The *Daquan* states: Various types of lin syndrome (strangury) occurring postpartum are caused by heat pathogens invading the bladder. Internal deficiency leads to frequent urination, while heat causes painful and difficult urination, hence it is called lin syndrome. There are also cases where qi deficiency combined with heat pathogens causes the heat to contend with blood, infiltrating the uterus, with blood exiting via urine, known as bloody lin syndrome. (While heat contending with blood exists, infiltration into the uterus is questionable; for instance, in jiaochang syndrome [bowel-bladder misdirection], is it not also due to infiltration?)
The *Sanyin Lun* states: The treatment methods for various lin syndromes before and after childbirth differ. Before childbirth, one should stabilize the fetus; after childbirth, one should eliminate blood stasis. For cold lin, hot lin, gao lin (mucous strangury), shi lin (calculus strangury), and qi lin (gas strangury), the general treatment principles are the same, but one must assess excess or deficiency to apply appropriate therapy. Qumai (Herba Dianthi) and Puhuang (Pollen Typhae) are the most important herbs for postpartum use. By investigating the etiology, one will not miss the key points of treatment. (Treating all five types of lin syndrome uniformly is contradictory; if so, cold and heat would not be distinguished.)
Xue Ji stated: Regarding the aforementioned symptoms, if there is deficient-heat in the bladder, use Liuwei Wan (Six-Ingredient Rehmannia Pill). If there is yin deficiency leading to an inability of yang to transform fluids, use Zishen Wan (Kidney-Nourishing Pill). Since Earth generates Metal, and Metal generates Water, one should nourish the source of transformation, while also referring to the "Frequent and Dribbling Urination" section for regulation.
A postpartum patient suffered from dribbling urination and occasional spontaneous leakage. After two years of unsuccessful treatment with diuretic and fire-clearing agents, I diagnosed it as deficiency of lung and kidney qi. She recovered using Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) combined with Liuwadihuang Wan (Six-Ingredient Pill with Rehmannia). (The previous use of diuretic and fire-clearing agents was excessive, necessitating this tonic approach.)
Maogen Tang (Imperata Rhizome Decoction): Treats various postpartum lin syndromes, regardless of cold, heat, gao, shi, qi, or xue lin. (This formula is very effective.)
Baimaogen (Rhizoma Imperatae, 5 qian), Qumai Sui (Spica Dianthi), Baifuling (Poria, 2.5 qian each), Kuizi (Fructus Abutilonis), Renshen (Radix Ginseng, 1.25 qian each), Puhuang (Pollen Typhae), Taojiao (Arbor Vitae Gum), Huashi (Talcum), Gancao (Radix Glycyrrhizae, 0.6 qian each), Zi Bei (Concha Arcae, one piece, ground), Shishouyu Naosha (Otolithus Otolithus, two pieces, ground). (Zi Bei and Shishouyu Naosha are key herbs for treating shi lin.)
Decoct these ingredients, divided into two doses, with three slices of fresh ginger and twenty stalks of Dengxin Cao (Caulis Junci). Take orally after decocting with water. Alternatively, grind into powder, take 2 qian per dose, washed down with a decoction of Mutong (Caulis Akebiae). If there is qi stagnation and obstruction, wash down with a decoction of Mutong and Ju Pi (Pericarpium Citri Reticulatae).
Huashi San (Talcum Powder): Treats postpartum hot lin syndrome.
Huashi (Talcum, 1.25 liang, ground), Tongcao (Medulla Tetrapanacis), Cheqianzi (Semen Plantaginis), Kuizi (Fructus Abutilonis, 1 liang each).
Grind into powder. Mix with Jiangshui (fermented grain liquid) and take one fangcun bi (approx. 5g) per dose; taking two fangcun bi is optimal.
Huashi Tonglin San (Talcum Unblocking Lin Decoction): Used when postpartum urination is tight, astringent, and obstructed. This condition arises from blood-heat accumulating in the small intestine, irregular menstruation, and indulgence in toxic-hot foods, resulting in lin-strangury.
Chifuling (Red Poria), Zexie (Rhizoma Alismatis), Mutong (Caulis Akebiae), Huanglian (Rhizoma Coptidis), Zhuling (Polyporus, 0.8 qian each), Baizhu (Rhizoma Atractylodis Macrocephalae), Qumai (Herba Dianthi), Shanzhizi (Fructus Gardeniae), Cheqianzi (Semen Plantaginis, equal parts), Huashi (Talcum, 0.4 qian).
Decoct with twelve stalks of Dengxin Cao. Take orally warm on an empty stomach.
Shiwei San (Pyrosma Leaf Powder): Treats sudden urinary strangury in women.
Shiwei (Folium Pyrrosiae), Huangqin (Radix Scutellariae), Mutong (Caulis Akebiae), Yu Bai Pi (Cortex Ulmi), Kuizi (Fructus Abutilonis), Qumai Sui (Spica Dianthi), Gancao (Radix Glycyrrhizae, equal parts).
Cut into pieces. Take 8 qian per dose with three slices of fresh ginger. Decoct with water and take warm before meals.
Jiawei Siwu Tang (Modified Four Substances Decoction): All lin syndromes belong to heat; this formula has proven effective repeatedly.
Danggui (Radix Angelicae Sinensis), Chuanxiong (Rhizoma Chuanxiong), Chi Shaoyao (Radix Paeoniae Rubra), Sheng Dihuang (Radix Rehmanniae Preparata), Gancao Shao (Apex Radicis Glycyrrhizae), Dunniumxi (Radix Achyranthis Bidentatae), Mutong (Caulis Akebiae, 1 qian each), Taoren (Semen Persicae, peeled and cored, five seeds), Huashi (Talcum, 1.5 qian), Muxiang (Radix Aucklandiae).
Decoct with water and take orally.
Another Prescription: Treats postpartum lin syndrome, painful urination, and bloody lin syndrome.
Baimaogen (Rhizoma Imperatae, 0.5 qian), Qumai (Herba Dianthi), Cheqianzi (Semen Plantaginis, 2 liang each), Liyu Chi (Cyprinus Carpio Teeth, 100 pieces, ground), Tongcao (Medulla Tetrapanacis, 3 liang), Dongkuizi (Fructus Abutilonis, 0.3 qian).
Boil in three sheng of water until one sheng remains. Add the fish tooth powder and take on an empty stomach.
Zhang's Formula: Treats postpartum urinary retention and strangury.
Chenpi (Pericarpium Citri Reticulatae, 1 liang, white pith removed), ground into powder. Take 2 qian with warm wine on an empty stomach. Urination resumes immediately after one dose. (The small intestine governs qi pathways, controlled by the liver. Chenpi and Xingren are both qi-moving herbs, hence their efficacy.)
Prescription for Sudden Urinary Retention
Xingren (Semen Pruni Armeniacae, 14 seeds, peeled and cored), stir-fried and ground into powder. Mix with drink and consume in one dose.
Moxibustion at the Navel Method: Treats postpartum urinary retention, abdominal distension like a drum, and confusion/unconsciousness. This is likely due to internal accumulation of cold pathogens prior to childbirth, causing improper movement of the bladder during delivery. (Applicable for accumulated cold; as stated, "when qi transforms, urination occurs.")
Fill the navel with salt. Peel the coarse skin off more than ten stalks of Congbai (White Part of Allii Fistulosi) and bind them together. Cut to a thickness of one finger and place on the salt. Place a large moxa cone on top of the green onion cake and ignite. When heat sensation enters the abdomen, urination will resume immediately. This is highly effective.
