Rhizoma). Boil one and a half zhong of water with three slices of ginger until reduced to seven parts. Take warm. (*Zhongmiao Fang*) *Yijian* names this Jiawei Ru Ling Tang, adding Bian Dou (Phaseoli Semen).
Chaihu Siling Tang is used for excessive thirst and difficult urination.
Chaihu (Bupleuri Radix), Huang Qin (Scutellariae Radix), Shaoyao (Paeoniae Radix Alba) (each one qian), Baizhu (Atractylodis Macrocephalae Rhizoma), Fuling (Poria), Zhuling (Polypori Umbilicati Sclerotium), Zexie (Alismatis Rhizoma) (each eight fen).
Boil two zhong of water until reduced to eight parts. Strain, add one spoonful of Jiangzhi (Ginger Juice). Take cold. (*Shanghan Zhengzhi Mingtiao*) For disharmony of summerheat and food, post-vomiting/diarrhea abdominal distension, borborygmus, difficult urination, or persistent diarrhea, treat with Weiling Tang. (*Yuanbing Ji*) (Note: Based on *Lingnan Weisheng Fang*.)
Summerheat-Resolving Formulas
*Guangji Fang* Bian Dou Tang treats Huoluan (cholera-like syndrome) with vomiting and dysentery.
Biandou Ye (Lablab Leaf, cut one sheng), Xiangru Ye (Mosla Leaf, cut one sheng), Mugua (Chaenomelis Fructus, one piece cut), Ganjiang (Zingiberis Rhizoma, one liang cut).
Boil the above four ingredients in six sheng of water to obtain two sheng five he. Strain and take warm in three divided doses, spaced by the time it takes to walk six or seven li. (*Waitai Miyao*) *Jifeng Fang* Caodoukou San treats persistent Huoluan (cholera-like syndrome) with vomiting/diarrhea and fine-tight pulse indicating internal cold. It removes Biandou (Lablab Seed) from this formula, adding Caodoukou (Alpiniae Katsumadai Semen), Huoxiang (Agastaches Herba), and Chenjupi (Citri Reticulatae Pericarpium).
Formula for Huoluan (cholera-like syndrome) and Spleen-Stomach Qi deficiency with abdominal distension and anorexia.

Crush and sift the above ingredients into a powder. Divide into four doses. Each dose boiled with one zhan of water, two jujubes, and half slice of Shengjiang (Zingiberis Rhizoma Recens) until reduced to six parts. Strain and take warm at any time. (*Taiping Shenghui Fang*)
Wubi Xiangru San treats irregular vomiting/diarrhea, Huoluan (cholera-like syndrome) with restlessness/thirst, umbilical/abdominal cramping pain, muscle cramps, cold sweat over the body, and cold limbs. (*Chuanjia Mibao*) (Formula seen in *Zhongshu* chapter.) *Huo Ren Shu* states that for summer Huoluan (cholera-like syndrome) due to summerheat, Xiangru San should be taken, submerged in well water until extremely cold, and taken in one dose for efficacy. (*Bencao Yan Yi* says: Mosla is eaten as a vegetable in summer; indispensable for treating Huoluan (cholera-like syndrome); always effective.)
Zengsun Suopi Yin treats post-Huoluan (cholera-like syndrome) irritability caused by excessive intake of warming herbs. Soaked until ice-cold, especially suitable. (*Yijian Fang*) (Formula seen in *Zhongshu* chapter.)

Finely chop and mix the above ingredients. Take one fen per dose, boil with one zhan of water, seven cun of Xiebai (Allii Macrostemonis Bulbus), and half slice of Shengjiang (Zingiberis Rhizoma Recens) until reduced to six parts. Strain and take slightly warm at any time. (*Taiping Shenghui Fang*) Another formula, Lugen San, for Huoluan (cholera-like syndrome) with mental restlessness and dry retching, removes Huoxiang (Agastaches Herba) and Xiebai (Allii Macrostemonis Bulbus) from this formula, adding Chenjupi (Citri Reticulatae Pericarpium) and Mugua (Chaenomelis Fructus).
Formula for Huoluan (cholera-like syndrome) with heart/abdominal distension/pain, fullness, shortness of breath, before vomiting/diarrhea occurs.
Take Liaoruo Ye (Polygoni Multiflori Vine Leaf), finely cut two sheng. Boil in five sheng of water for three boils. Take in one dose. (*Zhouhou Beiji Fang*) Formula for Huoluan (cholera-like syndrome) muscle cramps: Take one handful of Liao (Polygonum hydropiper), remove ends, boil in two sheng of water to obtain one sheng. Take in one dose. (*Qianjin Fang*) (Note: Ancient formulas used Polygonum for Huoluan, hence listed here.)
Heat-Clearing Formulas

