One bone is called the *Bigu* (femur), with its upper end resembling a pestle, inserting into the acetabulum of the *Bishu* (hip joint), and its lower end resembling a hammer head, connecting to the tibia. Collectively named *Gu* (thigh), it is the general term for the two major supporting bones of the lower body, commonly known as the thigh bone. In cases of falling from a horse or twisting injuries causing bone fracture and tendon swelling, where the area turns dark purple and feels cold, and white blisters appear externally, this is due to bone fragmentation and leakage of Qi. Such conditions are rarely curable. However, if the patient is young with abundant Qi and Blood, although there is physical swelling and pain without mental confusion or white blisters, it is treatable. The method involves massaging the fractured bones with both hands and manipulating them back into position. Then, press the injured area with fingers. For bones that have not dislocated, wrap them in a bamboo splint. Administer *Zhenggu Zijin Dan* orally every morning. After three days, open the splint to inspect; if there are irregularities, knead the tendon knots to smooth them out, apply *Wanling Gao*, and re-wrap with the bamboo splint.
The patella, also known as *Lianhai* or *Bin Gu*, is round and flat, covering the ends of the upper and lower segments of the *Jian Gu* (femur). Its inner surface has tendons attached. These tendons pass upward over the thigh to the flanks, and downward over the tibia to the dorsum of the foot. If subjected to traumatic injury and the patella shifts upward, the tendons become swollen, affecting the tendons within the popliteal fossa (*Nao*). Since these popliteal tendons connect upward to the waist and hips, symptoms often include pain upon bending the waist. If the patella shifts downward, the popliteal fossa swells, or the calf and abdomen become cold and hard, resulting in dragging the foot and walking obliquely when moving. If the patella dislocates laterally outward, the medial tendons swell; if medially inward, the tendons straighten and the popliteal fossa swells. One should carefully observe the direction of the bony displacement, manipulate according to technique to restore alignment. Internally administer *Bu Jin Wan*, externally apply *Ding Tong San* for moxibustion and warming, and fumigate with *Baxian Xiaoyao Tang* for recovery.
The tibia, located below the knee and above the ankle, is commonly known as the *Jing Gu*. This region consists of two bones: the anterior one is called *Cheng Gu* or *Jing Gu*, which is thick; the posterior one is called *Fu Gu*, which is thin, and is also commonly known as *Laotang Gu*. If injured by trauma, with the sharp end of the bone protruding outward through the skin, causing unceasing bleeding from ruptured flesh, fine groaning from pain, and reduced food intake, especially if the person has inherent deficiency of Qi and Blood, it will inevitably lead to critical danger or death. Manual therapy should be used to press the tendons and correct the bone to restore position. Apply *Wanling Gao*, wrap with a bamboo splint, and secure with white cloth. First, administer *Zhenggu Zijin Dan*, followed by *Jianbu Huqian Wan*.
