Huo Luan (Acute Vomiting and Diarrhea) arises suddenly with chaotic symptoms, characterized by severe pain in the heart and abdomen, accompanied by vomiting and diarrhea. If vomiting and diarrhea fail to occur, it is termed Gan Huo Luan (Dry Huo Luan), where the tongue curls up, tendons contract, and the genitals may retract into the abdomen, indicating a difficult-to-treat condition.
[Note] When one desires to vomit but cannot, and desires to defecate but cannot, accompanied by severe pain in the heart and abdomen, this condition is named Gan Huo Luan (Dry Huo Luan), also known as Jiao Chang Sha (Intestine-Turning Plague). If the tongue curls and tendons contract, causing the testicles or genitals to retract into the abdomen, it is considered refractory.
Huoxiang Zhengqi San (Agastache Powder to Rectify the Qi), Er Xiang Tang (Two Fragrances Decoction), Ganlu Yin (Sweet Dew Drink).
Huo Luan can be caused by mixed pathogenic factors such as Wind-Cold, Summerheat, Dampness, food stagnation, and water retention. The primary treatment involves regulating Zheng Qi (Upright Qi) using formulas like Huoxiang Zhengqi San. Huoxiang Zhengqi San consists of Huoxiang (Patchouli Herba), Zi Su Ye (Perilla Leaf), Chen Pi (Aged Tangerine Peel), Banxia (Pinellia Rhizoma), Fuling (Poria), Gancao (Licorice Radix et Rhizoma), Baizhi (Angelicae Dahuricae Radix), Jiegeng (Platycodonis Radix), Dafupi (Arecae Pericarpium), and Houpo (Magnoliae Officinalis Cortex); if muscle spasms (Zhuan Jin) occur, add Mugua (Chaenomelis Fructus) and Wuyu (Evodiae Fructus); for Summerheat pathogens, combine with Xiangru (Moslae Herba) to form Er Xiang Tang; for Dampness pathogens, add Cangzhu (Atractylodis Rhizoma); for severe Summerheat-Heat, use Chensha Liuyi San (Cinnabar Six-One Powder) or Wuling San (Five-Ingredient Powder with Poria) plus Shigao (Gypsum Fibrosum), Huashi (Talcum), and Han Shuishi (Calcitum), named Ganlu Yin; for extreme Cold with cold limbs and hidden pulse, use Pao Chuanwu (Prepared Aconiti Lateralis Radix) and Pao Chuanfu (Prepared Aconiti Radix) combined with Lizhong Tang (Regulate the Middle Decoction).
[Note] Huo Luan disease results from the mixing of pathogenic factors such as Wind-Cold, Summerheat, Dampness, food, and water, which disrupt the intestines and stomach, causing turbid and clear qi to interfere with each other, leading to severe abdominal pain, vomiting, and diarrhea. Huoxiang Zhengqi San comprises Huoxiang, Ziyeye, Chenpi, Banxia, Fuling, Gancao, Baizhi, Jiegeng, Dafupi, and Houpo. For Summerheat causing predominant vomiting, combining with Xiangru Yin forms Er Xiang Tang. For Dampness causing predominant diarrhea, add Cangzhu. For severe Summerheat-Heat, use Chensha Liuyi San, or Wuling San added with Shigao, Huashi, and Hanshuishi, named Ganlu Yin. For extreme Cold with cold limbs and hidden pulse, use Pao Chuanwu and Pao Chuanfu combined with Lizhong Tang.
San Yang (Three Yang) Heat accumulation injures Body Fluids (Jin Ye), leading to dryness and obstruction of Benmen (Cardiac Orifice) and Youmen (Pyloric Orifice). If Benmen fails to receive food, it is called Ye Ge (Dysphagia); if Youmen fails to release food, it becomes Fan Wei (Regurgitation). These two conditions persist over time, narrowing the transmission pathways, causing Po Men (Anal Orifice) to become dry and difficult to pass stool. Chest pain with hard stools resembling sheep feces, along with spitting saliva and hematemesis, indicates a fatal prognosis.
