Wind is the leader of all diseases. When it strikes the face, the pathogen descends to the Yangming channel; if severe, it enters the Spleen. When it strikes the nape, the pathogen descends to the Taiyang channel; if severe, it enters the Kidney. When it strikes the side, the pathogen descends to the Shaoyang channel; if severe, it enters the Liver. The pathological changes are diverse; one must be cautious and not neglect this.
In wind disease, presenting with headache, excessive sweating, aversion to wind, pain in the axilla, inability to turn the body, and a floating, wiry, and rapid pulse, this indicates Wind Pathogen invading the Liver. It should be treated with Xiao Chai Hu Tang (Minor Bupleurum Decoction). Chapter 12: Treatment of Cold Disease Pulse and Symptoms
When cold pathogen causes disease, the Kidney is affected first. If it lodges among the Five Zang Organs, it causes pulling pain along the meridians. If it lodges in the "Eight Emptinesses" (axillae, groins, etc.), evil blood stagnates. Accumulating over time without resolving, it transforms into stubborn disease. Can one not be cautious?
In cold disease, presenting with bone pain, Yin Bi (bi-syndrome caused by cold pathogen obstructing meridians), abdominal distension, lower back pain, difficult defecation, pulling pain in the shoulder, back, neck, and nape, and a deep, slow pulse, this indicates Cold Pathogen invading the Kidney. It should be treated with Gui Zhi Jia Ge Gen Tang (Guizhi Decoction plus Kudzu Root). Treatise on Cold Damage and Miscellaneous Diseases, Volume Six
Differentiation of Taiyang Disease Pulse, Symptoms, and Treatment - Upper
The characteristic of Taiyang disease is a floating pulse, stiffness and pain in the head and nape, and aversion to cold.
Taiyang disease, presenting with fever, sweating, aversion to wind, and a moderate pulse, is named Wind Strike (Zhong Feng).
Taiyang disease, whether fever has already appeared or not, always includes aversion to cold, body pain, vomiting and regurgitation, and a tight pulse in all positions (Yin and Yang), is named Cold Damage (Shang Han).
On the first day of Cold Damage, the Taiyang channel is affected. If the pulse remains quiet, the disease has not transmitted. If there is a strong desire to vomit, or restlessness, and the pulse is rapid and urgent, this indicates transmission.
On the second or third day of Cold Damage, if Yangming and Shaoyang symptoms do not appear, this indicates no transmission.
Taiyang disease, presenting with fever and thirst but no aversion to cold, is Warm Disease (Wen Bing). If after sweating, the body becomes burning hot, it is named Wind-Warmth (Feng Wen). In Wind-Warmth disease, the pulse is floating in all positions, there is spontaneous sweating, heaviness of the body, excessive sleepiness, snoring in nasal breathing, and difficulty speaking. If purgatives are wrongly used, there will be difficulty in urination, staring gaze, and incontinence. If fire therapy is wrongly used, the complexion may turn slightly yellow; in severe cases, it resembles convulsive epilepsy with intermittent spasms. If smoke fumigation is used, one error may prolong the illness, but a second error will hasten death.
Diseases with fever and aversion to cold originate from Yang. Diseases without fever but with aversion to cold originate from Yin. Those originating from Yang heal in seven days; those originating from Yin heal in six days, because Yang corresponds to the number seven and Yin to the number six.
In Taiyang disease, if headache persists for more than seven days and heals spontaneously, it is because the pathogen has completed its course through the channel. If it is about to transmit to the next channel, acupuncture at the Foot-Yangming channel can prevent transmission and cure the disease.
The time when Taiyang disease tends to resolve is from Si hour to Wei hour.
For patients with wind disease, if the exterior syndrome is resolved but the patient still feels unwell, they will heal in twelve days.
If a patient has high body heat but desires to wear clothes, the heat is in the skin/surface, and the cold is in the bone/marrow (deep interior).
If a patient has low body temperature but does not desire to cover up, the cold is in the skin/surface, and the heat is in the bone/marrow (deep interior).
In Taiyang Wind Strike, the pulse is floating on the surface (Yang) and weak on the inside (Yin). Floating Yang indicates spontaneous heat generation; weak Yin indicates spontaneous sweating. With shivering aversion to cold, chilliness from wind, mild fever, nasal congestion (ming), and dry vomiting, Gui Zhi Tang (Cinnamon Twig Decoction) should be administered.
Gui Zhi Tang Formula
Gui Zhi (Ramulus Cinnamomi) three liang (bark removed), Shao Yao (Radix Paeoniae Alba) three liang, Gan Cao (Radix Glycyrrhizae) two liang (roasted), Sheng Jiang (Rhizoma Zingiberis Recens) three liang (sliced), Da Zao (Fructus Jujubae) twelve pieces (split).
The above five ingredients are crushed. Boil in seven sheng of water over low heat to obtain three sheng. Remove the dregs. Take one sheng at a comfortable temperature. Shortly after taking, drink more than one sheng of hot thin porridge to assist the medicinal power. Cover with blankets for about an hour until a slight, continuous sweat appears over the whole body. This is best. Do not allow profuse sweating like flowing water, or the disease will not be eliminated. If sweating occurs after the first dose and the disease improves, stop subsequent doses; there is no need to finish the entire decoction. If no sweating occurs, administer again according to the previous method. If still no sweating, shorten the interval between doses, finishing three doses within half a day. For severe cases, administer day and night, observing for twenty-four hours. If symptoms persist after finishing one dose, prepare another. If no sweating occurs, continue to two or three doses. Contraindicated foods include raw/cold, sticky/smooth, meat, wheat products, five pungent spices, dairy/wine, and foul-smelling items.
Taiyang disease, presenting with headache, fever, sweating, and aversion to wind, is treated with Gui Zhi Tang (Cinnamon Twig Decoction). (Formula seen previously).
Taiyang disease, presenting with stiffness and discomfort in the nape and back, as well as sweating and aversion to wind, is treated with Gui Zhi Jia Ge Gen Tang (Guizhi Decoction plus Kudzu Root).
Gui Zhi Jia Ge Gen Tang Formula

