A patient presented with distension and oppression in both hypochondriac regions, a desire to cough but inability to do so, a metallic taste of blood in the mouth, distending pain throughout the body especially in the buttocks and thighs, fatigue, anorexia, irritability, thirst, and a large pulse. This is irritability due to blood loss (Xue Tuo Fan Zao). He was administered child's urine mixed with wine, and scarification (Bian) was performed on the affected areas, expelling a significant amount of dead blood and necrotic tissue. Suddenly, he developed fever, irritability, and sweating. A single-ingredient Ginseng Decoction (Dushen Tang) was given for three doses, which slightly alleviated the symptoms. Subsequently, several doses of herbs to tonify Qi and Blood and clear Liver Fire were used, leading to slight improvement in appetite. Later, the Single Ginseng Decoction was taken intermittently, all symptoms completely subsided, and appetite increased sharply. However, he could not sleep much. Guipi Tang (Restore the Spleen Decoction) was modified with Gardenia Fruit (Shanzhi) and Bamboo Shavings (Zhuru), and after four doses, he slept soundly. Due to mental overexertion, he experienced irritability, thirst, and spontaneous sweating with a large but forceless pulse. Danggui Buxue Tang (Angelica Sinensis Decoction for Tonifying Blood) was administered for two doses, stabilizing the condition. Then, Shiquan Dabu Tang (Ten Ingredients Great Supplementing Decoction) without Chuanxiong Rhizome (Chuanxiong) was used, adding Ophiopogon Tuber (Maimen), Schisandra Berry (Wuweizi), Moutan Bark (Mudanpi), Phellodendron Bark (Digupi), Ephedra Root (Mahuanggen), and fried wheat grains (Chao Fumai). After several doses, sweating stopped, necrotic tissue sloughed off, and after more than twenty doses, new granulation tissue grew.
Another patient presented with irritability, flushed face, dry mouth with thirst, and a flooding pulse that felt empty upon deep pressure. I stated: "This is irritability due to Blood deficiency." Danggui Buxue Tang (Angelica Sinensis Decoction for Tonifying Blood) was prescribed for two doses, and the symptoms ceased immediately. Later, he developed fever in the late afternoon. Siwu Tang (Four Substance Decoction) was modified with Bupleurum Root (Chaihu), Moutan Bark (Mudanpi), Phellodendron Bark (Digupi), Phellodendron Bark (Huangbai), and Anemarrhena Rhizome (Zhimu). The fever subsided, and the wound healed. Li Dongyuan said: "Fever with aversion to cold, extreme unquenchable thirst, and a large but forceless pulse are not indications for Baihu Tang (White Tiger Decoction); this is irritability due to Blood deficiency, and Danggui Buxue Tang should be used." Mr. Pei said: "Skin heat with restlessness, red eyes, and flushed face, accompanied by a flooding yet deficient pulse, indicate Blood deficiency. If mistakenly treated with Baihu Tang (White Tiger Decoction), it may lead to danger or even death."
A third patient presented with sweating on the forehead, fever, thirst, shortness of breath, irritability, bone pain, and non-sloughing necrotic tissue. The necrotic tissue was surgically removed, revealing fresh bleeding. Administration of Scutellaria Root (Huangqin) and Coptis Rhizome (Huanglian) worsened the condition. The pulse was flooding yet faint. This was caused by simultaneous deficiency of Qi and Blood with flourishing pathogenic fire. Siwu Tang (Four Substance Decoction) was modified with Ginseng Radix et Rhizoma (Renshen), [missing character in original text, likely Poria], Atractylodes Macrocephala Rhizome (Baizhu), and prepared Licorice Root (Zhicao), with small amounts of Bupleurum Root (Chaihu) and fried Scutellaria Root (Chao Huangqin). After two doses, head sweating stopped abruptly. Then, Ophiopogon Tuber (Maimen), Schisandra Berry (Wuweizi), and Cinnamon Twig (Rougui) were added. After two doses, all symptoms subsided. Later, Ginseng Radix et Rhizoma (Renshen), [same missing herb], Angelica Sinensis Radix (Danggui), Atractylodes Macrocephala Rhizome (Baizhu), fried White Peony Root (Chao Shaoyao), Prepared Rehmannia Root (Shudi), Ophiopogon Tuber (Maimen), and Schisandra Berry (Wuweizi) were used for over ten doses. The necrotic tissue sloughed off, and the pus became thick. However, new flesh did not grow. By doubling the dose of Atractylodes Macrocephala Rhizome (Baizhu) in the previous formula, the wound healed.