Emetic Formulas
Formula for Huoluan (cholera-like syndrome) with heart/abdominal distension/pain, fullness, shortness of breath, before vomiting/diarrhea occurs. Two sheng of salt boiled in five sheng of water to obtain two sheng. Take in one dose to induce vomiting and cure. (*Zhouhou Beiji Fang*) (Note: Based on *Jingui Yaolue* formula for gluttony, undigested food, hard/full painful abdomen.) *Duanju Fang* treats sudden heart/abdominal pain, undigested food, or chronic cold causing fullness/Huoluan (cholera-like syndrome). Make three sheng of salty soup, make it very salty, drink one sheng hot, stimulate throat to vomit (*Qianjin Fang* stimulates throat to vomit) until undigested food is gone. If not vomiting, repeat. After vomiting, drink again. Three emetic cycles are best. Rest quietly. Superior to other decoctions/pills. (*Yi Xin Fang*) *Qianjin Fang* Huoluan Gudu (Note: These are two conditions), undigested food, accumulated cold, heart/abdominal fullness, Guiqi formula (Same as Duanju). Commoners consider this a rustic method and disdain it, preferring to die. For such diseases, this must be used first. *Defang* uses half a zhan of salt soaked in several bowls of hot water, have patient drink all, up to several bowls without stopping. Then sweep throat with chicken feather to induce immediate vomiting. All salt soup expelled, condition cured. (*Yifang Jijie* burns salt, mixes with hot water, induces vomiting with finger.)
Liu Zongyuan of Tang compiled Salt Soup Formula to Save Three Deaths and Treat Huoluan (cholera-like syndrome): In the 10th month of the 11th year of Yuanhe, I suffered from Gan Huoluan (dry cholera-like syndrome), unable to vomit or purge, sweating cold sweat heavily, Qi ceased. Fang Wei from Henan transmitted this soup. Upon entering mouth, vomiting occurred, Qi restored. Method: One large spoonful of salt, roasted until yellow, one sheng of child's urine. Mix moderately warm and take. Soon vomiting/purging occurs, then cured. (*Bencao Tujing*) *Taiping Shenghui Fang* adds Shengjiang (Zingiberis Rhizoma Recens).
Emergency Formula for Gan Huoluan (Dry Cholera-Like Syndrome).
Decoct fresh Shengjiang (Zingiberis Rhizoma Recens) to make thick juice, add roasted salt, drop a few drops of Tongyou (Toona Sinensis Oil). Administer frequently, two to three bowls. Use goose feather to probe throat to induce vomiting. If no vomiting, urgently use Guiti San (Melon Seed Powder) to induce vomiting. If still no vomiting, use Beijji Wan (Emergency Rescue Pill) to purge. There is hope for survival. If delayed or lenient, death is imminent. (*Cangsheng Siming*) (Note: *Bianzheng Lu* treats Gan Huoluan (Dry Cholera-Like Syndrome) with one liang of Ren Shen (Ginseng Radix et Rhizoma) and seven Guiti (Momordicae Fructus), decocted in one bowl. The idea is clever, but during strong vomiting induced by Guiti (Melon Seed), how can Ren Shen (Ginseng Radix et Rhizoma) stabilize the Stomach? Better to tonify appropriately after vomiting. Therefore, not adopted here.)
Purgative Formulas
For Gan Huoluan (Dry Cholera-Like Syndrome) with blockage of stool and urine, extreme distress, urgently administer Badou Sanwei Wan (Three-Ingredient Pill with Croton Seed) to achieve rapid purgation.
Badou (Crotonis Fructus, one hundred pieces, roasted熟, skin and heart removed), Ganjiang (Zingiberis Rhizoma, three liang, Cui Shi replaced with five liang of Mangxiao (Natrii Sulfas), same as *Qianjin Fang*), Dahuang (Rhei Radix et Rhizoma, five liang).
First crush Ganjiang (Zingiberis Rhizoma) and Dahuang (Rhei Radix et Rhizoma) into powder. Separately crush Badou (Crotonis Fructus) into paste. Mix with the previous two powders. Add honey slowly to form pills. Take with water. Initial dose: three pills, size of wutong seeds. After taking, massage abdomen to promote rapid purgation. If no effect after a while, give hot drink. If still no effect, take one more pill. Purging should occur soon. After purging, rest well, adjust diet temperature as appropriate. If thirsty, sip thin millet gruel. Prescribed by Zhang Wenzhong. (*Waitai Miyao* citing Xu Renze) *Qianjin Yueling* Disheng Beijji Wan mainly treats Gan Huoluan (Dry Cholera-Like Syndrome), various heart/abdominal pains. (This is the same formula.)