The above six ingredients are first boiled in one dou of water. Boil the Ge Gen until the volume reduces by two sheng, removing the foam on top. Add the other herbs. Boil to obtain three sheng. Remove the dregs. Take one sheng warm. Cover to induce a slight sweat. No hot porridge is needed. Nursing care and contraindications follow the Guizhi method.
After Taiyang disease is incorrectly treated with purgation, if the Qi rushes upward, Gui Zhi Tang (Cinnamon Twig Decoction) may be given using the previous method. If the Qi does not rush upward, do not give it.
If Taiyang disease has lasted three days, and after sweating, emesis, purgation, or warm needle therapy, the condition remains unresolved, this is a "Bad Disease" (Huai Bing). Gui Zhi Tang should not be given. Observe the pulse and symptoms, determine what error was made, and treat according to the current syndrome.
Gui Zhi Tang (Cinnamon Twig Decoction) is originally for releasing the muscle layer. If the patient has a floating-tight pulse, fever, and no sweating, it must not be given. Keep this in mind to avoid errors. Patients who consume alcohol regularly should also not receive Gui Zhi Tang, as it will likely cause vomiting due to their dislike of sweet substances.
For patients with a chronic history of asthma, adding Hou Po (Magnolia Bark) and Xing Ren (Apricot Seed) to Gui Zhi Tang is beneficial.
Anyone who vomits after taking Gui Zhi Tang will subsequently vomit pus and blood.
After Taiyang disease is treated with sweating, if sweating continues uncontrollably, the patient feels aversion to wind, has difficulty urinating, slight stiffness in the four limbs making flexion and extension difficult, Gui Zhi Jia Fu Zi Tang (Guizhi Decoction plus Prepared Aconite Root) should be administered.

After Taiyang disease has lasted eight or nine days, presenting like malaria with fever and aversion to cold, more heat than cold, no vomiting, normal bowel and bladder function, occurring two or three times a day, and a slightly slow pulse, recovery is imminent. If the pulse is slightly weak with aversion to cold, this indicates deficiency of both Yin and Yang; do not use sweating, emesis, or purgation. If the face shows redness, the disease has not resolved because a slight sweat could not be achieved, leading to body itching. Gui Zhi Ma Huang Ge Ban Tang (Half-Half Decoction of Guizhi and Mahuang) is suitable.
Gui Zhi Ma Huang Ge Ban Tang Formula (Mahuang Tang seen in later volumes)
Mix three he of Gui Zhi Tang and three he of Ma Huang Tang to make six he. Take all at once. Nursing care follows the Gui Zhi Tang method.
In Taiyang disease, if after initially taking Gui Zhi Tang, vexation worsens and does not resolve, first puncture Fengfu (GV16) and Fengchi (GB20), then administer Gui Zhi Tang (Cinnamon Twig Decoction).
After Taiyang disease takes Gui Zhi Tang, if profuse sweating occurs and the pulse becomes surging (Hong Da), administer Bai Hu Tang (White Tiger Decoction). If the presentation resembles malaria, occurring twice a day, Gui Zhi Er Ma Huang Yi Tang (Two Parts Guizhi and One Part Mahuang Decoction) is suitable.

In Cold Damage, presenting with a floating pulse, spontaneous sweating, frequent urination, irritability, slight aversion to cold, and cramping in the feet, administering Gui Zhi Tang to attack the exterior is an error. This leads to cold extremities, dry throat, irritability, and vomiting. Prepare Gan Cao Gan Jiang Tang (Licorice and Dried Ginger Decoction) to restore Yang. Once the cold extremities resolve and feet warm up, prepare Shao Yao Gan Cao Tang (Peony and Licorice Decoction) to extend the legs. If stomach Qi is disharmonious causing delirium, administer Tiao Wei Cheng Qi Tang (Regulate the Stomach and Direct Downward Decoction) in small amounts. If further sweating and moxibustion/needle burning were wrongly applied, Si Ni Tang (Four Reversals Decoction) should be administered.

Question: In Taiyang disease, with complete symptoms, why does the condition worsen with Gui Zhi treatment, leading to cold extremities, dry throat, irritability, vomiting, and delirium? Master says: This is Yang Dan Syndrome, which cannot be attacked. The Cun position pulse is floating; floating indicates Wind and Deficiency. Wind generates heat, and deficiency causes cramping. Wrongly attacking the exterior causes sweating and loss of Yang. Excessive sweating depletes fluids, fluid depletion causes tendon cramping, Yangming internal binding causes irritability and delirium. Use Gan Cao Gan Jiang Tang to restore Yang, Shao Yao Gan Cao Tang to save fluids, and Tiao Wei Cheng Qi Tang to stop delirium. This illustrates that treating Bad Disease requires following the pulse and symptoms.
Yang Dan Syndrome, presenting with fever without tidal fever, sweating, dry throat, restless sleep, and calmness at midnight, should be treated with Di Huang Ban Xia Mu Li Suan Zao Ren Tang (Rehmannia, Pinellia, Oyster Shell, and Sour Jujube Seed Decoction). If there is thirst, irritability, red urine, and delirium, Zhu Ye Shi Gao Huang Qin Ze Xie Ban Xia Gan Cao Tang (Bamboo Leaf, Gypsum, Scutellaria, Alisma, Pinellia, and Licorice Decoction) should be administered.