[Note] San Yang Heat accumulation refers to the heat pathogens in the Stomach, Small Intestine, and Large Intestine accumulating without dispersal, burning and injuring Body Fluids. The upper opening of the Stomach is Benmen, the upper opening of the Small Intestine is Youmen, and the lower opening of the Large Intestine is Po Men. Once the Body Fluids of these three fu organs are damaged, the three orifices naturally become dry, obstructing the passage of water and grain. Dryness of Benmen narrows the pathway for receiving food and drink, so food cannot descend, resulting in Ye Sai (Obstruction/Dysphagia). Dryness of Youmen narrows the pathway for expelling digested food, so food enters but is expelled back out, resulting in Fan Wei (Regurgitation). After these conditions persist for a long time, the transmission path of the Large Intestine narrows, so Po Men naturally becomes dry and difficult to pass stool. Chest pain like stabbing indicates injury to the Epigastrium. Stools resembling sheep feces indicate exhaustion of Body Fluids. Spitting saliva and hematemesis indicate stagnation of Blood flow; all are signs of death.
Renshen Lige Wan (Ginseng Throat-Opening Pill), Gongsu San (Mercury-Sulfur Powder).
Wuzhi Yin (Five Juices Drink) and Dahuang Tang (Rhubarb Decoction) clear Dryness-Heat; Dingxiang (Caryophylli Flos) and Chenxiang (Aquilariae Lignum Resinatum) combined with Sijunzi Tang (Four Gentlemen Decoction), Liujunzi Tang (Six Gentlemen Decoction), or Lizhong Tang treat Deficiency-Cold. For constipation and obstruction, Lige Wan should be used; for incessant vomiting, Gongsu San is administered first. Lige Wan is based on Xiaochengqi Tang (Minor Order the Left Decoction) plus Renshen (Ginseng Radix et Rhizoma), Gancao, Danggui (Angelicae Sinensis Radix), Huoxiang, Binglang (Arecae Semen), Taoren (Persicae Semen), Huomaren (Cannabis Semen), formed into pills with honey. Gongsu San consists of one part Mercury and two parts Sulfur, ground into ink-like powder, taken with aged wine and Ginger Juice (Shengjiang Zhi) for immediate relief.
[Note] "Wuzhi" refers to Wuzhi Yin, used to clear Dryness and dehydration. "Dahuang" refers to Dahuang Tang, consisting solely of Dahuang (Rhei Radix et Rhizoma), prepared by frying slices with Ginger Juice until black-yellow, dosage adjusted to patient strength (2-3 qian per dose), added with a pinch of aged rice and two stalks of Scallion White (Congbai), decocted and strained for treating Heat accumulation. Adding Dingxiang and Chenxiang to Sijunzi Tang, Liujunzi Tang, or Lizhong Tang treats Deficiency-Cold. "Lige" refers to Lige Wan, composed of Zhike (Aurantii Fructus), Houpo, Dahuang, Renshen, Gancao, Muxiang (Aucklandiae Radix), Danggui, Huoxiang, Binglang, Taoren, and Huomaren, formed into pills with honey. "Gongsu" refers to Gongsu San.
Sijunzi Tang (Four Gentlemen Decoction), Siwu Tang (Four Substances Decoction), Erchen Tang (Two Aged Decoction), Ershisiwei Liuqi Yin (Twenty-Four Ingredients Flowing Qi Drink).
For Qi deficiency and Blood depletion, Sijunzi Tang and Siwu Tang are appropriate; for excessive Phlegm and Qi stagnation, Erchen Tang and Ershisiwei Liuqi Yin are appropriate. Other treatments follow those for Vomiting. Exhausting one's efforts in planning can only achieve human diligence while awaiting the will of Heaven.
[Note] Qi deficiency (Qi Shao) is suitable for Sijunzi Tang; Blood depletion (Xue Ku) is suitable for Siwu Tang; excessive Phlegm is suitable for Erchen Tang; Qi stagnation is suitable for Ershisiwei Liuqi Yin. The remaining treatment methods are the same as for Vomiting. Although this disease requires exhausting one's mind and planning, it ultimately amounts to doing one's utmost while awaiting destiny.
